Antibiotic prophylaxis for mammalian bites.

BACKGROUND: Bites by mammals are a common problem and they account for up to 1% of all visits to hospital emergency rooms. Dog and cat bites are the most common and people are usually bitten by their own pets or by an animal known to them. School-age children make up almost a half of those bitten. Prevention of tetanus, rabies and wound infection are the priorities for staff in emergency rooms. The use of antibiotics may be useful to reduce the risk of developing a wound infection. OBJECTIVES: To determine if the use of prophylactic antibiotics in mammalian bites is effective in preventing bi

[Antibiotic prophylaxis in gastroduodenal surgery].

The incidence of postoperative wound infections is increased up to 35% after gastroduodenal surgery, when gastric motility and acidity are decreased, as in case of gastric ulcer or cancer, obstruction, bleeding, antacid therapy. The endogenous flora contaminating the operative-site consists of organisms of the oropharynx and the jejunum and includes anaerobes like bacteroides, aerobes like streptococci, staphylococci, E. coli. Antimicrobial prophylaxis is therefore indicated in these high risk patients. All groups of antibiotics have been used, however 1st and 2nd generation cephalosporins are

[Antibiotic prophylaxis in orthopedic surgery].

The efficiency of prophylactic antibiotics in total hip replacement has been demonstrated by several studies. The rate of infection decreased approximatively from 3% to 1% with a cephalosporine 1 protocol. The rate of infection was not different after a five-day administration of cephalosporine 1 and a 2-day treatment of cephalosporine 2. In open fractures, the value of prophylactic antibiotics has to take in account the usual organisms of peroperative contamination as well as the anaerobes. Short administration can be used, however no randomized studies have proven their efficiency. For clean

[Antibiotic prophylaxis in gynecologic surgery].

Hysterectomies are contaminated procedures; surgical wound related sepsis ranges from 10 to 25%. Several randomised controlled studies demonstrated a significant benefit with prophylactic antibiotics. The study results were more controversial for abdominal hysterectomies. Ureidopenicillins, cephamycins and other antibacterial agents active on Gram negative rods and anaerobes can be recommended as a preoperative flash at anaesthesia induction, followed by a second injection whenever the procedure duration exceeds 3 hours. In case of extended hysterectomy for cancer, broad spectrum antibiotic pr

Antibiotic prophylaxis in cesarean section.

Several controversies exist regarding the use of antibiotic prophylaxis in cesarean section. Studies have investigated effectiveness, choice of drug, timing, duration, and appropriate route of administration. Efficacy of antibiotic prophylaxis in preventing postoperative endometritis in certain high-risk patients has been well documented. Factors identifying patients at high risk for endometritis include: indigent socioeconomic status, labor prior to cesarean section, rupture of chorioamniotic membranes, and number of vaginal examinations. Although many different antibiotics have been tested,

Topical antibiotic prophylaxis in neurosurgery.

Topical antibiotics have been used prophylactically in clean operations by many neurosurgeons. Many others have not used them, however. Review of the experimental literature shows that topical antibiotics will reduce the rate of wound infection in experimentally contaminated wounds. Experience in other surgical disciplines suggests that the infection rate of operations with an intrinsically high risk of infection (greater than 15%) can be reduced substantially by the use of topical antibiotics. There is no sound scientific evidence documenting the benefit of prophylactic topical antibiotics in

Antibiotic prophylaxis in orthopedic surgery.

Antimicrobial prophylaxis, to reduce the incidence of postoperative infections, has been a controversial subject. The administration of antibiotics just prior to and up to 3 hours after operation has been demonstrated to be effective in preventing experimental staphylococcal wound infection in animals. However, the use of prophylactic antibiotics in orthopedic surgery has been tested in appropriate clinical trials only in the treatment of fractured hip. In that setting, prophylactic antibiotics were found to be efficacious. Although used extensively in other procedures such as total hip replac

Antibiotic Prophylaxis of Infective Endocarditis.

Links between infective endocarditis (IE) and dental and other invasive procedures were first identified in the 1920s, and the use of antibiotic prophylaxis (AP) to prevent IE was first recommended by the American Heart Association in 1955. Recognising the weak evidence to support this practice and the wider risks of anaphylaxis and antibiotic resistance, guidelines in the USA and Europe have been rationalised in the last decade with restriction of AP to those patients perceived to be at the highest risk. In the UK, the National Institute for Health and Care Excellence controversially recommen

[Antibiotic prophylaxis and prostate biopsy].

