Pre- and postganglionic vasomotor dysfunction causes distal limb coldness in multiple system atrophy.

BACKGROUND: The detailed pathophysiology of limb coldness in multiple system atrophy (MSA) is unknown. METHODS: We evaluated cutaneous vasomotor neural function in 18 MSA patients with or without limb coldness, and in 20 healthy volunteers as controls. We measured resting skin sympathetic nerve activity (SSNA) and spontaneous changes of the sympathetic skin response (SSR) and skin blood flow (skin vasomotor reflex: SVR), as well as SVR and reflex changes of SSNA after electrical stimulation. The parameters investigated were the SSNA frequency at rest, amplitude of SSNA reflex bursts, absolute

Autonomic dysfunction in chronic inflammatory demyelinating polyradiculoneuropathy.

OBJECTIVES: Autonomic deficits in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) have not been adequately quantitated. The Composite Autonomic Severity Score (CASS) is a validated instrument for laboratory quantitation of autonomic failure derived from standard autonomic reflex tests. We characterized dysautonomia in CIDP using CASS. METHODS: Autonomic function was retrospectively analyzed in 47 patients meeting CIDP criteria. CASS ranges from 0 (normal) to 10 (pandysautonomia), reflecting summation of sudomotor (0-3), cardiovagal (0-3), and adrenergic (0-4) subscores. Sever

Neural control of the pineal gland.

The rhythm in melatonin production by the mammalian pineal gland is generated by the circadian timing system. The components of that system which mediate the function are: (1) visual projections through the retinohypothalamic tract to the circadian pacemaker, the suprachiasmatic nucleus (SCN) of the hypothalamus; (2) the SCN which generates a circadian signal transmitted by SCN projections to the parvocellular autonomic component of the paraventricular nucleus; (3) paraventricular nucleus projections to the upper thoracic intermediolateral cell column; (4) preganglionic sympathetic fibers to t

Overview of the Anatomy, Physiology, and Pharmacology of the Autonomic Nervous System.

Comprised of the sympathetic nervous system, parasympathetic nervous system, and enteric nervous system, the autonomic nervous system (ANS) provides the neural control of all parts of the body except for skeletal muscles. The ANS has the major responsibility to ensure that the physiological integrity of cells, tissues, and organs throughout the entire body is maintained (homeostasis) in the face of perturbations exerted by both the external and internal environments. Many commonly prescribed drugs, over-the-counter drugs, toxins, and toxicants function by altering transmission within the ANS.

Segmental distribution and central projections of renal afferent fibers in the cat studied by transganglionic transport of horseradish peroxidase.

The segmental and central distributions of renal nerve afferents in adults cats and kittens were studied by using retrograde and transganglionic transport of horseradish peroxidase (HRP). Transport of HRP from the central cut ends of the left renal nerves labelled afferent axons in the ipsilateral minor splanchnic nerves and sensory perikarya in the dorsal root ganglia from T12 to L4. The majority of labeled cells (85%) were located between L1 and L3. A few neurons in the contralateral dorsal root ganglia were also labeled. Labeled cells were not confined to any particular region within a dors

Effects of pre-ganglionic decentralization or post-ganglionic excision of the superior cervical ganglia on brain edema and heat stroke in rats.

The preventive effect of pre-ganglionic decentralization (Sympathetic trunk resectioN) or postganglionic excision (ganglionectomy) of the superior cervical ganglia on thermal injury induced brain edema or the development of heat stroke was assessed in rats. Brain edema was induced by cold or heat injury to the cortex in 24 rats. The results showed that decentralization, but not excision, of the superior cervical ganglia greatly inhibited the formation of brain edema which was subsequently induced. When heat stroke was induced by exposing 24 rats to an ambient temperature of 41 degree C, the la

The in vitro effects of vinblastine and vincristine on the responses of the guinea-pig vas deferens to nerve stimulation.

1. Neither vinblastine nor vincristine (1x10-4M) appeared to cause a specific blockade of transmission through the guinea-pig hypogastric ganglion, since responses to preganglionic nerve trunk stimulation were lost no sooner than responses to postganglionic nerve trunk stimulation. 2. The responses to postganglionic nerve trunk stimulation were blocked by vinblastine or vincristine at a time when responses to stimulation of intramural nerve fibres were little affected. 3. The results do not support the suggestion that inhibition of the exocytotic release of transmitter is responsible for the e

[Mechanisms of release of adrenaline and noradrenaline from the caudal mesenteric ganglion of dogs].

Adrenaline (A) and noradrenaline (N) release from the caudal mesenteric ganglion (CMG) during preganglionic nerve stimulation is due to acetylcholine action on N--and M-cholinoreceptors of ganglion's adrenergic structures (GAS) and probably, primarily on SIF--cells, whereas during peripheral reflexes of CMG it is related to the excitation of noncholinergic afferent neurons which affect the corresponding receptors of GAS. A and N release is inhibited via M-receptors. There is a complex relationship between receptor fields of GAS. Hemicholinium as well as N--cholinoblocking agents, during first

Postganglionic Sudomotor Dysfunction and Brain Glucose Hypometabolism in Patients with Multiple System Atrophy.

