Sensory integration dysfunction affects efficacy of speech therapy on children with functional articulation disorders.

BACKGROUND: Articulation disorders in young children are due to defects occurring at a certain stage in sensory and motor development. Some children with functional articulation disorders may also have sensory integration dysfunction (SID). We hypothesized that speech therapy would be less efficacious in children with SID than in those without SID. Hence, the purpose of this study was to compare the efficacy of speech therapy in two groups of children with functional articulation disorders: those without and those with SID. METHOD: A total of 30 young children with functional articulation dis…

[Lipotransfer after free-flap tongue reconstruction can improve dysphagia and articulation disorders: a case report].

Atrophy of tongue after free-flap reconstruction often leads to functional problems like dysphagia and articulation disorders. Lipotransfer might resolve this issue. We describe a case of repeated lipotransfer into an atrophied reconstructed tongue.A 66-year-old patient with a squamous cell carcinoma of the tongue underwent hemiglossectomy and reconstruction of the tongue by a free radial forearm flap. After atrophy of the reconstructed tissue, repeated sessions of lipotransfer were undertaken to augment the atrophic tissue. Three sessions of lipotransfer resulted in satisfactory augmentation …

Occlusal Sensitivity to Articulating Foils in Patients With Temporomandibular Disorders and Healthy Controls.

OBJECTIVES: To compare occlusal tactile acuity (OTA), the ability to detect and discriminate occlusal contacts, between patients with chronic painful temporomandibular disorders (TMDp) and healthy controls and to examine the influence of pain and oral behaviours on foil perception. METHODS: Forty TMDp patients (34 female, 6 male) and 40 healthy controls (20 female, 20 male) completed OTA testing with foils 8-56 μm thickness and a sham stimulus (48 trials). Participants bit in maximal intercuspal position and gave yes/no responses regarding foil perception. Oral behaviours were assessed using

[Kinematic magnetic resonance tomography in functional disorders of joints and vertebral articulations].

Imaging studies of peripheral joints and of vertebral junctions are of major importance in the diagnosis of many degenerative diseases of the musculoskeletal system. Radiograms with flexion and extension views are regarded as standard for diagnosing abnormalities of the spine, the patello-femoral joint, and other articulations. With magnetic resonance imaging (MRI), kinematic studies can be performed analogously. With high gradient field strengths and in-phase and opposed-phase gradient echo sequences it is possible to study the motion of joints continuously. First results show that this techn

The occurrence of developmental apraxia of speech in a mild articulation disorder: a case study.

Although Developmental Apraxia of Speech (DAS) often is considered a severe communication disorder, speech-language pathology literature supports the idea that DAS also can be exhibited in children with mildly disordered articulation. The case study is that of an elementary age client with a seemingly mild "r" articulation problem but who also presented characteristics consistent with DAS. Awareness of the possible presence of DAS when assessing and planning remediation for clients exhibiting mild articulation problems could increase the effectiveness of our professional services to these chil

Audiovisual materials are effective for enhancing the correction of articulation disorders in children with cleft palate.

INTRODUCTION: Children with cleft palate frequently show speech disorders known as compensatory articulation. Compensatory articulation requires a prolonged period of speech intervention that should include reinforcement at home. However, frequently relatives do not know how to work with their children at home. OBJECTIVE: To study whether the use of audiovisual materials especially designed for complementing speech pathology treatment in children with compensatory articulation can be effective for stimulating articulation practice at home and consequently enhancing speech normalization in chi

These results suggest that articulation disorders may indicate a central auditory processing deficit, particularly a delay in right-hemisphere auditory maturation. It has been observed that in children with central auditory processing disorder, whatever speech sounds they process defectively during …

Atrophy of the tongue with persistent articulation disorder in myasthenia gravis: report of 10 patients.

Ten patients with atrophy of the tongue, from a group of 752 with generalized acquired myasthenia gravis (MG), were studied. Tongue atrophy developed late in the majority of patients and was accompanied by tongue paresis (70% of the cases) and eventually associated to atrophy of other muscles of the palate, especially the uvula. All the patients exhibited severe forms of MG with bulbar involvement, mainly persistent dysphonia and dysphagia, almost always refractory to treatment. There is no correlation among atrophy of the tongue, sex, and thymus pathology. There is correlation between severen

Using electropalatography (EPG) to diagnose and treat articulation disorders associated with mild cerebral palsy: a case study.

Some children with mild cerebral palsy have articulation disorders that are resistant to conventional speech therapy techniques. This preliminary study investigated the use of electropalatography (EPG) to diagnose and treat a long-standing articulation disorder that had not responded to conventional speech therapy techniques in an 8-year-old boy (D) with a congenital left hemiplegia. The targets for EPG therapy were speech errors affecting velar targets /k, g, eta/, which were consistently fronted to alveolar placement [t, d, n]. After 15 sessions of EPG therapy over a 4-month period, D's abil…

Progressive apraxia of speech presenting as isolated disorder of speech articulation and prosody: a case report.

Apraxia of speech (AOS) is a rare disorder of motor speech programming, and few case reports have included sufficient description of both clinical findings and lesion localization. We report a case with an isolated progressive speech articulation deficit and brain involvement restricted to the left superior frontal gyrus. This case suggests that slowly progressive AOS may be a clinical disorder distinct from primary progressive aphasia, and that it can occur without language disorders or bucco-facial apraxia. Our findings highlight the importance of the left hemisphere in speech articulatory d

Exploring the Psychosocial Impact and Developing a Quality of Life Scale for Postpalatoplasty and Noncleft Patients With Functional Articulation Disorders.

This study was performed to develop and validate the quality of life scale for postpalatoplasty and noncleft FAD (Functional Articulation Disorder) patients and to survey their quality of life (QoL) status. A mixed-methods scale development process yielded a 20-item instrument. Psychometric properties were evaluated in 128 children (41 postpalatoplasty FAD, 87 noncleft FAD). Speech clarity was assessed in a subset (n=38). The scale demonstrated excellent reliability (Cronbach α=0.963) and validity. Postpalatoplasty FAD patients reported significantly poorer QoL than noncleft FAD patients, with

Three-dimensional ultrasound evaluation of tongue posture and its impact on articulation disorders in preschool children with anterior open bite.

Background Tongue posture plays an important role in the etiology of anterior open bite (AOB) and articulation disorders, and is crucial for AOB treatment planning and posttreatment stability. Clinical assessment of tongue posture in children is unreliable due to anatomical limitations. The aim of the study was to present functional diagnostics using three-dimensional ultrasound (3DUS) assessment of resting tongue posture in comparison to clinical assessment, and the associations between the improper tongue posture, otorhinolaryngological characteristics, and articulation disorders in preschoo…

Visual training and correction of articulation disorders by use of dynamic palatography: serial observation in a case of cleft palate.

The dynamic palatograph is an electrical apparatus that generates a visual display of constantly changing palatolingual contact as a function of time, using an artificial palatal plate with affixed electrodes. This paper describes a technique of speech therapy incorporating dynamic palatography for a cleft palate patient. The patient, a 6-year-old Japanese girl with a repaired unilateral cleft lip and palate, had been judged to demonstrate articulation disorders involving contact of the tongue with the hard palate or alveolus following surgical improvement of velopharyngeal function. Prior to …