OBJECTIVES: Perioperative antibiotic prophylaxis is considered a safe and valid option from the urologist to prevent infective complications after prostate biopsy procedure. The transrectal approach is the most experienced by European urologists and the prevalence of infective complications from this procedure is increasing. Aim of this mini-review article is to improve the urologist knowledge on the prevention of infective complications after prostate transrectal biopsy. METHODS: Recent publications from Medline and Cochrane Library have been collected. Data from the abstracts presented at t

[Antibiotic prophylaxis in prostate biopsy].

INTRODUCTION: Prostate biopsy is nowadays one of the most frequent diagnostic procedures in urology. The incidence of bacteraemia, bacteriuria and infective complications is higher after the transrectal procedure than after the transperineal one. A survey demonstrated that 98% of the urologists in USA use antibiotics to prevent infective complications. The transrectal prostate biopsy is the only diagnostic intervention procedure in urology for which an antibiotic prophylaxis is recommended, also for low-risk patients, by the guidelines of the European Association of Urology. If the perineal ro

UNLABELLED: SIGNIFICANT INCIDENCE OF SPLENECTOMY: Asplenism concerns a significant number of subjects: in France 6,000 to 9,000 patients undergo splenectomy annually. More than half of these patients are adults. NECESSARY PREVENTION: An asplenic adult is exposed to serious infections, predominantly caused by pneumococci but also by other encapsulated bacteria. Antibiotic prophylaxis is necessary and must be included in the overall management scheme in patients undergoing planned splenectomy. IN CLINICAL PRACTICE: In France the official recommendation for antibiotic prophylaxis in the aspleni

[Pathways for surgical antibiotic prophylaxis].

Surgical site infections (SSIs) are a notable cause of hospital morbidity and mortality. Antibiotic prophylaxis has demonstrated a significant reduction in infection rate in clean-contaminated surgery and in clean surgery to a limited extent. To make antibiotic prophylaxis effective it is necessary to choose the right antibiotic, to administer it preoperatively and maintain sufficient serum and tissue levels through the operation. Open issues remain: antibiotic prophylaxis duration in prosthetic surgery, its use in hernia repair, breast surgery and mini-invasive surgery.

Antibiotic prophylaxis for cesarean section.

BACKGROUND: The single most important risk factor for postpartum maternal infection is cesarean delivery. OBJECTIVES: The objective of this review was to assess the effects of prophylactic antibiotic treatment on infectious complications in women undergoing cesarean delivery. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group trials register (January 2002) and the Cochrane Controlled Trials Register (The Cochrane Library, Issue 4, 2001). SELECTION CRITERIA: Randomized trials comparing antibiotic prophylaxis or no treatment for both elective and non-elective cesarean se

Antibiotic prophylaxis at Cesarean delivery.

OBJECTIVE: Our aim was to compare the efficacy of ampicillin, cefotetan, and ampicillin/sulbactam in the prevention of post-Cesarean endomyometritis. METHODS: Consenting patients undergoing Cesarean delivery at the University of Louisville Hospital were enrolled in a prospective, double-blinded randomization to receive either ampicillin/sulbactam (Group 1), cefotetan (Group 2), or ampicillin (Group 3) single dose antibiotic prophylaxis following umbilical cord clamping. The primary outcome variable was the frequency of endomyometritis in the respective groups. RESULTS: Among 301 randomized p

Antibiotic prophylaxis in cutaneous surgery.

BACKGROUND: Postoperative wound infections, endocarditis, and late contamination of prosthetic implants are potential complications of cutaneous surgery. OBJECTIVE: To review current American Heart Association guidelines and their application to cutaneous surgery, and to determine how practicing physicians approach this problem. METHOD: Surveys were sent to dermatologists to determine what kind of preoperative screening was being done and how antibiotics were being used. RESULTS: The most common indications for prophylaxis were manipulation of infected tissue in patients undergoing any proc

Today, hepatic and biliary surgery includes conventional biliary surgery, laparoscopic surgery, interventional radiology, endoscopy and hepatic resection surgery. In conventional biliary surgery, the benefit of antibiotic prophylaxis has been demonstrated. Among the risk factors, some are specific (age > 65 years, gallstones in the common bile duct with or without jaundice, history of acute cholecystitis or of biliary surgery) and the others are non specific such as the CDC new index risk (for scoring from 0 to 3, wound infection rates are respectively 1.36, 2.01, 7.11, 11.54%). Antibiotic prophylaxis in recurrent erysipelas.

Recurrences of erysipelas are especially prevalent in patients suffering from local impairment of circulation and intervention might thus be of benefit. Therefore a prospective, randomized, open study was undertaken to evaluate whether daily antibiotic prophylaxis would reduce the risk of recurrence. Patients with venous insufficiency or lymphatic congestion who had suffered two or more episodes of erysipelas during the previous 3 years and were admitted to the Infectious Disease Department at Roslagstull Hospital, Stockholm, Sweden, between November 1988 and November 1991 were included.
