Ischemic stroke in younger people is common, and often remains unexplained. There is a well-documented association between unexplained stroke in younger people, and the presence of a patent foramen ovale. Therefore, in the absence of a clear cause of stroke, the heart is often assessed in detail for such lower risk causes of stroke. We report a patient who presented with paraprosthetic leak complicated by dissection of the interatrial septum after mitral valve replacement. A review of the literature provides confirmation that only 3 cases have been previously reported of this potential, albeit extremely rare, complication of prosthetic mitral valve replacement. Prosthesis oversizing and improper mitral annular handling appeared to be the predisposing factors of this complication.

Papillary fibroelastoma is rare but one of the most common benign primary cardiac tumors after myxoma. This lesion may be associated with embolization, angina and sudden death. The incidental finding of a small pedunculated papillary fibroelastoma arising from the atrial septum detected by transesophageal two-dimensional echocardiography (TEE) in a patient undergoing coronary artery bypass grafting is reported. The advantage of TEE in diagnosing intracardiac tumors is also described.

A heart is described with an atrioventricular septal defect in which the lower part of the atrial septum ("septum primum") was well developed and in which superior and inferior bridging leaflets were attached neither to the crest of the ventricular septum nor the inferior rim of the atrial septum. Diagnostic pre-operative angiographic and echocardiographic features and the method of surgical correction are outlined.

BACKGROUND: Pacing at the right atrial septum (RAS) appears to be superior to that performed at the right atrial appendage for preventing paroxysmal atrial fibrillation and for reducing complications. However, atrial lead insertion at the RAS is difficult and time-consuming. Although a few useful devices have been reported, handling of the atrial lead around the RAS is sometimes difficult even with those devices. BACKGROUND: Total anomalous pulmonary venous connection with intact interatrial septum is extremely rare.
AIMS: In these patients, a right to left shunt through a ventricular septal defect or a patent ductus arteriosus is mandatory to maintain the systemic circulation. Mechanical or physiological restriction of shunting through these pathways results in rapid clinical deterioration.
DISCUSSION: We describe the anatomical findings with surgical repair in one such 10-day-old baby.

A 44-year-old woman was referred to our centre for interventional cardiac catheterisation. The diagnostic work-up after a preceding ischaemic stroke led to the assumption of a patent foramen ovale due to a positive bubble study. This case presentation of a 73-year-old man introduces a unique tricuspid valve repair with "endocardium leafletization" for Ebstein anomaly. In this procedure, an atrialized portion of the right ventricular endocardium was delaminated, starting from the level of the true tricuspid annulus, to create a neoleaflet in continuity with the displaced true leaflet. We report an uncommon case of atrial septal aneurysm associated with mitral valve prolapse. A 28 year old woman was studied with transthoracic and transesophageal echocardiography (TEE). Transthoracic echocardiography suggested mitral valve prolapse. TEE with color mapping was performed. Atrial septal aneurysm and mitral valve prolapse was found. In 7/340 patients (2.1%) examined by transesophageal echocardiography, aneurysms of the intraatrial septum were observed. In no patient were additional shunts present. In 3/7 patients mitral valve and in 2/7 patients tricuspidal valve prolapse was found. Intraatrial septum aneurysms seem to have a higher frequency than that indicated by transthoracical echocardiography. The incidence observed by transesophageal echocardiography corresponds to the incidence of pathological findings.
