Low anaerobic threshold and increased skeletal muscle lactate production in subjects with Huntington's disease.

Mitochondrial defects that affect cellular energy metabolism have long been implicated in the etiology of Huntington's disease (HD). Indeed, … normal in HD subjects but notably the HD patients and presymptomatic mutation carriers had a lower anaerobic threshold than the control subjects. The low anaerobic threshold of HD patients was associated with an increase in the concentration of plasma lactate. We also analyzed in vitro muscular cell cultures and found that HD cells produce more lactate than the cell …

Limited value of anaerobic threshold for assessing functional capacity in patients with heart failure.

Exercise tolerance was assessed in 146 patients with cardiac dysfunction in terms of anaerobic threshold (ATge). Patients were divided into four classes according to the peak oxygen uptake: Class A (72 patients) exceeding 1000 ml/min; Class B (27 patients) 800-999 ml/min; Class C (37 patients) 500-799 ml/min; and Class D (10 patients) below 500 ml/min. An incidence of the ATge breakpoint was lower in patients of Class C (38%) than in those of either Class B (70%, p < 0.05) or Class A (87%, p < 0.05). The ATge could not be determined in any patients in Class D. The V-slope method improved the a…

The relationship of plasma catecholamine and lactate during anaerobic threshold exercise in mitochondrial myopathies.

Sympathetic system activation is considered one of the main factors influencing lactate production during exercise in normal individuals. In order to assess the role of such activation in mitochondrial myopathies, we compared blood catecholamine levels to those of lactate during an intermittent exercise performed at workloads near anaerobic lactate threshold. Following an initial increment, the patients (n = 10) exhibited a steady-state blood lactate shifted right relative to controls (n = 7), the peaks being respectively 665 +/- 29% and 322 +/- 11% of baseline. Plasma catecholamine increase i

Controversies in the physiological basis of the 'anaerobic threshold' and their implications for clinical cardiopulmonary exercise testing.

This article reviews the notion of the 'anaerobic threshold' in the context of cardiopulmonary exercise testing. Primarily, this is a review of the proposed mechanisms underlying the ventilatory and lactate response to incremental exercise, which is important to the clinical interpretation of an exercise test. Since such tests are often conducted for risk stratification before major surgery, a failure to locate or justify the existence of an anaerobic threshold will have some implications for clinical practice. We also consider alternative endpoints within the exercise response that might be b…

[Effects of the exercise intensities standardized by anaerobic threshold on EEG and heart rate variability].

The purpose of this study was to examine the effects of different exercise intensities with the anaerobic threshold (AT) as the standard on electroencephalograph (EEG) and heart rate variability (HRV). Eleven healthy males, with a mean age of 22 (SD 1.48) years, performed submaximal exercise to determine their ATs, and underwent four experimental conditions including rest (rest), 20% less than the AT level (-20), the AT level (AT), and 20% more than the AT level (+20) for about 20 minutes. EEG and electrocardiogram (ECG) were taken for 15 minutes before and after each experimental condition, r…

Determination of anaerobic threshold for assessment of functional state in patients with chronic heart failure.

The use of anaerobic threshold in assessment of aerobic capacity was evaluated in 34 normal subjects and 47 patients with various kinds of chronic heart disease. Anaerobic threshold was determined as the oxygen consumption (VO2) at which a linear relationship between pulmonary ventilation (VE) and VO2 was lost during progressive treadmill exercise. Anaerobic threshold determined in this manner was validated with that determined by blood lactate measurements in eight normal subjects and nine cardiac patients (r = .962, p less than .001). Thereafter, anaerobic threshold was determined only by re…

Nonexercise prediction equation for oxygen uptake at anaerobic threshold in Chinese patients with cardiovascular disease.

BACKGROUND: Oxygen uptake at anaerobic threshold (VO2AT) is an important indicator for guiding cardiovascular disease (CVD) rehabilitation training and evaluating cardiac function, which varies according to race and specific disease conditions. However, there is no well-established nonexercise prediction equation for VO2AT. OBJECTIVES: This study aims to develop a nonexercise predictive equation for VO2AT that is applicable to Chinese patients with CVD. METHODS: A cohort was established with 22,336 cases of cardiopulmonary exercise testing data from CVD patients across 20 hospitals in China.…

Anaerobic threshold assessment through the ventilatory method during roller-ski skating testing: right or wrong?

