Indications for His bundle recordings.

While the extrapolation of data derived from His bundle studies performed over the past 10 years has lessened the need to apply the technique for diagnostic clinical purposes, His bundle recordings remain a useful clinical research tool for better understanding of cardiac electrophysiology. Some of the areas of continued research to which the technique will most certainly be applied are: (1) study of reentrant phenomena involving the atrial, junctional, and ventricular regions of the heart, (2) investigation of the mechanisms of action of new and old drugs on the heart, (3) evaluation of effec

[Indications for His bundle and left bundle branch pacing].

His bundle pacing (HBP) allows ventricular excitation through the entire cardiac conduction system, resulting in a better synchronicity and efficacy of contraction compared to myocardial pacing. Due to better, dedicated implantation tools and exact practical implantation recommendations, HBP has developed into a form of stimulation that can be successfully applied with reasonable time and effort in >90% of patients. The rate of lead dislodgement and threshold increase is similar to conventional pacemaker systems. Despite a rather weak data base and a paucity of randomized trials, HBS represent

Deep Negative Deflection in Unipolar His-Bundle Electrogram as a Predictor of Excellent His-Bundle Pacing Threshold Postimplant.

Background His-bundle pacing (HBP) is a physiological form of pacing. Although high capture thresholds are common, few predictors of low HBP threshold have been determined. We aimed to identify electrophysiological predictors. Methods Fifty-one patients (53% with atrioventricular block) underwent HBP for bradycardia with an intrinsic QRS duration of <120 ms. Attempts to anchor the HBP lead were guided by unipolar His-bundle electrograms (HB EGMs) recorded with an electrophysiology recording system. Patients were followed-up for >6 months. Results In total, 153 attempts at anchoring the HBP lea

Long-Term Results of His Bundle Pacing.

Right ventricular pacing is associated with pacing-induced cardiomyopathy in some patients. His Bundle Pacing (HBP) is an alternative site to pace to achieve ventricular contraction with fewer adverse hemodynamic effects. HBP has been shown to be safe and feasible in the short term. The few studies that look at long-term results of HBP are promising with regard to electrophysiological, echocardiographic, and clinical outcomes. Further randomized clinical trials are needed to fully understand the long-term effects of HBP.

Comparison of Left Bundle Branch and His Bundle Pacing in Bradycardia Patients.

OBJECTIVE: The aim of this study was to assess pacing and electrophysiological parameters, as well as short-term outcomes, among patients undergoing left bundle branch pacing (LBBP) or His bundle pacing (HBP). BACKGROUND: There are limited data directly comparing different conduction system pacing modalities. METHODS: Consecutive patients undergoing de novo conduction system pacing for bradycardia indications were evaluated. Procedural and fluoroscopic times and pacing characteristics were compared between groups at implantation and at 3-month follow-up. RESULTS: This study included 251 sub

Reversal of left bundle branch block-induced cardiomyopathy by His bundle pacing.

A 61-year-old woman was referred to our institution for evaluation of severe nonischemic dilated cardiomyopathy and left bundle branch block (LBBB). After permanent His bundle pacing, the LBBB was immediately corrected; however, the right bundle branch was injured during the procedure. Subsequent recovery of the right bundle branch block and normalization of heart function were observed during follow-up. This case indicates that LBBB might result in the development of nonischemic cardiomyopathy and emphasizes the necessity of a temporary pacemaker during His bundle pacing for patients with LBB

Drowning of a girl with anomaly of the bundle of His and the right bundle branch.

We report an unusual case of a 17-year-old girl who died in an filled bathtub. There were at least two attacks of unconsciousness in her premortem history. In April 1994, electrocardiography had shown an incomplete right bundle branch block. At autopsy, unspecific signs of drowning and hypoplasia of the terminal part of the atrioventricular bundle of His and the right bundle branch within the cardiac conduction system were found. There were no other obvious autopsy findings related to the sudden death of this girl. In addition, toxicologic examination for illegal drugs and alcohol was complete…

His bundle pacing: troubleshooting at implantation.

His bundle pacing produces electrocardiograms (ECGs) that give the impression that it is physiological like no other form of cardiac pacing requiring ventricular stimulation. Several special features of the implantation technique of a His bundle lead that distinguish it significantly from the implantation of other endocardial leads need to be considered. This overview provides a number of practical "hands-on" tips and tricks, from implantation set-up (12-lead ECG, no electrophysiology unit, no His bundle catheter), venous access, handling of delivery catheters and leads, interpretation of intr

Subthreshold Stimulation for His Bundle Pacing.

One of the drawbacks of permanent His bundle pacing has been the relatively high pacing thresholds. The present experimental study was proposed to address this issue. In this article the authors present preliminary evidence that His bundle pacing can be achieved with subthreshold stimulation, thereby providing for increased battery life and consequently longer replacement intervals. Possible mechanisms underlying the paradoxical effects of subthreshold stimulation before and after the onset of complete atrioventricular block are also discussed.

Future Developments in His Bundle Pacing.

Because there has been a significant push toward His bundle pacing (HBP), multiple studies and advancements are underway to provide new and improved delivery tools and lead designs, allowing one to apply this technology in daily practice. A better understanding of the pacing configurations and ultimately development of dedicated algorithms will alleviate some of these aforementioned challenges. Ultimately, with such technological advances and mounting clinical evidence, one can surely anticipate HBP to revolutionize the field of cardiac pacing.

[Electrocardiographic aspects of the pathology of the bundle of His].

The ECG appearances of His bundle pathology are very variable and endocavitory recordings are often required to confirm the diagnosis which may be suspected by close analysis of the surface ECG. Truncular AVB is a serious condition as the block is infranodal but with a supraventricular and therefore a narrow QRS complex: it may be median with a double His potential or proximal and distal, which are more difficult to diagnose. Hisian extrasystoles are particularly polymorphic, either overtly, suggesting successively atrial and ventricular extrasystoles, or masked with deceptive pseudo-1st or 2n

His Bundle Pacing for Cardiac Resynchronization.

Cardiac resynchronization therapy with left ventricular coronary sinus or epicardial pacing is an established therapy for patients with systolic heart failure and intraventricular conduction system delay, particularly left bundle branch block. Despite large-scale randomized trials demonstrating the efficacy of this therapy, clinical and echocardiographic nonresponse rates remain as high as up to 40%. His bundle pacing (HBP) has reemerged as a means to provide physiologic resynchronization in patients with bundle branch block by correcting QRS by direct capture of the His-Purkinje system. This
