Cardiomyopathies: An Overview.

BACKGROUND: Cardiomyopathies are a heterogeneous group of pathologies characterized by structural and functional alterations of the heart. AIMS: The purpose of this narrative review is to focus on the most important cardiomyopathies and their epidemiology, diagnosis, and management. METHODS: Clinical trials were identified by Pubmed until 30 March 2021. The search keywords were "cardiomyopathies, sudden cardiac arrest, dilated cardiomyopathy (DCM), hypertrophic cardiomyopathy (HCM), restrictive cardiomyopathy, arrhythmogenic cardiomyopathy (ARCV), takotsubo syndrome". RESULTS: Hypertrophic …

Cardiomyopathies of infancy.

Cardiomyopathies in infancy constitute an ill-defined group of conditions. Recently, however, a number of specific disorders of metabolism and mitochondrial abnormalities have been described. Idiopathic or primary cardiomyopathies remain poorly defined. Endocardial fibroelastosis, the most prominent member in this group, probably represents a reaction of the endocardium to various insults. The different types of cardiomyopathy (CM) present a similar clinical picture and require the expertise of a group of specialists for proper diagnosis. Therapy is mostly supportive, as only a few cases of CM…

[Cardiomyopathies in childhood].

Primary cardiomyopathies are heart muscle diseases of unknown cause. In hypertrophic cardiomyopathy (HCM) a hypertrophy of unknown aetiology results either in asymmetric septal hypertrophy (mostly with obstruction) or in concentric hypertrophy of the left ventricle (mostly without obstruction). This condition is rare in childhood, partly inherited and sometimes observed already in newborns. Sudden cardiac death is a main complication in the second decade. In non-obstructive HCM endomyocardial biopsy is the tool to differentiate secondary forms. Dilated cardiomyopathy is recognized by aetiologi…

Cardiomyopathies in Pregnancy.

Cardiomyopathies are uncommon but potentially life-threatening causes of cardiovascular disease in pregnancy. Pregnancy-associated hemodynamic changes may reveal unrecognized cardiomyopathy or exacerbate existing myocardial dysfunction. This review summarizes the diagnosis and management of dilated cardiomyopathy, hypertrophic cardiomyopathy, left ventricular hypertrabeculation/noncompaction cardiomyopathy, Takotsubo cardiomyopathy, and other rare cardiomyopathies during pregnancy. Our discussion will include pregnancy-specific hemodynamic considerations, risk stratification, medical managemen…

Cardiomyopathies and anaesthesia.

Cardiomyopathy is considered as a heart muscle disease of multiple aetiologies, unlike other cardiac diseases related to a definitive pathop … evices or biventricular pacing but may also be diagnosed at anaesthetic pre-assessment. We describe cardiomyopathies such as dilated cardiomyopathy, hypertrophic cardiomyopathy, post-partum cardiomyopathy and Takotsubo cardiomyopathy in brief and their anaesthetic management.

Mechanisms in cardiomyopathies.

The three main clinical types of cardiomyopathy are: hypertrophic cardiomyopathy; dilated cardiomyopathy; and restrictive cardiomyopathy. In each case the basic cellular mechanisms still remain to be defined.

Tailored therapeutics for cardiomyopathies.

The term cardiomyopathy is used to describe a large family of complex heart muscle disorders of diverse aetiology and pathophysiology. For d … st to other areas of medicine, cardiology is characterized by few aetiology-targeted therapies, but cardiomyopathies offer an ideal model for innovation because, in many individuals, the disorder has a monogenic cause, the expression of which is modified by complex genetic mechanisms, comorbidities and lifestyle. Elucidation of the complex cellular and molecular p …

Cardiomyopathies and Adrenal Diseases.

Cardiomyopathies are myocardial disorders in which heart muscle is structurally and/or functionally abnormal. Previously, structural cardiomyocyte disorders due to adrenal diseases, such as hyperaldosteronism, hypercortisolism, and hypercatecholaminism, were misunderstood, and endomyocardial biopsy (EMB) was not performed because was considered dangerous and too invasive. Recent data confirm that, if performed in experienced centers, EMB is a safe technique and gives precious information about physiopathological processes implied in clinical abnormalities in patients with different systemic di…

Proteasome dysfunction in cardiomyopathies.

The ubiquitin-proteasome system (UPS) plays a critical role in removing unwanted intracellular proteins and is involved in protein quality control, signalling and cell death. Because the heart is subject to continuous metabolic and mechanical stress, the proteasome plays a particularly important role in the heart, and proteasome dysfunction has been suggested as a causative factor in cardiac dysfunction. Proteasome impairment has been detected in cardiomyopathies, heart failure, myocardial ischaemia, and hypertrophy. Proteasome inhibition is also sufficient to cause cardiac dysfunction in heal…

Inherited cardiomyopathies--Novel therapies.

Cardiomyopathies arising due to a single gene defect represent various pathways that evoke adverse remodeling and cardiac dysfunction. While the gene therapy approach is slowly evolving and has not yet reached clinical "prime time" and gene correction approaches are applicable at the bench but not at the bedside, major advances are being made with molecular and drug therapies. This review summarizes the contemporary drugs introduced or being tested to help manage these unique disorders bearing a major impact on the quality of life and survival of the affected individuals. The restoration of th…

[Pathogenesis of inflammatory cardiomyopathies].

The development of dilated cardiomyopathy from chronic myocarditis is caused by the persistence of enteroviral infections and chronic myocardial inflammation. New molecularbiological and immunohistochemical techniques improve diagnosis and allow predictions of the development of the disease. This facilitates the decision for a specific immunomodulatory therapy.

Molecular genetics of cardiomyopathies.

Familial hypertrophic cardiomyopathy (FHCM) is a heterogeneous disease with an autosomal dominant Mendelian inheritance and variable penetrance. Several mutations in the beta-myosin heavy chain (beta MHC) gene, the first gene identified for this disease, have been described that co-segregate with the inheritance of the disease. All the mutations in the beta MHC gene encode for the globular head of the myosin protein except for the deletion mutation which encodes for the carboxy-terminus (rod) of the protein. The clinical features associated with some of the mutations in the beta MHC gene have

[Right-side hypertrophic cardiomyopathies].

A complex of clinicoinstrumental methods (two-dimensional and Doppler echocardiography, cardiac catheterization, etc.) was used for the detection of right heart hypertrophic cardiomyopathies (RHHCMP) (52 patients aged 16 to 67). Right heart local apical HCMP (20 patients) was characterized by regional hypertrophy (1.45 +/- 0.027 cm) and hypokinesia of the apical segment of the interventricular septum (IVS). Hypertrophy of the IVS second/third and RV apex, a RV peak-like shape on a ventriculogram were revealed in apical RHHCMP (17 patients). Obstructive asymmetric RHHCMP (3 patients) was manife…
