Subchondroplasty: Treatment of Bone Marrow Lesions in the Lower Extremity.

Bone marrow lesions are associated with pain, disease progression, and cartilage loss in inflammatory and noninflammatory conditions, and are related to mechanical loading and subchondral stress. Treatment, particularly in the foot and ankle, is challenging. In the subchondroplasty procedure, flowable, synthetic, calcium phosphate bone filler is injected into the defect region, improving subchondral bone integrity and allowing remodeling back into healthy cancellous bone. The procedure is a promising treatment option for bone marrow lesions, particularly in the foot and ankle. The benefits are

Single bone straight line graphs for the lower extremity.

Valuable information about growth and growth prediction in the lower extremity has been provided in the past by Anderson and Green, Moseley, and Menelans. Greater patient expectations and advanced techniques for lower extremity lengthening require more precise information regarding the growth characteristics of each long bone. A simple method for predicting growth is presented for the femur and tibia. Straight line graphs similar to one described previously by Moseley for the entire lower extremity have been drawn separately for the femur and tibia. Information from a contemporary population a

Curettage with cement augmentation of large bone defects in giant cell tumors with pathological fractures in lower-extremity long bones.

BACKGROUND: Thorough curettage and cement augmentation is the procedure of choice for treating giant cell tumor lesions, particularly those associated with large defects. Its association with pathological fractures has not been studied to a great extent, although a pathological fracture following a giant cell tumor is not a contraindication to treatment by curettage and cementation. We present our experience of bone cementation following intralesional curettage for treatment of giant cell tumors of the long bones of lower limbs with associated pathological fractures. MATERIALS AND METHODS: A

A Case of Extranodal Metastasis of Primary Bone Lymphoma in the Lower Extremity.

Primary bone lymphoma is a rare disease, accounting for less than 5% of all extranodal lymphoma. Although the femur is cited as being the most common site, primary bone lymphoma is rare, accounting for less than 1% of all non-Hodgkin's lymphoma. Herein we present a case of diffuse B-cell-type malignant lymphoma manifested as a large soft-tissue mass of the leg, via metastasis of primary non-Hodgkin's lymphoma of the femur, which went untreated. We highlight the advantages of various imaging modalities used throughout the process of diagnosis and treatment because accurate and early diagnosis a

[Treatment principles in bone reconstruction and limb lengthening of the lower extremity].

Limb length discrepancies, axis deviations and bone loss on a congenital, traumatic or infectious basis are treated with restoration of axis and bone elongation. The intention is to improve gait and function, to relieve pain caused by imbalance in the musculoskeletal system and to prevent degenerative joint disease. Modern motorized intramedullary nails spare the patients for external fixators, and thus complications are being reduced. X-ray, CT-based planning and a thorough clinical examination is essential for a good result. Complications are frequent, and treatment can be lengthy. Modern te

Radiographic reconstruction of lower-extremity bone fragments: a first trial.

PURPOSE: The correct rotational alignment of the proximal and the distal bone fragments is an essential step in a long-bone deformity correction process. In order to plan the deformity correction, plain radiographs are conventionally used. But as three-dimensional information of the complex situation is not available, the correct amount of rotation can only be approximated. Thus, the objective of this study was to develop a system to assess the rotational relationship between the proximal and distal fragments of a long bone (tibia or femur) based on a set of two calibrated X-ray radiographs.

Dual-fibular grafting for massive bone gaps in the lower extremity.

Fifty-two patients who had a tumor in the lower extremity (thirty-seven, a giant-cell tumor and fifteen, a small osteosarcoma that had not metastasized) had dual-fibular strut-grafting to bridge the osseous gap resulting from excision of the tumor. The size of the gap ranged from nine to twenty-four centimeters. The average time until union ranged from ten to twenty months. The ipsilateral fibula was used in thirty-six patients and both fibulae, in the remaining sixteen patients. Immediate reimplantation and adequate fixation of the fibular graft to the proximal and distal ends of the bone to

Prosthetic replacement of tumour-destroyed diaphyseal bone in the lower extremity.

A description is given of a prosthesis for replacement of tumour-destroyed diaphyseal bone in the lower extremity. It can be used in a salvage procedure and as an alternative to intramedullary nailing or plating procedures. The method is not complicated provided that mechanically sound devices are used and technical errors are avoided. The operation time is short and the patients can be mobilized early.

Loading Enhances Glucose Uptake in Muscles, Bones, and Bone Marrow of Lower Extremities in Humans.

CONTEXT: Increased standing time has been associated with improved health, but the underlying mechanism is unclear. OBJECTIVES: We herein investigate if increased weight loading increases energy demand and thereby glucose uptake (GU) locally in bone and/or muscle in the lower extremities. METHODS: In this single-center clinical trial with a randomized crossover design (ClinicalTrials.gov ID, NCT05443620), we enrolled 10 men with body mass index between 30 and 35 kg/m2. Participants were treated with both high load (standing with weight vest weighing 11% of body weight) and no load (sitting)

Progressive bone and joint abnormalities of the spine and lower extremities in cerebral palsy.

Bone and joint changes in cerebral palsy result from muscle spasticity and contracture. The spine and the joints of the lower extremity are most commonly affected. Scoliosis may progress rapidly and may continue after skeletal maturity. Increased thoracic kyphosis and lumbar lordosis, spondylolisthesis, spondylolysis, and pelvic obliquity may accompany the scoliosis. Progressive hip flexion and adduction lead to windswept deformity, increased femoral anteversion, apparent coxa valga, subluxation, deformity of the femoral head, hip dislocation, and formation of a pseudoacetabulum. In the knee,

Reconstruction of soft tissue and bone defects in lower extremity with free flaps.

This article reports our experiences treating soft tissue and bone defects in the lower extremity with free flaps. One of the most common causes for lower extremity wounds are high-energy injuries. These kinds of injuries contain soft tissue and bone defects beyond neurovascular complications. The rate of infection and nonunion is very high in these injuries. Between 1988 and 1996, we applied 33 flaps to 28 patients. The numbers and kinds of the free flaps are as follows: 12 latissimus dorsi, nine radial forearm, seven lateral arm, three vascularised fibula grafts with skin, one gracilis, and

Pediatric obesity and traumatic lower-extremity long-bone fracture outcomes.

BACKGROUND: Pediatric obesity is associated with lower-extremity injuries and poor outcomes after blunt trauma. Our aim was to determine if obese pediatric patients with femur and tibia fractures have more severe injury patterns and worse outcomes compared with those of nonobese patients. METHODS: We performed a retrospective cohort study of obese and nonobese pediatric patients with femur or tibia fractures treated at two Level I trauma centers from 2004 to 2010. Patients weighing 95th percentile or greater for age and sex were classified as obese. Patients were compared regarding demographi

[Surgical management of bone metastases of the lower extremity with AO interlocking nail].

Between January 1991 and June 1995 we have operated on 19 patients (9 male, 10 female) with 22 skeletal metastases of the lower limb (19 femora, 3 tibiae) using a static interlocking nail. Closed intramedullary nailing without resection of the metastasis has been established as our standard procedure. We have stabilized 15 patients with advanced osteolysis and seven pathological fractures. Sixteen patients underwent postoperative local radiation therapy with 40 Gy. As intraoperative complications we have observed one fracture of an osteolysis and one death due to fat embolism. Postoperatively
