Cementation and interface analysis of early failure cases after hip-resurfacing arthroplasty.

PURPOSE: The use of inappropriate cementation techniques has been suggested as an adverse factor for the long-term survival of hip-resurfacing arthroplasty. Inadequate initial fixation, thermal osteonecrosis and interface biological reactions are possible causes of failure. We analysed morphological changes associated with the cementation technique in a large collection of retrieved femoral components. METHODS: One hundred and fifty femoral components (mean time to failure of 8.3 months±11.0) obtained at revision surgery were analysed morphometrically and histopathologically. Cement mantle an…

Inefficacy of the cementation of femoral head collapse in glucocorticoid-induced osteonecrosis.

The objective of this paper was to assess the efficacy of cementation of the femoral head in stage III glucocorticoid-induced osteonecrosis. Ten hips (nine patients) were treated by the injection of low-viscosity cement to reduce the collapse. The follow up included clinical and radiological assessments preoperatively and at 3, 6 and 12 months after surgery. The visual analogue scale (VAS) score, the Lequesne index and the Western Ontario and McMaster University Osteoarthritis Index (WOMAC) score did not show any significant improvement. Eight of the ten hips showed a worsening of the collapse…

Giant cell tumor of bone treated by curettage, cementation, and bone grafting.

Giant cell tumor, a relatively rare, occasionally malignant tumor of bone, remains a difficult therapeutic problem for the orthopedic surgeon. Various treatment options, including curettage alone, curettage with bone grafting, radiation, and curettage with acrylic cementation have been utilized and reported. Therapeutic complications such as infection, recurrence, and loss of support of the articular surface have continued to plague the treatment of giant cell tumors. This article reports the results of 10 consecutive cases of giant cell tumor of bone treated by curettage and methyl methacryla…

Pathological predictors for curettage and cementation outcome in proximal tibial giant cell tumors.

Proximal tibia giant cell tumors (GCT) are aggressive with high-recurrence rate, function-affecting and benign neoplasms. Therefore, it is of interest to report the pathological predictors for curettage and cementation outcome in proximal tibial giant cell tumors. Hence, 32 patients treated with curettage, poly-methyl-methacrylate (PMMA) cementation and locking plate fixation between 2018 and 2022 was included in this study. The average age of patients was 28.1 ± 6.9 years and most tumors were Grade 2 (campanacci grade) with 62.5% cortical involvement. The average musculo-skeletal tumor societ…

The Cementation Mechanisms and Mechanical Properties of Different Soil-Rock Mixtures-Slurry Cements.

This investigation focused on the cementation mechanisms and mechanical properties of soil-rock mixtures-slurry cement (SRM-SC) to ensure the safety of tunnels during operation. SRM-SC specimens were prepared with different types of slurry and rock contents based on an actual slurry injection ratio. The macroscopic level analysis involved measuring the specimens' uniaxial compressive strength and shear strength, determining the strength parameters, and analyzing the damage forms. At the microscopic level, the surface morphology and composition of the specimens were examined using scanning elec…

A minimally invasive technique to remove broken cemented stems and its reconstruction with cement-in-cement.

INTRODUCTION:: Historically the removal of fractured stems was associated with complex revision and loss of bone stock. This study describes a minimally invasive procedure for extraction of the distal part of a broken cemented stem that does not compromise the bone stock and results with the cement-in-cement reconstruction technique. MATERIALS AND METHOD:: 7 patients with an average age of 74 years who underwent total hip replacement (THR) revision for fracture of femoral stems were included. The average follow-up was 8.7 years. For the removal of the distal fragment of the broken stem, a set

Enhancing the efficiency and sustainability of soil Bio-Cementation improvement using Persian gum.

Microbial-induced calcite precipitation (MICP) has emerged as a sustainable soil improvement method over the past few decades. Despite its promise, challenges persist in controlling the injection rate of the cementation solution (CS) and mitigating the emission of harmful gases during the bio-cementation process. This study introduces Persian Gum (PG) as an additive to the MICP process to address these issues, enhancing the practical applications of bio-cementation. Direct shear tests were conducted to evaluate the effects of PG on the cementation process and the shear strength characteristics…

Cement-in-cement acetabular revision with a constrained tripolar component.

Dislocation of a total hip replacement (THR) is common following total hip arthroplasty (THA). When nonoperative management fails to maintain reduction, revision surgery is considered. The use of constrained acetabular liners has been extensively described. Complete removal of the old cement mantle during revision THA can be challenging and is associated with significant complications. Cement-in-cement revision is an established technique. However, the available clinical and experimental studies focus on femoral stem revision. The purpose of this study was to present a case of cement-in-cement

Porosity reduction in bone cement at the cement-stem interface.

