Astronaut psychiatric selection procedures: a Japanese experience.

This paper presents the results of the first application of a structured NASA psychiatric interview for Japanese astronaut selection. We undertook a semi-structured, diagnostic, clinical psychiatric interview with 45 Japanese astronaut applicants for the position of mission specialist-astronaut, and performed "select-out" psychiatric evaluations according to DSM-III-R psychiatric diagnostic criteria. Two applicants (4.4%) who met Axis I or Axis II disorder criteria in DSM-III-R were evaluated as "Disqualified." There were 13 applicants "Qualified with Reservation"; the majority of their diagno

Risk of herniated nucleus pulposus among U.S. astronauts.

INTRODUCTION: Astronauts have complained of back pain occurring during spaceflight, presumably due to the elongation of the spine from the lack of gravity. Herniated nucleus pulposus (HNP) is known to occur in aviators exposed to high Gz and has been diagnosed in several astronauts in the immediate post-spaceflight period. It is unknown whether astronauts exposed to microgravity are at added risk for developing HNP in the post-spaceflight period due to possible in-flight intervertebral disc changes. METHODS: For a preset study period, incidence rates of HNP were compared between the U.S. astr

[Selection of vestibular system function in Chinese astronauts].

OBJECTIVE: To examine and evaluate the vestibular function of Chinese astronaut candidates in order to select the astronauts with good vestibular function. METHOD: Fifty-nine male candidates, the fighter plane pilots, participated in the Caloric tests, and 38 pilots among all the candidates experienced the parallel swing and Coriolis acceleration stimuli in tests. The parameters, such as vestibule-autonomic nervous reflex symptoms, electrogastrography (EGG), electronystagmography (ENG), ECG, blood pressure, postural equilibrium ability were employed to evaluate the vestibular function. Otoacoustic Estimate of Astronauts' Intracranial Pressure Changes During Spaceflight.

PURPOSE: To investigate the potential correlation between prolonged exposure to microgravity on the International Space Station and increased intracranial fluid pressure, which is considered a risk factor for the astronauts' vision, and to explore the feasibility of using distortion product otoacoustic emissions as a non-invasive in-flight monitor for intracranial pressure changes. METHODS: Distortion product otoacoustic emission phase measurements were taken from both ears of five astronauts pre-flight, in-flight, and post-flight. These measurements served as indirect indicators of intracran

Regulation of circadian blood pressure: from mice to astronauts.

PURPOSE OF REVIEW: Circadian variation is commonly seen in healthy people; aberration in these biological rhythms is an early sign of disease. Impaired circadian variation of blood pressure (BP) has been shown to be associated with greater target organ damage and with an elevated risk of cardiovascular events independent of the BP load. The purpose of this review is to examine the physiology of circadian BP variation and propose a tripartite model that explains the regulation of circadian BP. RECENT FINDINGS: The time-keeper in mammals resides centrally in the suprachiasmatic nucleus. Apart f

Albumin, oral contraceptives, and venous thromboembolism risk in astronauts.

A venous thromboembolism (VTE) event occurred in a female astronaut during long-duration spaceflight. Multiple factors may have contributed to this risk, including the use of combined (progestin + estrogen) oral contraceptives (cOC). Biochemistry data from 65 astronauts were evaluated for associations with cOC use and with sex. The female astronauts who used cOCs had lower concentrations of serum albumin and higher concentrations of transferrin, a protein involved in the clotting cascade, than the male astronauts and the female astronauts who were not taking cOCs (P < 0.001). The women who use

Pharmaceutical use by U.S. astronauts on space shuttle missions.

We evaluated in-flight use of medications from astronaut debriefings after 79 U.S. Space Shuttle missions. From the 219 records obtained (each representing one person-flight), 94% included some medication being taken during flight; of that number, 47% were for space motion sickness, 45% for sleep disturbances, and smaller percentages for headache, backache, and sinus congestion. Drugs were taken most often orally, followed in decreasing order of frequency by intranasal, intramuscular, and rectal routes. Drugs for space motion sickness were taken mostly during the first 2 d of flight, drugs for

Orthostatic hypotension in patients, bed rest subjects, and astronauts.

Orthostatic hypotension after even short space flights has affected a significant number of astronauts. Given the need for astronauts to function at a high level of efficiency during and after their return from space, the application of pharmacologic and other treatments is strongly indicated. This report addresses the clinical problem of orthostatic hypotension and its treatments to ascertain whether pharmacologic or physiologic treatment may be useful in the prevention of orthostatic hypotension associated with space flight. Treatment of orthostatic hypotension in patients now includes incre

Precision Medicine in Astronauts: Principles and Targets in Molecular Assessment.

