Gene Therapy Approaches to Biological Pacemakers.

Bradycardia arising from pacemaker dysfunction can be debilitating and life threatening. Electronic pacemakers serve as effective treatment options for pacemaker dysfunction. They however present their own limitations and complications. This has motivated research into discovering more effective and innovative ways to treat pacemaker dysfunction. Gene therapy is being explored for its potential to treat various cardiac conditions including cardiac arrhythmias. Gene transfer vectors with increasing transduction efficiency and biosafety have been developed and trialed for cardiovascular disease

Impact of biological therapy on spondyloarthritis.

PURPOSE: In this review, we focus on the clinical and radiological aspects related to the biological therapy of ankylosing spondylitis (AS), axial spondyloarthritis (SpA) and psoriatic arthritis (PsA). METHODS: A review was carried out in the main medical databases to evaluate the available literature. RESULTS: Even if there is emerging interest for the role of biological agents other than tumour necrosis factor (TNF)-α inhibitors in spondyloarthritis, anti-TNF-α treatment is currently the only effective therapy for patients in whom conventional therapy with non-steroideal anti-inflammatory …

Therapy of myositis: biological and physical.

PURPOSE OF REVIEW: To give an update on reported use and effects of biological and physical therapies in patients with myositis. RECENT FINDINGS: The most promising biological treatment in polymyositis, dermatomyositis and juvenile dermatomyositis is B-cell blockade by rituximab. Anti-Jo or anti-Mi-2 antibodies were predictors of response suggesting different molecular pathways in different subsets of myositis. T-cell blockade with abatacept is a new possibility, as is blockade of interleukin-1, interleukin-6 or type I interferon, but controlled studies are needed. Metabolic abnormalities may

Future for biological therapy for uveitis.

PURPOSE OF REVIEW: To review biological therapies as they pertain to the treatment of inflammatory eye diseases, especially uveitis. RECENT … itional cytokines, cell surface markers, adhesion molecules, and accessory molecules are targets of biological therapy, but the relevance of these targets in eye inflammation is sometimes just theoretical. And any disruption of the immune response entails potential risk. SUMMARY: Biological therapies offer tremendous potential in the treatment of ocular inflammatio …

[Mastocytosis, classification, biological diagnosis and therapy].

In mast cell (MC) disorders (mastocytosis), clinical symptoms are caused by the release of chemical mediators from MCs, the pathologic infiltration of neoplastic MCs in tissues, or both. Cutaneous mastocytosis is a benign disease in which MC infiltration is confined to the skin. In pediatric cases cutaneous mastocytosis might regress spontaneously. Systemic mastocytosis (SM) is more frequently diagnosed in adults and is a persistent (clonal) disease of bone marrow-derived myelomastocytic progenitors. The somatic c-kit mutation D816V is found in the majority of such patients. The natural clinic

Biological therapy of autoimmune blistering diseases.

INTRODUCTION: Autoimmune blistering skin diseases are a group of disorders subdivided according to the location of blister formation: intraepidermal blistering in the pemphigus group and subepidermal in the pemphigoid group. These conditions are clinically heterogeneous and are treated with systemic corticosteroids and/or other forms of immunosuppression on the basis of clinical subtype and disease severity. These approaches may not be effective for the induction and maintenance of clinical response or need to be stopped because of intolerable side effects. AREAS COVERED: Biological therapies

Biological therapy of salivary duct carcinoma.

BACKGROUND: The incidence of human epidermal growth factor receptor 2 positivity in salivary duct carcinoma ranges from 25 to 100 per cent and is associated with a poor outcome. Salivary duct carcinoma has significant pathological similarities to ductal carcinoma of the breast. METHODS AND RESULTS: A 49-year-old man developed lung and liver metastasis a few months after surgery and adjuvant radiotherapy for salivary duct carcinoma of the parotid gland. There was no response to palliative chemotherapy with doxorubicin. We followed the biological model of breast cancer, whereby two-thirds of hu

[Severe asthma and biological therapies].

Severe asthma is a relatively common, chronic, heterogeneous inflammatory disease affecting around 4-10% of all asthmatics. Most of these have uncontrolled eosinophilic asthma, the treatment of which accounts for >50% of healthcare costs attributed to the disease. Management of severe asthma requires a thorough investigation to exclude comorbidities and determine the underlying type of inflammation and resulting asthma phenotype, so that suitable treatments can be chosen. Systematic assessment and evaluation of treatment must then be performed every 3-6 months. Severe asthma is mostly driven b

[Biologic therapies in adult asthma].

BIOLOGIC THERAPIES IN ADULT ASTHMA. The range of therapeutic options for asthma has greatly expanded in recent times, particularly with the advent of biologic therapies. These cutting-edge treatments are specifically designed for patients suffering from severe, treatment-resistant asthma, even when all contributing factors are optimally managed. In France, there are now five available biologic therapies, each utilizing monoclonal antibodies that specifically target crucial components of the T2-type inflammatory pathways in the respiratory system. Clinical trials and real-world studies have val

Emerging biologic therapies for hypercholesterolaemia.

LDL-cholesterol (LDL-C) is one of the most well-established risk factors for CV disease. Indeed, therapies that decrease LDL-C are proven to effectively reduce the risk of atherosclerotic CV disease. Monoclonal antibodies (mAbs) that target proprotein convertase subtilisin/kexin type 9 (PCSK9) have recently gained traction as a promising therapeutic strategy. Areas covered: In this review, the authors discuss the effectiveness of mAbs against PCSK9 in lowering low-density lipoprotein cholesterol (LDL-C) and other atherogenic lipid fractions. The discontinuation in the development of bococizuma

Biologic therapy in the management of asthma.

PURPOSE OF REVIEW: Current asthma management relies on inhaled corticosteroids, but some asthma is not well controlled with inhaled steroids alone or in combination with long-acting bronchodilators or leukotriene pathway inhibitors. The field of biologic therapy has grown dramatically in the past two decades, with current availability of three molecules, with two distinct and highly selective approaches to interfering with the allergic and eosinophilic airway inflammation common to most asthma. This review summarizes current and future options for incorporating biologic therapy into the overal

Biologic therapy and risk of infection.

Biologic compounds are being used more frequently to treat a multitude of systemic inflammatory conditions. These novel compounds are composed of antibodies or other peptides that act through one of three mechanisms: inhibiting inflammatory cytokine signaling (typically tumor necrosis factor or TNF), inhibiting T-cell activation, or depleting B-cells. The increase in use and ever expanding list of new immune modulating therapies make knowledge of the infectious complications associated with immune modulation even more important. Of particular concern is the risk for developing atypical and opp
