Infections caused by Cunninghamella bertholletiae are rare but severe. Only 32 cases have been reported as yet, but in 26 of these this species was a contributing cause of the death of the patient. This opportunistic mould in the order Mucorales infects immunocompromized patients suffering from haematological malignancies or diabetes mellitus, as well as solid organ transplant patients. The lung is the organ most often involved. Two cases of primary cutaneous infection have been previously reported subsequent to soft-tissue injuries. We report a case of primary cutaneous C. bertholletiae zygom…

Two cases of disseminated systemic zygomycosis caused by Cunninghamella bertholletiae in compromised persons were studied. The underlying disease in one patient was hematopoietic dysplasia (preleukemic syndrome) and in the other, postnecrotic hepatic cirrhosis. A definitive diagnosis of zygomycosis caused by C bertholletiae is achieved only by isolation and identification of the fungus and by demonstration of tissue invasion.

After closed reduction and casting of a tibial fracture in a 69-year-old diabetic man, a gangrenous lesion developed under the cast. Cultures of debrided tissue revealed multiple bacteria, including Bacillus sp, plus two fungi, Fusarium sp and a zygomycete identified as Cunninghamella bertholletiae. Large nonseptate hyphae in thrombosed blood vessels suggested that C bertholletiae played a major role in the development of the infection. Recovery of organisms from our patient's wound and from the cast padding suggests that the wound infection was acquired from nonsterile dressings.

An adult female killer whale (Orcinus orca) was transported to the Port of Nagoya public aquarium in June 2010. While the animal was being m … he hyphae demonstrated immunohistochemical cross-reactivity with Rhizomucor spp. and Cunninghamella bertholletiae was cultured. Bacteriological culture revealed the presence of Proteus mirabilis, Pseudomonas aeruginosa and Pseudomonas oryzihabitans. This case represents the first documentation of zygomycosis associated with C. bertholletiae in a marine mammal.

Cunninghamella bertholletiae is a rare cause of pulmonary mucormycosis. We describe a cluster of invasive pulmonary infections caused by C. bertholletiae in 4 immunocompromised patients that occurred during a 2-year period at 1 center. Three of the patients were receiving antifungal prophylaxis with itraconazole. Presenting symptoms were fever unresponsive to antibacterial chemotherapy, hemoptysis, and infiltrates on chest radiograms. Three patients were treated with liposomal amphotericin B. Only 1 patient survived.

A case of pneumonia caused by Cunninghamella bertholletiae is described in a patient with chronic lymphatic leukemia. The species of Cunninghamella, a genus in the order Mucorales, are characterized by the formation of conidia on the surface of an inflated conidiophore (vesicle). C. bertholletiae, not C. elegans, is the most appropriate name for this fungus. This zygomycete was resistant by in vitro sensitivity testing to amphotericin B. The source of the infection is not known.

Pulmonary mucormycosis occur in immunosuppressed patients with a neutropenic treatment or a malignant neoplasia. Although the histological study alone cannot enable species identification, the pathologist usually makes the diagnosis of mucormycosis and can eliminate other filamentous mycoses such as aspergillosis or scedosporiosis. Pulmonary infection caused by Cunninghamella bertholletiae is a very rare lung mucormycosis, particularly in young patients. The histological features of this filamentous mycosis are unusual making the diagnosis of mucormycosis difficult. Mycological study is then c…

Human infection by Cunninghamella bertholletiae occurs almost exclusively in immunocompromised patients. Infections due to this microorganism have been most frequently diagnosed in patients with hematological malignancies, with neutropenia and in diabetes mellitus patients. This work reports a case of fungal infection by Cunninghamella bertholletiae isolated from blood in a man with a complex clinical picture, involving diabetes and pharmacological immunosupression. Blood culture at room temperature and at 37 degrees C on Sabouraud agar grew a single mold with characteristic properties of Cunn…

The present study determined the hepatoprotective and antioxidant potential of the extract obtained from Amazon nut residues. The brown walnut shell of Bertholletia excelsa was collected and extracted sequentially for 48 h with different ethanol:water ratios and the dry extract was obtained by the spray dryer method. Antioxidant activities were evaluated by testing DPPH radicals, ABTS, total phenolics, flavonoids and cellular antioxidant. Subsequently, in vitro and in vivo tests were carried out to evaluate the protective effect of the extract after induction of liver damage with CCL4. Biochem…

A 22-year-old Japanese woman was diagnosed with hemophagocytic lymphohistiocytosis and subsequently was treated with etoposide and cyclophosphamide. On Day 22, multiple nodular lesions appeared in the bilateral lungs. Neither the administered antibiotics nor the antifungal agent were effective, and she died suddenly of respiratory failure on Day 35. An autopsy revealed disseminated zygomycosis and a pulmonary infarction due to the embolization of an angioinvasive fungus, which was later identified as Cunninghamella bertholletiae using in situ hybridization of 18S rRNA. C. bertholletiae is aggr…

Zygomycosis is an invasive mycotic disease caused by fungi of the class Zygomycetes. Infections caused by zygomycetes, are known for their difficulty of diagnosis and treatment. Cunninghamella bertholletiae (Cb), is a saprobic fungus commonly found in the soil of temperate climates. Pulmonary infections caused by Cb are being identified with increasing frequency among patients on immunosuppressive therapy, and these infections usually have a fatal outcome. We present a rare case of pulmonary zygomycosis caused by Cb in a 10 year old male child with acute lymphoblastic leukemia. In spite of int…

Pediatric infections caused by Cunninghamella bertholletiae (C. bertholletiae) are exceedingly rare with only few published case reports. Herein, we describe five cases of C. bertholletiae diagnosed and treated in Pediatric Hematology-Oncology Patients. These cases indicated that pediatric C. bertholletiae infection presents with non-specific clinical features, is associated with high fatality, and frequently involves multiple anatomical sites, most commonly the lungs, demonstrating epidemiological patterns distinct from those observed in adults. From a diagnostic perspective, mNGS enables rap…

Mucormycosis is an uncommon fungal infection, which generally develops in immunosuppressed hosts. In particular, pulmonary infection by Cunninghamella bertholletiae, a rare species of Mucor, is characterized by invasiveness and high mortality. Herein a case of pulmonary mucormycosis due to C. bertholletiae in a female patient with chronic renal insufficiency, secondary to microscopic polyarteritis, is reported. The patient survived after successful treatment with a cumulative dose of 1508 mg of amphotericin B, phased reduction of glucocorticoid therapy and chest tube drainage of a pneumothorax…