The field of space medicine has served the population of professional astronauts well over the many decades of human space travel, which is reflected in the considerable success of astronauts executing missions and returning to Earth in good health (rare comorbidities notwithstanding). At the same time, there are considerable gaps in our biomedical knowledge that will only scale as less fit individuals embark upon spaceflight, and we enter more extreme environments such as the Moon and Mars for longer duration missions. One means to partially fill this knowledge gap is to expand the practice o

Analysis of Spaceflight-Associated Biometric and Refractive Changes in Astronauts.

PURPOSE: To study changes in ocular biometry and refraction in the largest cohort to date of astronauts who have experienced long-duration spaceflight on the International Space Station (ISS). DESIGN: A prospective cohort study. PARTICIPANTS: 29 astronauts. METHODS: Preflight and postflight cycloplegic refraction and ocular biometry measurements were obtained from 56 eyes among 29 subjects. For each eye, the preflight-to-postflight changes in spherical equivalent (SE), axial length (AL), average corneal curvature (K), and anterior chamber depth (ACD) were calculated. Time perception in astronauts on board the International Space Station.

We perceive the environment through an elaborate mental representation based on a constant integration of sensory inputs, knowledge, and expectations. Previous studies of astronauts on board the International Space Station have shown that the mental representation of space, such as the perception of object size, distance, and depth, is altered in orbit. Because the mental representations of space and time have some overlap in neural networks, we hypothesized that perception of time would also be affected by spaceflight. Ten astronauts were tested before, during, and after a 6-8-month spaceflig

Fludrocortisone does not prevent orthostatic hypotension in astronauts after spaceflight.

BACKGROUND: During stand/tilt tests after spaceflight, 20% of astronauts experience orthostatic hypotension and presyncope. Spaceflight-induced hypovolemia is a contributing factor. Fludrocortisone, a synthetic mineralocorticoid, has been shown to increase plasma volume and orthostatic tolerance in Earth-bound patients. The efficacy of fludrocortisone as a treatment for postflight hypovolemia and orthostatic hypotension in astronauts has not been studied. Our purpose was to test the hypothesis that astronauts who ingest fludrocortisone prior to landing would have less loss of plasma volume and

Staying in bed to benefit ESA's astronauts and Europe's citizens.

Since Yuri Gagarin's historic first flight into space in April 1961, it has quickly become evident that the space environment influences the human body in many different ways and causes it to adapt in ways that can lead to problems when returning to Earth's gravity. Much research has been performed in the meantime and our understanding of what happens to our bodies in space improved considerably during the Mir space station and Space Shuttle/Spacelab era. However, many questions, particularly regarding how to counteract those changes that we now know take place, still need to be addressed thro

Dynamic visual acuity as a biometric for astronaut performance and safety.

Astronauts are exposed to an austere and constantly changing environment during space travel. To respond to these rapid environmental changes, high levels of dynamic visual acuity (DVA) are required. DVA is the ability to visualize objects that are in motion, or with head movement and has previously been shown to decrease significantly following spaceflight. Decreased DVA can potentially impact astronauts while performing mission critical tasks and drive space motion sickness. In this paper, we suggest that DVA assessment during spaceflight and during G-transitions should be considered to help

Multiple latent viruses reactivate in astronauts during Space Shuttle missions.

Latent virus reactivation and diurnal salivary cortisol and dehydroepiandrosterone were measured prospectively in 17 astronauts (16 male and 1 female) before, during, and after short-duration (12-16 days) Space Shuttle missions. Blood, urine, and saliva samples were collected during each of these phases. Antiviral antibodies and viral load (DNA) were measured for Epstein-Barr virus (EBV), varicella-zoster virus (VZV), and cytomegalovirus (CMV). Three astronauts did not shed any virus in any of their samples collected before, during, or after flight. EBV was shed in the saliva in all of the rem

Distance and Size Perception in Astronauts during Long-Duration Spaceflight.

Exposure to microgravity during spaceflight is known to elicit orientation illusions, errors in sensory localization, postural imbalance, changes in vestibulo-spinal and vestibulo-ocular reflexes, and space motion sickness. The objective of this experiment was to investigate whether an alteration in cognitive visual-spatial processing, such as the perception of distance and size of objects, is also taking place during prolonged exposure to microgravity. Our results show that astronauts on board the International Space Station exhibit biases in the perception of their environment. Objects' heig

Predicting Spaceflight-Associated Neuro-Ocular Syndrome in International Space Station Astronauts.

IMPORTANCE: Spaceflight-associated neuro-ocular syndrome (SANS) could severely impact astronaut performance during long-duration spaceflight (LDSF). The pathophysiology of SANS remains elusive, and no predictors of SANS have been discovered; reliable prediction may offer novel insight into individual susceptibility and interperson variability for SANS and permit prelaunch preparations for in-flight treatment. OBJECTIVE: To conduct a preliminary analysis of SANS flight data to determine mission-to-mission trends in SANS diagnosis and optic disc edema (ODE) severity. DESIGN, SETTING, AND PARTI
