Acute Blindness.

Sudden loss of vision is an ophthalmic emergency with numerous possible causes. Abnormalities may occur at any point within the complex vision pathway, from retina to optic nerve to the visual center in the occipital lobe. This article reviews specific prechiasm (retina and optic nerve) and cerebral cortical diseases that lead to acute blindness. Information regarding specific etiologies, pathophysiology, diagnosis, treatment, and prognosis for vision is discussed.

Acute blindness.

A 15-year-old man presenting with cortical blindness as the initial symptom of mitochondrial encephalomyopathy, lactic acidosis and stroke-like episodes (MELAS) is reported. He showed fluctuating consciousness and severe occipital headache with nausea and vomiting. T2 and diffusion-weighted magnetic resonance imaging showed high signal intensity in the occipital lobes. Electroencephalography showed diffuse sharp waves with focal epileptic discharges over the posterior region. The nature of stroke-like episodes and seizure mechanisms is unexplained in MELAS. Consequently, the possible mechanism…

Measles blindness.

Measles remains a major problem in developing countries, where it affects an estimated 30 million children a year and causes up to one million deaths annually. Measles blindness is the single leading cause of blindness among children in low income countries, accounting for an estimated 15,000 to 60,000 cases of blindness per year. There is a close synergism between measles and vitamin A deficiency that can result in xerophthalmia, with corneal ulceration, keratomalacia, and subsequent corneal scarring or phthisis bulbi. High-dose oral vitamin A supplementation is recommended for all children w…

Postoperative blindness.

This chapter discusses the cases of postoperative blindness reported in the literature and the theories that attempt to explain the mechanisms involved. Although uncommon, alterations in vision and blindness after anesthesia for major surgical procedures, particularly cardiopulmonary bypass or spine surgery, are well documented, with an incidence varying between 0.05% and 1%. Accurate incidence data are unavailable because it is not known what percentages are reported. However, the large number of case reports over many years has provided some significant information. Although sustained compre…

[Sudden blindness].

Blindness means no light perception. In that case one observes a fixed dilated pupil. An unilateral amaurosis of sudden onset is caused either by an ocular or an optic nerve condition. The most common causes are: --occlusion of the central retinal artery --apoplexy of the papilla --traumatic lesions to the optic nerve Bilateral blindness occurring suddenly is for anatomic reasons due to a chiasmatic lesion. It is mainly associated with pituitary apoplexy. More commonly bilateral blindness develops sequentially and may be discovered only after the second event. Besides other ocular diseases bil…

Childhood blindness.

Eighty five visually handicapped children who attended the eye department were assessed. The children had a mean age of 3.6 years--range 0.3-14.7 years. The aetiological factors for severe visual handicap (visual acuity less than 3/60) were evaluated into prenatal factors; factors operating at birth and the immediate perinatal period; or factors operating in childhood. Genetic counselling and improvements in the management of pre-term infants is recommended in the reduction of childhood blindness. A national computerised childhood blind register should be established.

Onchocerciasis--river blindness.

Onchocerciasis results from infestation by the nematode Onchocerca volvulus and is characterized by troublesome itching, skin lesions, and e … cercal ocular disease covers a large spectrum of manifestations, which in severe cases, may lead to blindness. Diagnosis is usually made by direct visualization of the larvae emerging from superficial skin biopsies, "skin snips." In some cases, the microfilariae can also be directly observed at the slit lamp when migrating into the anterior chamber of the e …
