A patient with congestive heart failure and mild renal impairment developed exfoliative dermatitis following the addition of captopril to her therapy. The skin reaction responded to drug withdrawal and the administration of corticosteroids, though the patient subsequently succumbed to the complications of an incidental myocardial infarction. Recognition and prompt treatment of this potentially fatal dermatological crisis is stressed. The newer generation of angiotensin converting enzyme inhibitors may have a lesser propensity for cutaneous complications.

A 51 Year old man, was taking captopril 200 mg daily' for the management of essential hypertension. Twenty two days after starting captopril, he developed flaccid vescicles and bullae over his scalp, face, trunk and mucous membranes of the mouth. Clinical and histopathological features were consistent with the diagnosis of pemphigus vulgaris' Complete disappearance of the skin lesions within. 15 days of withdrawing the drug and absence of any recurrences in one year follow-up suggest that captopril was the probable offending drug.

The authors report three cases of Captopril induced lichenoid eruption, already unmentionned, as far as they know. This cutaneous adverse reaction seemed to be dose related. Late onset was possible, even after six months of therapy. Violaceous papules or plaques were localized (two cases) or disseminated (one case) on the skin. We report a combined analysis of 235 normotensive IDDM patients with microalbuminuria who participated in two 24-month double-blind, randomised, placebo-controlled trials to assess the effects of captopril 50 mg twice daily on the progression to overt clinical albuminuria. PURPOSE: To report a rare case of lichen planus pemphigoides (LPP) possibly induced by captopril.
CASE SUMMARY: A 74-year-old woman developed pruriginous and bullous lichenoid eruption after starting captopril for hypertension. Histopathological and immunological features were consistent with the diagnosis of LPP that was managed by discontinuing captopril and corticosteroid therapy. With a brief description of the physiological role of the renin-angiotensin-aldosterone system in the regulation of blood pressure the most important clinicopharmacological aspects of the converting enzyme inhibitor, captopril, is outlined. Hemodynamic and humoral effects, pharmacokinetics, indications, contraindications, dosage, side effects, drug interactions and usage in diagnostic procedures of captopril is described. The possibility of captopril therapy in children is also discussed.

Seven women with primary pulmonary hypertension underwent hemodynamic evaluation, at rest and during exercise, before and after the oral administration of captopril. Dose-response curves were generated for the 25-, 50- and 100-mg doses. Captopril significantly reduced systemic blood pressure and systemic vascular resistance; these effects persisted at submaximal levels of exercise. Captopril, an oral active dipeptidylcarboxypeptidase inhibitor with antihypertensive properties, has been reported to have the following cutaneous side effects: macular and papular skin eruptions, urticaria, angioedema, mouth ulcers, pemphigus, and pityriasis rosea-like eruptions. Here, to the best of our knowledge, is the first case in which a pityriasis rosea-like eruption evolved into a lichenoid drug eruption. Also discussed is the remarkable similarity in the side effects of captopril, gold compounds, d-penicillamine, and organic mercurials.

We describe a 19-year-old man who developed muscle weakness due to hypokalemia. His blood pressure on admission was considered to be relatively high, and we performed captopril renography. His baseline renogram was normal, while the results of captopril renography showed delayed patterns similar to those of renovascular hypertension.
