Gastric emptying of two radiolabelled antacids.

The rate of gastric emptying of two antacids, magaldrate and Maalox, was investigated using scintigraphy. Successful labelling of the antacids was carried out with 99mTc. The stability of the 99mTc-labelled antacids was satisfactory and there was no difference in antacid capacity between the labelled and unlabelled antacids. The studies were carried out on 15 healthy male volunteers. After an eight hour fast each subject ingested a standardised meal of 95.7 MJ (400 kcal). One hour later 10 ml of one of the two antacids previously labelled with 99mTc was administered. Serial detection by anteri…

[Effect of antacids on mineral metabolism].

Side effects of antacid therapy are dose dependant and compound related. High dose antacid intake may lead to fluid retention in the body depending on the sodium content of the different antacid preparations. Sodium bicarbonate ingestion provokes metabolic alkalosis and alkaliuria, the "nonsystemic calcium and magnesium containing antacids" cause these changes too, but to a lower degree. Urinary pH elevation favours the precipitation of calcium and magnesium salts, predisposing to renal stone formation. In patients with renal insufficiency the calcium and magnesium absorption may lead rapidly …

Small bowel obstruction from an antacid bezoar: a ranitidine-antacid interaction?

A 63-year-old man had an acute small bowel obstruction while being treated with ranitidine and self-prescribed antacid. On exploration, a firm mass, chemically similar to the ingested antacid, was found impacted in the terminal ileum. We believe the effect of ranitidine on gastric secretory volume was the cause of the antacid concretion.

Cefuroxime antacid interactions.

Cefuroxime sodium a second generation semi-synthetic cephalosporin is effective for treating meningitis, lower respiratory tract infections, gonorrhea, bone and joint infections and is approved for surgical prophylaxis. There are number of synergistic and as well as antagonistic drug interactions reported for this antibiotic. Withstanding to gastric irritations caused by this antibiotic, antacids may possibly be co-administered with cefuroxime, which may result in severe adverse drug interaction. The present work describes the effect of magnesium carbonate, magnesium hydroxide, magnesium trisi…

Effect of antacids on salicylate kinetics.

The effect of an antacid on the absorption of aspirin can vary depending on certain physical characteristics of the brand of aspirin chosen. In a study carried out to test the effect of a commonly used antacid on the absorption of a brand of aspirin, no differences were seen in the area under the curve (AUC) of plasma concentrations of salicylic acid versus time, Cmax and Ka of salicylic acid without or with antacid. However, the antacid was seen to alter the pH of urine in four of the six subjects investigated. The study, when extended to examine the effect of antacid on steady-state kinetics

Antacids and peptic ulcer--a reappraisal.

Antacids can reduce gastroduodenal acidity for long periods if taken in substantial quantities after food. Their healing effect on gastric ulcer is minimal, if present at all, and easily overwhelmed by the benefit obtained from admission to hospital. Intensive antacid therapy appears effective in healing duodenal ulcer and preventing haemorrhage from stress ulcer, and is comparable in these respects with cimetidine but with a higher incidence of side-effects. Clinical impression strongly suggests that antacids relieve pain in peptic ulcer but objective confirmation is lacking.

Antacid-induced osteomalacia.

The case of a 49-year-old woman suffering from generalized skeletal pain and multiple fractures accompanied by severe hypophosphataemia and low urinary phosphorus excretion is reported. She had been taking large amounts of antacids containing aluminum hydroxide for many years. A diagnosis of antacid-induced osteomalacia was made. It was confirmed by biological work-up, radiographs and bone biopsy. A dramatic biological, osteodensitometric, and clinical improvement was achieved by withdrawal of antacids and phosphorus administration. The literature concerning this unusual condition has been rev…

New aspects of clinical pharmacology of antacids.

Most antacids contain magnesium and/or aluminium hydroxide but some include also bicarbonate and calcium carbonate. The net effect of antacid action is the reduction in gastric acidity and in peptic activity but recent studies indicate that aluminium-containing antacids exhibit the cytoprotective activity or enhancement of natural mucosal defense mechanisms. This protective action has been attributed, in part, to the release of endogenous prostaglandins but other mediators such as hexa-aquo-aluminium cation and nitric oxide have also been proposed. Aluminium-containing antacids are known to ac…

Gastroesophageal reflux disease. When antacids aren't enough.

Gastroesophageal reflux disease is a very common condition that is usually manifested by heartburn or regurgitation. Reflux esophagitis, caused by mucosal exposure to the backflow of caustic gastric contents, is primarily a result of lower esophageal sphincter dysfunction. Diagnostic workup varies but commonly includes esophagoscopy, 24-hour esophageal pH monitoring, and radiography of the upper gastrointestinal tract. Treatment, which progresses from simple life-style changes and antacids to histamine2 receptor antagonists to omeprazole (Prilosec) or surgery, is tailored to individual needs a…

Drug spotlight program: antacids and anticholinergic drugs.

Controlled clinical trials have provided some useful evidence of the effectiveness or lack of effectiveness of antacids and anticholinergic drugs. However, few such trials are free of serious methodologic or clinical criticism, and the use of these agents cannot be defined with any degree of scientific certainty. Their principal uses still focus on reduction of gastric acid content, and their best use requires individualization for each patient. With anticholinergic drugs there is little if any pharmacologic effect without some side-effects, and dose adjustment may be more important than the c…

Antacid bezoar in a premature infant.

A premature infant with severe enteroviral infection was treated with Maalox for gastric bleeding. Bezoars formed, which were shown to contain high concentrations of magnesium. An association between antacid administration and bezoar formation in neonates has been previously suggested but has not until now been clearly documented.

Hyperosmolality due to antacid treatment.

Magnesium trisilicate mixture is an antacid used commonly in our Intensive Care Unit in the prevention and treatment of stress ulcers. In this case the administration of large doses over a period of time led to the development of massive hyperosmolality, cerebral dehydration and coma. Management with hypotonic fluid resulted in complete recovery.

In vitro antidiabetics-antacid interactions.

The adsorption of the oral antidiabetics metformin hydrochloride, glibenclamide, acetohexamide, tolbutamide, carbutamide, tolazamide and glymidine on various antacids or adsorbents was studied at 37 degrees C. The antacids or adsorbents used were magnesium trisilicate, aluminium hydroxide, calcium carbonate, magnesium oxide, bismuth oxycarbonate, talc, kaolin as well as charcoal. None of the substances tested, with the exception of charcoal, exhibited superior adsorptive properties for all the drugs. Magnesium trisilicate and calcium carbonate were the best adsorbents for metformin hydrochlori…
