Perioperative Care of Children with Congenital and Acquired Heart Disease Undergoing Non-Cardiac Surgical Procedures.

Children with congenital and acquired heart disease are at elevated risk for perioperative morbidity and mortality. These patients have high rates of coexisting conditions that necessitate noncardiac surgical procedures, some of which may occur outside of centers with specific expertise in pediatric cardiac care. The heterogeneity of anatomic and physiologic states in congenital heart disease presents a challenge to clinicians caring for these patients. This article aims to educate the general pediatric practitioner on the cardiac physiology associated with high-risk categories of congenital a…

Pulmonary Complications After Cardiac Surgical Procedures: A Tertiary Centre Audit of Elective and Urgent Cases.

BACKGROUND: Inadequate time for preoperative optimization can lead to adverse outcomes after urgent cardiac surgical procedures. In this audit, we compared the incidence of postoperative pulmonary complications (PPCs) and other adverse outcomes after elective and urgent cardiac surgical procedures. METHODS: Adult patients who underwent nonemergency open heart surgical procedures were included. PPCs were defined as a composite of atelectasis, pneumonia, acute respiratory distress syndrome, respiratory failure, and pulmonary aspiration. Logistic regression analysis identified factors associated…

Outcomes of Concomitant Cardiac Surgical Procedures Performed During Pediatric Lung Transplantation in the United States.

Data on concomitant cardiac surgery (CCS) performed during pediatric lung transplantation (LTx) is limited. Therefore, we conducted a multi-institutional analysis to identify the incidence and outcomes of CCS in pediatric (< 18 years) LTx recipients by merging data (2004-2023) from the United Network for Organ Sharing (UNOS) and Pediatric Health Information System (PHIS) databases. Of the total of 596 pediatric LTx recipients, 87 (15%) underwent CCS. The majority of these cardiac surgeries were atrial septal defect (ASD) closure (90%) followed by aortic arch/descending aortic repair (3%), atri

Frequency of non-cardiac surgical procedures in neonates undergoing cardiac surgery: potential use of setting family and care team expectations.

INTRODUCTION: Most children with CHD who undergo cardiac surgery in infancy require anaesthesia for additional non-cardiac procedures early in childhood. The purpose of our study was to characterise the use of non-cardiac procedures during the first year of life among neonates requiring cardiac surgery in order to set family and care team expectations. MATERIALS AND METHODS: We conducted a retrospective descriptive study comprising data collection from the medical record since programme inception from September 1, 2018, to November 3, 2025. The records of all patients undergoing neonatal card

Dissociation Between Measures of Macrocirculation and Microcirculation in Patients Undergoing Cardiac Surgical Procedure.

BACKGROUND: In the postoperative period of cardiac surgical procedure, there is an imbalance in the ratio of oxygen supply to oxygen consumption that leads to organic dysfunction and death. There is evidence of microcirculation involvement in cardiac surgical procedure, and a dysregulated inflammatory response similar to sepsis can occur. METHODS: We present a cohort of 280 consecutive adults who were monitored in the postoperative period after cardiac surgical procedure. We performed serial measurements of macrocirculatory indices, indices of global oxygenation, CO2-derived indices, and perf

Cardiac catheterization and interventional procedures as part of staged surgical palliation for hypoplastic left heart syndrome.

BACKGROUND: Cardiac catheterization and interventional procedures are an important part of staged surgical palliation for hypoplastic left heart syndrome (HLHS). METHODS: We reviewed our experience of interventional procedures for HLHS patients treated between 01/1996 and 12/2010. RESULTS: Overall, 222 neonates received a Norwood operation. Of them, 181 underwent 554 catheterizations with 243 interventions. Recoarctation was treated by balloon angioplasty (BA) in 46 patients. The pressure gradient dropped from 29 ± 14 mm Hg to 6 ± 7 mm Hg (P <.001); the diameter of the stenosis increased by

Clinical characteristics and surgical management of vascular complications in patients undergoing cardiac catheterization: interventional versus diagnostic procedures.

The purpose of this report is to define the clinical characteristics and outcome of surgical management of vascular complications after interventional cardiac catheterization and to contrast them to those after diagnostic cardiac catheterization. From October 1985 to December 1989, 101 patients were treated for 106 vascular complications after 1866 interventional and 5046 diagnostic cardiac catheterizations at the University of Michigan Medical Center. Interventional catheterizations resulted in 69 vascular complications in 64 patients (frequency 3.4%). The most common interventions included c

Fontan procedure: imaging of normal post-surgical anatomy and the spectrum of cardiac and extracardiac complications.

Univentricular congenital heart diseases include a range of entities that result in a functionally single ventricular chamber. Although the only curative therapy is cardiac transplantation, there are several palliative surgical techniques that prevent ventricular volume overload, diverting part or all the systemic venous circulation into the pulmonary arteries. The modern Fontan procedure, which consists of anastomosing both the superior (SVC) and inferior vena cava (IVC) to the right pulmonary artery (RPA), is nowadays the last step before transplantation. The importance of imaging in these e

Surgical procedure for targeting arrhythmogenic substrates in the treatment of ventricular tachycardia associated with cardiac tumors.

BACKGROUND: Complete tumor resection is a standard strategy in the surgical treatment of ventricular tachycardia (VT) associated with cardiac tumors. Recently, an intraoperative electroanatomic mapping system (CARTO) has enabled surgeons to target the localized arrhythmogenic substrate for partial resection and/or cryoablation in nonresectable cardiac tumors. OBJECTIVE: The purpose of this study was to evaluate the surgical procedures and late outcomes of the treatment of VT associated with cardiac tumors. METHODS: We examined six patients (age 1-65 years) who had undergone surgical treatmen