The fatigue failure of bone cement, leading to loosening of the stem, is likely to be one mode of failure of cemented total hip replacements. There is strong evidence that cracks in the cement are initiated at voids which act as stress risers, particularly at the cement-stem interface. The preferential formation of voids at this site results from shrinkage during polymerisation and the initiation of this process at the warmer cement-bone interface, which causes bone cement to shrink away from the stem. A reversal of the direction of polymerisation would shrink the cement on to the stem and red

Biomechanical effects of cement neck and interspinous process device on locking lumbar interbody cementation.

BACKGROUND: Locking lumbar interbody cementation is a surgical option in patients with osteoporosis and low mobility. It can quickly stabilize the spine construct and prevent cage subsidence. However, establishing a stable bridging neck cement between the vertebrae and disc is a key procedure. METHODS: The validated lumbosacral model analyzed the stress cracking risks for five cement neck diameters under flexion, extension, bending, and twisting. The key indices included disc mobility and neck stress. The biomechanical impact of the interspinous process device was evaluated in high-stress fra…

Cementation of a Metal Dual Mobility Liner in Patients Undergoing Revision Total Hip Arthroplasty.

BACKGROUND: Dual mobility liners for primary and revision total hip arthroplasties can lead to decreased dislocation and revision rates. However, there are a lack of data analyzing their utilization when cementing into well-fixed acetabular components in a variety of scenarios. Therefore, the purpose of this study was to examine cementation of dual mobility liners into well-fixed existing acetabular components from previous hip procedures, into new acetabular components, or directly into the acetabulum without prior implants. We specifically aimed to assess the following: (1) aseptic revision-…

Long Term Results of Liner Polyethylene Cementation Technique in Revision for Peri-acetabular Osteolysis.

Patients with peri-acetabular osteolysis around a well fixed cementless acetabular component may be treated with liner exchange. When the locking mechanism is unreliable or unavailable, cementing the liner into the fixed acetabular component is a feasible option. The purpose of this study was to evaluate the clinical and radiographic long term results of this technique. Forty hip revisions with liner cementation in 37 patients were performed. The minimum follow up was 10 years. Modified Harris Hip Score and recent x rays were reviewed. Four hips were re-revised. Two patients were diagnosed wit…

Dangers with cementation under low-viscosity state: Cement arterio-venogram and bone cement implantation syndrome.

Cement arterio-venogram is a rare event with cement extrusion into femoral nutrient vessels. In literature it is known to be benign with no significant clinical sequelae. It is postulated that it is due to high cement implantation pressure, that results in optimal cement filling quality. All previously reported cases were female patients, and it is thought to be a female only phenomenon due to the relatively narrow femoral canal leading to higher pressures during cementation. In this case series we report 3 cases different to existing literature. All 3 patients showed a cement arterio-venogram…

Rapid fault healing from cementation controls the dynamics of deep slow slip and tremor.

Despite its status as one of the most important discoveries in geophysics, the physical mechanism(s) responsible for slow slip events (SSEs) are not well understood. Here, we synthesize observations of deep SSEs in the Cascadia Subduction Zone and argue that rapid, cohesive fault strengthening may control the dynamics of deep SSEs. Cohesive strength is frequently ignored in constitutive laws used to describe fault rheology in numerical simulations of earthquakes and SSEs alike. To demonstrate its importance, we perform and analyze a suite of petrological experiments that simulate fault healing

Cementation of proximal femoral nails of the very elderly subject in per-trochanteric fractures.

The management of pertrochanteric fractures (PTF) in the very elderly relies on early verticalisation to limit complications of the decubitus and this requires stable osteosynthesis allowing immediate full support without risk of mechanical failure. The aim of the study was to analyse the value of cementing the cervicocephalic blade during osteosynthesis with a proximal femoral nail. A prospective bicentric comparative study was con- ducted. Patients over 90 years of age with PTF were included. Centre A used a PFNA (Proximal Femoral Nail Antirotation) nail without blade cementing and Centre B

Cementation of a polyethylene liner into a metal shell. Factors related to mechanical stability.

BACKGROUND: Various clinical situations may make polyethylene liner exchange desirable in the setting of a well-fixed metal shell. Options have included a simple polyethylene liner exchange or revision of the entire acetabular shell. A recently introduced technique involves cementation of a new liner into a well-fixed metal shell. The purposes of this study were to quantify the mechanical integrity of this method of liner fixation, to evaluate the factors that may influence immediate liner fixation, and to compare this construct with a standard locking mechanism. METHODS: One modular acetabul…

Open reduction and cementation for femoral head fracture secondary to avascular necrosis: preliminary report.

Current treatment for femoral head avascular necrosis has shown good results in early stages of disease, but are not as impressive after progression to collapse. We treated 19 patients (20 hips) with Stage III avascular necrosis (AVN) by open reduction augmented by methylmethacrylate cementation. Follow up ranges from 6 months to 2 years (average = 8.7 months). We followed patient progress with pre- and post-operative Harris Hip Scores, Womac Osteoarthritis Index and a Health Status Questionnaire (SF36). All patients realized immediate post-operative pain relief and improvement in function. Ha…
