[Pernicious anemia].

Pernicious anaemia is the most common cause of cobalamin deficiency. Nervous disorders associated with cobalamin deficiency are neuropathy, optic atrophy, dementia and myelopathy (subacute combined degeneration). In this case, symptoms are those of posterior and lateral column dysfunction of the spinal cord, with diminished vibratory sensation, ataxia, weakness of limbs, hyperreflexia, extensor plantar response and spasticity. Macrocytosis and anaemia are often lacking. There is an inverse correlation between the degree of anaemia and the extent of nervous impairment. The most sensitive tool f

[Perioperative anemia].

Anemia is a frequent finding, particularly in the elderly population, and usually indicative of a serious disease. The main causes of preoperative anemia are acute or chronic hemorrhage, iron deficiency, renal insufficiency, inflammatory and neoplastic diseases. A preexisting mild anemia may be enhanced or unmasked by surgically induced bleeding or repeated diagnostic phlebotomies, and by a postoperative erythropoietic dysfunction caused by the surgical trauma, irrespective of any hemorrhage. Low hemoglobin values are associated with a distinct increase of mortality and morbidity, both in the …

[Fanconi's anemia].

We have presented a case of Fanconi's anemia in six years old boy hospitalised in our Institute fifteen years ago. Beside hypoplastic pancitopenia, there is a growth retardation (under P3), late skeletal maturity, microcephalia, narrow rimae oculi, hyperpigmentetad skin, congenital abnormalities of both thumbs cryptorchidism, troubled speech, hyperagility. Cariogram was not performed, but present clinical-hematological signs confirme the diagnosis.

Microangiopathic Anemia.

BACKGROUND: Patients suffering from hemolytic anemia, thrombocytopenia, and organ damage may suffer from microangiopathic anemia, also called thrombotic microangiopathy (TMA). This condition is caused by many different pathogenic mechanisms and is always life-threatening due to vessel occlusion in vital organs. Rapid and careful workup is mandatory to identify the cause of TMA. To identify patients suffering from immune-mediated thrombotic thrombocytopenic purpura (iTTP), ADAMTS13 (a disintegrin and metalloproteinase with a thrombospondin type 1 motif, member 13) measurement is mandatory. All …

[Aplastic anemia].

Aplastic anemia is a rare disease with a large differential diagnosis, including neoplastic origin as well as congenital bone marrow failure syndromes. Investigations must be quick and precise. Treatment depends on the patient's age and consists of immunosuppression treatment or allogeneic bone marrow transplantation. Because of the risk of progression to other hematological diseases, a close specialized follow-up is recommended.

Hemolytic anemia.

Hemolysis presents as acute or chronic anemia, reticulocytosis, or jaundice. The diagnosis is established by reticulocytosis, increased unconjugated bilirubin and lactate dehydrogenase, decreased haptoglobin, and peripheral blood smear findings. Premature destruction of erythrocytes occurs intravascularly or extravascularly. The etiologies of hemolysis often are categorized as acquired or hereditary. Common acquired causes of hemolytic anemia are autoimmunity, microangiopathy, and infection. Immune-mediated hemolysis, caused by antierythrocyte antibodies, can be secondary to malignancies, auto…

Macrocytic anemia.

In approximately 2 to 4 percent of patients, laboratory evidence of macrocytosis is found. Macrocytic anemias are classified as those resulting from disorders of DNA synthesis of erythrocyte precursors in bone marrow (megaloblastic anemias) or those caused primarily by alcoholism, liver disease and hypothyroidism (nonmegaloblastic anemias). A blood smear should be performed to differentiate the two forms. Neutrophil hypersegmentation is one of the most sensitive and specific signs of megaloblastic anemia. Other testing should include vitamin B12 and red blood cell folate levels, reticulocyte c…

Fanconi anemia.

Fanconi anemia is a phenotypically and genotypically heterogeneous syndrome in which patients manifest various congenital abnormalities, bone marrow failure, and predisposition to malignancy. The primary dermatologic manifestations are pigmentation abnormalities (hyperpigmentation, hypopigmentation, café-au-lait spots) and cutaneous malignancies. The gene for one of the complementation groups (FACC) has been cloned, and the gene product has been shown to have a cytoplasmic localization, ruling out a direct role for the Fanconi anemia gene in DNA repair. A better understanding of the function o…

[Macrocytic anemia].

Macrocytic and/or megaloblastic anaemias of infants and children are more often due to a defective bone marrow production than to haemolysis. They are mostly related to folate and/or cobalamin deficiency or to a disturbance in the metabolism of one of these vitamins (enzyme deficiencies or defect of synthesis of their active forms). More rarely, these anaemias are associated with congenital deficiency of the enzymes involved in pyrimidine or purine biosynthesis. A few cases of thiamine-responsive megaloblastic anaemia have been reported. Some blood diseases may also associated with macrocytic

[Hepatogenic anemia].

Anaemias with multivariate pathogenesis are frequently observed in chronic liver diseases. Among our patients 44.4 per cent (132 of 297) had an anaemia. The hepatogenic anaemia which appeared most frequently in patients with liver cirrhosis and carcinoma of the liver was in most cases normochromic (75 per cent), at a small proportion hypochromic (17.4 per cent) and rarely hyperchromic (7.6 per cent). Initial phases of an anaemia were more frequently registered by means of the counting of erythrocytes than by means of the determination of haemoglobin, which is to be traced back to the frequentl

Megaloblastic anemia.

Most, but not all, megaloblastic anemia is produced by "ineffective erythropoiesis" in the bone marrow due to either folic acid or vitamin B12 deficiency. In folic acid deficiency the cause frequently is inadequate dietary intake, whereas vitamin B12 deficiency is almost always conditioned by some specific type of malabsorption. Anemia with oval macrocytes, few reticulocytes, moderate leukopenia, and thrombocytopenia is typical of both. Aplastic anemia, refractory anemias with cellular marrow, preleukemia, aleukemia, and erythroleukemia may have somewhat similar blood findings but are usually …
