Ankle fusion is a well-studied, classical procedure for conditions affecting the ankle joint, and has a long history with predictable results. Although there are many different approaches available, a few options are commonly used. The two main focuses of modern literature are on anterior plating versus traditional screw fixation, and on arthroscopic versus open techniques for fusion. This article discusses recent literature and then presents cases and techniques that are commonly used today. This includes arthroscopic ankle arthrodesis, traditional transfibular approach, anterior plating, and…

Stroke, spinal cord injury, and cerebral palsy are central nervous system disorders that impair voluntary movement due to dysfunctions in the brain, nervous system, and muscles. Cybernics treatment of HAL is being promoted as a treatment for these diseases. Besides, patients with central nervous system disorders who have symptoms of equinus, drop foot, and spasticity experience difficulties improving their ankle joint function during gait because the ankle joint of HAL is a passive mechanism, which cannot reflect the wearer's intention. Thus, it is hypothesized that ankle joints can be improve…

The biomechanics of the ankle present a unique set of challenges for arthroplasty surgery. Its biomechanics are not simple. Although the ankle joint may seem like a hinge, it is not in a line perpendicular to the tibia. The axis of rotation does not stay constant during range of motion, despite a relative congruency of this joint. Allowing for rotational forces must be accomplished, while maintaining the stability of the joint and its components. Success of the arthroplasty depends on how successful designs can dissipate these rotational forces, while maintaining the stability of the joint. It…

Injuries to the lateral ankle ligaments are the most common sports injuries. Determination of their severity and exclusion of relevant accompanying injuries requires a subtle clinical and a focussed radiological assessment. Treatment is non-operative and functional in the majority of cases. Consequent application of orthoses limiting supination and proprioceptive training are essential to avoid chronic instability. With recurrent ankle sprains one has to distinguish between functional and mechanical instability. The latter can be treated successfully with anatomic reconstruction and ligamentop

The etiology of chronic diseases of the ankle joint comprises a wide spectrum including chronic inflammatory processes and chronic degenerative, tumorous and neuropathic processes, as well as some specific syndromes based on chronic changes of the ankle joint. Of the inflammatory processes, chronic juvenile arthritis (JVC) is the most common disease. However, also Reiter disease, psoriasis or chronic monoarthritid diseases such as gout, as well as granulomatous diseases (tuberculosis, sarcoidosis) and fungal infections, may affect the ankle joint in a chronic course. Chronic degenerative chang…

The late reconstruction of a complex nonunion of the distal tibial metaphysis, the ankle, or the subtalar joint was undertaken in five patients through a posterior surgical approach and the application of a blade plate into the os calcis. This technique was preferred as a salvage operation when the surrounding soft tissue envelope was compromised by the initial injury, previous surgical procedures, or infection. Each of the cases was notable for a segmental bone loss, an infected nonunion, or a collapse of the talar body. In the presence of a major soft tissue defect, the internal fixation was

This paper reports fifteen cases of ball and socket articulation at the ankle followed up for an average of twelve years. All patients showed inequality of leg length. Ten patients showed coalitions of the bones of the hindfoot and nine patients had a reduction in the number of bony elements of the forefoot. Other associated anomalies are described. The abnormality seems to be part of a congenital short-limb malformation, perhaps modified by adaptive change associated with rigidity of the hindfoot. Surgical intervention was not required in any patient in the series.

Chen, C-H, Yeh, M-C, Liu, C, Tseng, Y-J, and Lu, S-T. Association of ankle joint moment and ankle joint velocity with 60° ipsilateral change-of-direction performance. J Strength Cond Res XX(X): 000-000, 2026-The rapid acceleration and deceleration associated with a change of direction (COD) is associated with high ankle loading, including high ankle moment and velocity, which have been linked to ankle sprain or chronic ankle instability. Although the correlations between ankle joint loading and contralateral COD (contra-COD) have been investigated, one study reported that ipsilateral COD (ipsi…

Osteoarthritis of the ankle joint affects a younger and often occupational and sport-active patient group than hip and knee arthritis and in the majority of cases has a secondary or posttraumatic origin. Corrective osteotomy is a biological and functionally attractive option for these young and active patients with asymmetric arthritis and correctable malalignment as the native joint is preserved and subsequent arthrosis can be potentially prevented or delayed by arthrodesis. Decisive for the success are consistent 3‑dimensional diagnostics, precise planning, correct interpretation of the indi…

The ankle joint has to bear the entire body weight on a relatively small joint surface. Incongruities, instabilities and deformities lead to painful arthrosis and considerable restrictions in everyday life. For many years, arthrodesis has proven to be the gold standard for end stage arthrosis; however, considering modern endoprostheses for the ankle joint it is no longer appropriate to offer only arthrodesis. The very good results of the 3rd generation 3‑component prostheses and the 4th generation 2‑component prostheses show how much the surgical treatment options for the ankle joint are curre…
