A continuous infusion of atracurium , at an average rate of 0.0061 +/- 0.0003 mg kg-1 min-1 for 128-233 min, to 12 anaesthetized patients undergoing prolonged surgical procedures, provided readily controllable neuromuscular blockade, and good operating conditions. When the infusion was discontinued, full spontaneous recovery from 90-100% blockade in a mean time of 42.2 +/- 3.1 min was as prompt as that after bolus administration of the drug. We examined the effects of atracurium and its breakdown product, laudanosine, on resting and stimulation-evoked release of 3H-noradrenaline (3H-NA) from sympathetic axon terminals of isolated right atria of guinea pigs. This report describes a life-threatening anaphylactic reaction in a 58-year-old woman who was scheduled for subacromial decompression of right shoulder joint. Few previous reports clarified that patients with an increased alpha 1 globulin fraction in their serum protein electrophoresis, will have resistance to the neuromuscular block of atracurium. Having a patient with diagnosis of Wegener's granulomatosis and a marked elevation of alpha 1 globulin fraction in her electrophoresis, we took this opportunity to monitor the neuromuscular block of atracurium. The data obtained, confirm previous studies showing that patients with a marked increase in alpha 1 globulin fraction may have a resistance to the action of atracurium.
