[Peripheral neurologic complications following percutaneous transaxillary aortography].

Today percutaneous transaxillary aortography is indicated cautiously because of severe neurological permanent injury. The nervous lesion is superficially caused by catheter manipulation or postangiographic hematomas resp. an aneurysma spurium. The symptomless time interval of hours or days proves the cause of hematoma. In the early stage an overlay of the damage causes is possible. The sensible and motorial deficit can be decisively influenced by an early exoneration of the neurovascular septum.

Successful revascularization of mesenteric infarction following aortography.

Two cases of bowel infarction following abdominal aortography are presented. In both patients, two of the three major arteries supplying the bowel were occluded before the study. The superior mesenteric artery in one and the inferior mesenteric artery in the other were the only arteries supplying the bowel and their lumens were reduced. After the aortogram, the residual lumen clotted, leading to bowel necrosis. Survival was made possible in these two cases by revascularizing the superior mesenteric artery and resecting the necrotic bowel.

Is aortography in lower limb ischaemia still abused?

A study in 1980 revealed that 38% of aortograms performed in a series had been referred for inappropriate reasons and only 49% had been clearly indicated. Recommendations were made to reduce the number of inappropriate investigations. An audit has been carried out of the aortography referrals made in this unit recently. This has shown that, despite changes in the management of peripheral vascular disease in the intervening years, by following the recommendations made in the previous paper, the number of inappropriate aortograms has been reduced to a minimum. The recommendations are endorsed.

Oblique projections in aortography following blunt thoracic trauma.

We report a patient with innominate artery false-aneurysm formation following blunt trauma to the thorax. Aortography in the standard anteroposterior and left anterior oblique (LAO) projections failed to delineate the injury. Its true extent was only demonstrable in the right anterior oblique (RAO) projection. When the clinical history and a chest radiograph suggest a high likelihood of injury to the aorta or great vessels, routine aortography may need to be performed in both the LAO and RAO projections.

Indications for aortography in blunt thoracic trauma: a reassessment.

The indications for aortography in patients sustaining blunt chest trauma have increased as the number of radiographic and clinical findings associated with traumatic rupture of the thoracic aorta (TRA) proliferate. No studies, however, have demonstrated whether these findings are useful in selecting patients for aortography. In order to determine the predictive value of reported associations in TRA, the presence or absence of nine radiographic and nine clinical findings associated with TRA were tabulated and tested for correlation with the results of aortography in 173 consecutive patients wh…

Upper rib fractures and mediastinal widening: indications for aortography.

Survival of patients with posttraumatic thoracic aortic rupture depends on early diagnosis. It is frequently stated that fracture of the first or second ribs and mediastinal widening are findings suggestive of thoracic aortic rupture. We found that the probability of sustaining thoracic aortic rupture is the same for patients with upper rib fractures as for those with other rib fractures (1/64 versus 5/149; p = 0.85). Also, our data fail to show a statistical difference in the incidence of thoracic aortic rupture associated with upper (first and second) rib fractures compared with no rib fract

Clinical and radiographic indications for aortography in blunt chest trauma.

To determine which clinical and radiographic findings are valuable in selecting patients with blunt chest trauma for aortography, we analyzed the medical records and admission chest radiographs of 76 consecutive victims of blunt chest trauma with suspected thoracic aortic rupture during the past 7 years. All patients were evaluated by history, physical examination, chest radiography, and aortography; a total of 70 clinical and radiographic findings were independently assessed in each patient. The following occurred with significantly greater frequency in patients with thoracic aortic rupture t…
