Few models of experimental brain stem surgery exist in the literature. Often, experiments were devised in the animal laboratory after a similar surgical procedure already had been attempted on human subjects. The brain stems of 24 mental hospital patients and 10 control patients were examined. Ten mental hospital patients were either deluded or auditorily hallucinated in clear consciousness, and of this group 8 presented with a diagnosis of schizophrenic syndrome. The nature and significance of lesions found in the midline reticular nuclei and the trigeminal n. are discussed. The results, and the dearth of other published material on this subject, indicate the need for further work in this field.

This report describes a study of brain stem blood flow (BBF) change under various grades of brain stem ischemia in a new experimental rat model. The main damage was caused by occlusion of the median and paramedian perforating arteries of the basilar artery. In this model, hyperperfusion was generally observed in cases of mild or moderate ischemia within 1 hr after recirculation and lasted for approximately 1 hr. Brain-stem auditory evoked potentials (BAEPs) were recorded in 23 children who had signs of brain-stem or cerebellar dysfunction. In patients with brain-stem gliomas, BAEPs were abnormal in all except one, in whom involvement of the brain-stem auditory pathway was limited to the midbrain tectum. The BAEPs were normal in neuronal ceroid lipofuscinosis, but abnormal bilaterally in inheritable leukoencephalopathies. A 67-year old woman having intractable chronic postherpetic neuralgia at the neck to forearm for two years was treated with subarachnoid block. Initially, bolus of 1% lidocaine 8 ml, with methylprednisolone acetate 20 mg was injected intrathecally at 6th cervical intervertebral space. Paroxysmal symptoms of frequent recurrence and short duration occurring mostly unilaterally and without loss of consciousness have been described under a vast variety of headings. Brain stem origin of these symptoms was presumed.
