Azathioprine-induced atrial fibrillation.

Azathioprine, a purine analogue that competitively inhibits the biosynthesis of purine nucleotides, is used in a wide range of conditions. Although its side-effects are well known, cardiac side effects like paroxysmal atrial fibrillation (AF) are based on only a few case reports. We describe here the case of a 55-year-old woman with primary biliary cirrhosis who presented a first-detected, symptomatic AF 2 h after azathioprine therapy which resolved after discontinuation of the drug with no predisposing factors for supraventricular arrhythmias (systemic hypertension, diabetes or coronary arter…

Azathioprine-Induced Rapid Hepatotoxicity.

Objective: To report a case of rapid hepatotoxicity seen within days of initiating azathioprine therapy as a possible adverse reaction. Case Summary: A 62-year-old female with Crohn's disease was being maintained on occasional oral prednisone when more aggressive therapy became indicated. The patient was started on azathioprine and continued a prednisone taper. Liver enzymes became elevated by the third day of therapy. Peak liver enzyme values on day 10 of therapy were aspartate aminotransferase 119 U/L, alanine aminotransferase 210 U/L, and alkaline phosphatase 460 U/L. The patient did not de…

Azathioprine-associated interstitial pneumonitis.

Seven renal allograft recipients taking azathioprine (Imuran) for immunosuppression developed bilateral pulmonary infiltrates and a falling pO2 that did not respond to antibiotic therapy. Open lung biopsies revealed changes ranging from diffuse alveolar damage (DAD) to usual interstitial pneumonia (UIP) culminating in pulmonary fibrosis. There was no evidence of immune deposits, eosinophilia, vasculitis, granulomas, or microorganisms by cultures and appropriate stains. Following discontinuance of Imuran, the two cases with DAD revealed a significant clearing of the lung infiltrates, whereas fo…

Azathioprine Withdrawal Is Cost-Effective in Patients with Crohn's Disease in Remission on Infliximab and Azathioprine.

BACKGROUND: In Crohn's disease, combination therapy with infliximab and azathioprine is more effective than either drug alone but is associated with a higher risk of therapy-related complications. Though therapy de-escalation can reduce risks and save costs, it is associated with a risk of Crohn's disease relapse. AIMS: We aimed to study the cost-effectiveness of de-escalation strategies in Crohn's disease patients in remission on infliximab and azathioprine. METHODS: We constructed a decision tree with Markov models for continuation of infliximab and azathioprine, discontinuation of azathio…

Azathioprine: its uses in dermatology.

This is a narrative review of azathioprine. This medication is immunomodulatory and immunosuppressive, and it has been used widely through different medical specialties to modify disease. It has been proven useful for several dermatoses and it has encountered success when used as an off-label indication for other dermatologic diseases. Its mechanism of action is described thoroughly, as well as precautions for monitoring adequate levels in patients using it. Dermatologists should also be aware of the possible adverse events it may present. In dermatology it can be used in bullous and autoimmun…

Neutrophilic dermatosis after azathioprine exposure.

IMPORTANCE: Azathioprine hypersensitivity syndrome can present clinically and histopathologically like Sweet syndrome. Shared clinical features include fever, constitutional symptoms, prompt response to systemic corticosteroid therapy, neutrophilia, and abrupt onset of erythematous cutaneous lesions. Histologically, both azathioprine hypersensitivity syndrome and Sweet syndrome are rich in neutrophils. OBSERVATIONS: An 81-year-old woman with Crohn disease presented with fever and an acute eruption of plaques on her extremities within 2 weeks of starting treatment with azathioprine. Laboratory…

Azathioprine for primary biliary cirrhosis.

BACKGROUND: Azathioprine is used for patients with primary biliary cirrhosis, but the therapeutic responses in randomised clinical trials have been conflicting. OBJECTIVES: To assess the benefits and harms of azathioprine for patients with primary biliary cirrhosis. SEARCH STRATEGY: Randomised clinical trials were identified by searching The Cochrane Hepato-Biliary Group Controlled Trials Register, the Cochrane Central Register of Controlled Trials (CENTRAL) in The Cochrane Library, MEDLINE, EMBASE, Science Citation Index Expanded, The Chinese Biomedical Database, and LILACS, and manual sear…

[Azathioprine and its side effects].

The clinical signs and symptoms of a patient induced us to consider critically the so far known side effects of azathioprine. The literature concerning 643 patients treated with azathioprine reports on reactions as changes in blood count, gastro-intestinal complaints, intrahepatic cholestasis, acute pancreatitis, and leukemia. Irreversible side effects occurred in four cases.

Azathioprine-induced fever in sarcoidosis.

OBJECTIVE: To report a case of drug-induced fever associated with azathioprine treatment in a patient with sarcoidosis. CASE SUMMARY: A 52-year-old man with pulmonary sarcoidosis presented to the emergency department with a 1-day history of fever (temperature 39.9 °C), chills, nausea, and vomiting. One week earlier, azathioprine 50 mg/day had been started for worsening dyspnea. The patient was admitted and evaluated for acute infectious processes. All of his home medications (hydroxychloroquine, prednisone, fluticasone/salmeterol, lovastatin, pantoprazole, zolpidem, ibandronate, albuterol), e…

Azathioprine for ocular inflammatory diseases.

PURPOSE: To evaluate treatment outcomes of azathioprine for noninfectious ocular inflammatory diseases. DESIGN: Retrospective cohort study. METHODS: Medical records of 145 patients starting azathioprine as a sole noncorticosteroid immunosuppressant at 4 tertiary uveitis services were reviewed. Main outcome measures included control of ocular inflammation, sustained control after tapering prednisone to </= 10 mg/day, and discontinuation of treatment because of side effects. RESULTS: Among 145 patients (255 eyes) treated with azathioprine, 63% had uveitis, 23% had mucous membrane pemphigoid, …

Fatal myelotoxicity after azathioprine treatment.

Azathioprine and 6-mercaptopurine have been used for many years in the treatment of inflammatory bowel disease. Approximately 0.3% of the population are homozygous for variant alleles associated with extremely low thiopurine S-methyltransferase enzyme activity. We describe the case of a young patient with ulcerative colitis, homozygous for TPMT*3A alleles, who suffered fatal azathioprine-induced myelotoxicity after standard dosing with azathioprine. Screening for decreased activity of TPMT in patients prior to azathioprine treatment is advised to minimize the risk of drug-induced toxicity.

Azathioprine in the management of autoimmune uveitis.

PURPOSE: To evaluate the role of azathioprine as an immunosuppressive for steroid-resistant autoimmune uveitis (AIU). METHODS: Patients using oral prednisolone for active AIU without clinical response were recruited. A standard protocol of oral prednisolone and azathioprine was used. RESULTS: Twenty-seven patients participated in the study: 3 with anterior uveitis, 1 pars planitis, 4 idiopatic panuveitis, 8 Vogt-Koyonagi-Harada syndrome, 3 Behcet disease, and 8 choroidoretinopathies. Complete response was observed in 92%. Eleven patients had well-tolerated minor side effects. CONCLUSION: Az…