BACKGROUND: Sudomotor dysfunction is common in patients with multiple system atrophy (MSA). Postganglionic sudomotor dysfunction in MSA, which can be assessed using quantitative sudomotor axon reflex testing (QSART), results from the degeneration of preganglionic sympathetic neurons and direct loss of postganglionic fibers. OBJECTIVE: We investigate whether abnormal QSART responses in patients with MSA are associated with disease severity. METHODS: In this retrospective study, patients with probable MSA who underwent both 18F-fluorodeoxyglucose positron emission tomography/computed tomograph

Anatomically and physiologically based guidelines for use of the sphenopalatine ganglion block versus the stellate ganglion block to reduce atypical facial pain.

This literature review is designed to develop guidelines needed for the use of a sphenopalatine ganglion block versus a stellate ganglion block to reduce atypical facial pain. We have reviewed the basic anatomy of both ganglia and the physiological responses usually associated with each, and have given an opinion on appropriate use of these therapeutic modalities.

[A case of acute idiopathic pandysautonomia--a histochemical study of sural nerve by acetylcholinesterase staining].

A 30-year-old man had an acute onset of orthostatic lightheadedness, sweating disturbance, paroxysmal cough and loss of potency. These symptoms reached the peak in two weeks, and then remitted very slowly. He was admitted to our hospital for further evaluation when he was 39 years old. Neurological examinations revealed right Horner's syndrome, dry skin and impotence, but neither motor nor sensory system was impaired. No abnormalities were found on routine examinations of the blood and cerebrospinal fluid, motor and sensory nerve velocities, computed tomography and electroencephalography. On s

Autonomic dysfunction in fibromyalgia syndrome: the role of small fiber damage.

OBJECTIVE: Cardiovascular autonomic symptoms, particularly orthostatic intolerance, are frequent in fibromyalgia syndrome (FMS) and markedly contribute to disability, yet their underlying mechanisms remain unclear. Small fiber pathology (SFP), detected in 30-50% of patients with FMS, may also involve postganglionic sympathetic fibers. This study examined the contribution of SFP to autonomic dysfunction and cardiovascular autonomic symptoms in FMS. METHODS: We prospectively enrolled 43 patients with FMS reporting orthostatic intolerance and 20 age- and sex-matched healthy controls. All partici

[Mechanism of changes in secretory cell polarization in submandibular salivary glands during experimental botulism].

The polarization level (membrane potential-MP) of varied cells from the submandibular salivary gland was studied in different stages of botulinic poisoning in rats. The MP level of the cells decreased only in the late period (30 days) of local injury to the gland. However, pilocarpine stimulation of the secretory activity over the same period of time made MP return to normal. The authors believe this to be related to the phenomenon of gland decentralization since botulism is likely to entail an injury to the preganglionic parasympathetic neuron innervating the salivary gland, whereas the postg

Norepinephrine Reuptake Inhibition, an Emergent Treatment for Neurogenic Orthostatic Hypotension.

The NET (norepinephrine transporter) is situated in the prejunctional plasma membrane of noradrenergic neurons. It is responsible for >90% of the norepinephrine uptake that is released in the autonomic neuroeffector junction. Inhibitors of this cell membrane transporter, known as norepinephrine reuptake inhibitors (NRIs), are commercially available for the treatment of depression and attention deficit hyperactivity disorder. These agents increase norepinephrine levels, potentiating its action in preganglionic and postganglionic adrenergic neurons, the latter through activation of α-1 adrenorec

Hemodynamic Response to Massive Bleeding in a Patient with Congenital Insensitivity to Pain with Anhidrosis.

A patient with congenital insensitivity to pain with anhidrosis (CIPA) underwent revision of total hip arthroplasty under general anesthesia with only propofol. During surgery, neither elevation of stress hormones nor hemodynamic changes associated with pain occurred; however, when blood was rapidly lost, compensatory tachycardia was observed. Although patients with CIPA are complicated with autonomic disturbance due to dysfunction of postganglionic sympathetic fibers, this compensatory response indicated that the adrenal glands in patients with CIPA secrete catecholamine as part of a compensa

Autonomic Nerve Fibers Aberrantly Reinnervate Denervated Facial Muscles and Alter Muscle Fiber Population.

The surgical redirection of efferent neural input to a denervated muscle via a nerve transfer can reestablish neuromuscular control after nerve injuries. The role of autonomic nerve fibers during the process of muscular reinnervation remains largely unknown. Here, we investigated the neurobiological mechanisms behind the spontaneous functional recovery of denervated facial muscles in male rodents. Recovered facial muscles demonstrated an abundance of cholinergic axonal endings establishing functional neuromuscular junctions. The parasympathetic source of the neuronal input was confirmed to be

The "Harlequin Sign". Case description and review of the literature.

Asymmetrical facial sweating and flushing has been named the "Harlequin Sign". This is a rare feature, as evident from only 12 cases described up to date. The "Harlequin Sign" represents a local autonomic dysfunction due to a cervical sympathetic deficit located at the pre or postganglionic level on the non-flushing side. We observed slow onset and progression of the "Harlequin Sign" in a 19-year-old man, with preexisting slight miosis on the non-flushing side. The differential diagnosis included other forms of dysautonomia and a secondary origin of this partial Horner's syndrome. Both pupils

[Laboratory evaluation of autonomic function].

The sympathetic and parasympathetic systems both contain efferent and afferent neurons. The efferent pathway consists of preganglionic neurons with myelinated axons the cell bodies of which are within the central nervous system and postganglionic neurons with unmyelinated axons the cell bodies of which are peripheral. The preganglionic sympathetic fibers arise in the intermediolateral column of the thoracic and upper lumber cord segments, and reach the sympathetic chain whence postganglionic fibers are distributed to the whole body, where they have particular influence on the control of blood