This study aimed at questioning the validity of the ventilatory method to determine the anaerobic threshold (respiratory compensation point [RCP]) during an incremental roller-ski skating test to exhaustion. Nine elite crosscountry skiers were evaluated. The skiers carried out an incremental roller-ski test on a treadmill with the V2 skating technique. Ventilatory parameters were continuously collected breath by breath, thanks to a portable gas exchange measurement system. Poling signal was obtained using instrumented ski poles. For each stage, ventilatory and poling signals were synchronized …

How consistent is the determination of the anaerobic threshold among members of the same cardiac rehabilitation team?

INTRODUCTION: Consistency in the anaerobic threshold (AT) determination is essential for cardiac rehabilitation teams comprising multiple professionals who conduct cardiopulmonary exercise tests (CPETs) and interpret their results. The effect of years of experience on interpreting CPET results, including the AT, has been examined. However, consistency in the AT determination among members of the same institution with experience beyond the training period has not been reported. METHODS: We evaluated consistency in the AT determination among four members at a cardiovascular hospital (three full…

The relationship between anaerobic lactate threshold and plasma catecholamines during incremental exercise in hereditary spastic paraplegia.

Lower limb muscle chronic hyperactivity in hereditary spastic paraplegia (HSP) is the consequence of motor corticospinal tract involvement, which in turn has been hypothesized to be of mitochondrial origin. In order to assess skeletal muscle aerobic metabolism and sympathetic response during exercise in 10 HSP patients, we evaluated their blood lactate and catecholamine levels during an incremental workload bicycle exercise. Lactate, but not epinephrine or norepinephrine, levels were significantly higher in the HSP patients than in control subjects, in both resting conditions and during exerci

Effects of Heart Rate-Synchronized Breathing on Anaerobic Threshold During Incremental Exercise Testing: A Preliminary Case Series.

Heart rate-synchronized breathing (HRSB) -- a technique in which the respiratory cycle is entrained to the cardiac cycle -- may optimize autonomic nervous function and ventilatory efficiency, potentially improving exercise tolerance. However, its effect on the anaerobic threshold (AT) remains unclear. This study examined the influence of HRSB on AT and related physiological indices during incremental cycle ergometry in healthy young adults. Three healthy male adults (mean age: 22.3 ± 1.2 years) underwent two cardiopulmonary exercise tests on separate days: Day 1 (normal breathing, control) and…

Estimation of anaerobic threshold and peak oxygen uptake from tracheal sound during cycle ergometer cardiopulmonary exercise test.

Cardiopulmonary exercise test (CPET) is widely used for assessing exercise capacity of individuals through ventilatory anaerobic threshold (AT) and peak oxygen uptake (VO2peak). However, CPET requires specialized equipment, resources, time, and trained personnel. In this study, we investigated the feasibility of estimating ventilatory AT and submaximal VO2peak using tracheal respiratory sounds recording with a wearable sensor (The Patch) during incremental cycle ergometer testing. Twenty-four healthy adults (18 females, age: 31.0 ± 9.1 years, body mass index: 23.5 ± 3.3 kg/m2) underwent CPET w…

Venous saturation and the anaerobic threshold in neonates after the Norwood procedure for hypoplastic left heart syndrome.

BACKGROUND: Reduction in oxygen delivery can lead to organ dysfunction and death by cellular hypoxia, detectable by progressive (mixed) venous oxyhemoglobin desaturation until extraction is limited at the anaerobic threshold. We sought to determine the critical level of venous oxygen saturation to maintain aerobic metabolism in neonates after the Norwood procedure (NP) for the hypoplastic left heart syndrome (HLHS). METHODS: A prospective perioperative database was maintained for demographic, hemodynamic, and laboratory data. Invasive arterial and atrial pressures, arterial saturation, oximet…
