Cementoplasty for managing painful bone metastases outside the spine.

OBJECTIVE: To illustrate the effect of treatment with cementoplasty in patients with painful bone metastases in the extraspinal region. METHODS: A retrospective study was conducted to review 51 consecutive patients who underwent cementoplasty under CT or fluoroscopic guidance, a total of 65 lesions involving the ilium, ischium, pubis, acetabulum, humeral, femur and tibia. In 5 patients with a high risk of impending fracture in long bones based on Mirels' scoring system, an innovative technique using a cement-filled catheter was applied. The clinical effects were evaluated using the visual ana…

Iliotibial band friction syndrome after knee cementoplasty: a case report.

Iliotibial band pathologies can result in lateral knee pain. These are commonly seen in runners and cyclists. Lateral knee pain following knee arthroplasty can be due to distal iliotibial band enthesopathy or impingement by the femoral component. Cementoplasty is a common procedure performed during treatment of osseous lesions. We describe a case of ITB friction syndrome due to small focus of cement following cementoplasty for GCT (giant cell tumour).

Assisted techniques for vertebral cementoplasty: why should we do it?

Assisted techniques (AT) for vertebral cementoplasty include multiple mini-invasive percutaneous systems in which vertebral augmentation is obtained through mechanical devices with the aim to reach the best vertebral height restoration. As an evolution of the vertebroplasty, the rationale of the AT-treatment is to combine the analgesic and stability effect of cement injection with the restoration of a physiological height for the collapsed vertebral body. Reduction of the vertebral body kyphotic deformity, considering the target of normal spine biomechanics, could improve all systemic potentia…

[Image-guided bone consolidation in oncology: Cementoplasty and percutaneous screw fixation].

Bone metastases are a common finding in oncology. They often induce pain but also fractures which impair quality of life, especially when involving weight-bearing bones. Percutaneous image-guided consolidation techniques play a major role for the management of bone metastases. Cementoplasty aims to stabilize bone and control pain by injecting acrylic cement into a weakened bone. This minimally invasive technique has proven its efficacy for bones submitted to compression forces: vertebra, acetabular roof, and condyles. However, long bone diaphysis should be treated with caution due to lower res…

Percutaneous cementoplasty for the treatment of extraspinal painful bone lesion, a prospective study.

PURPOSE: The current gold standard treatment of localized painful bone lesion is radiotherapy but this technique has limitations. Our study aims to demonstrate that cementoplasty is an efficient alternative for these palliatives indications when lesions involve extraspinal bones. We prospectively followed 20 patients who received a percutaneous cementoplasty on painful lytic bone lesions between May 2008 and May 2010. MATERIALS: Seventeen patients also had difficulty walking in relation to the pain experienced. The clinical indication for treatment was severe pain (≥4 on the numeric scale) du…

Reinforced cementoplasty for pelvic tumour lesions and pelvic traumatic fractures: preliminary experience.

OBJECTIVES: Pelvic bone pathological lesions and traumatic fractures are a considerable source of pain and disability. In this study, we sought to evaluate the effectiveness of reinforced cementoplasty (RC) in painful and unstable lesions involving the pelvic bone in terms of pain relief and functional recovery. METHODS: All patients with neoplastic lesion or pelvic fracture for whom a pelvic bone RC was carried out between November 2013 and October 2017 were included in our study. All patients who failed the medical management, patients unsuitable for surgery, and patients with unstable oste…

Complex posterolateral approach for C2 cementoplasty: A case report and review of the literature.

This case report describes a new approach to C2 vertebroplasty in a 70-year-old female with a C2 myeloma lesion with potential risk of fracture. Traditional anterolateral and transoral approaches were contraindicated due to a medialized carotid artery. An innovative direct posterolateral approach was used, combining over-the-wire and hydrodissection techniques bypassing the vertebral artery, achieving effective stabilization with no complications.

Percutaneous image-guided double oblique anterior approach to the acetabulum for cementoplasty.

AIM: To report the technique of percutaneous double oblique anterior access to the acetabulum and evaluate its feasibility and safety. MATERIALS AND METHODS: Pelvic computed tomography (CT) examinations of 60 patients (30 men and 30 women; mean age 62.6±13.2 years) were retrieved randomly from the picture archiving and communication system (PACS). A virtual intraosseous trajectory was defined on both sides on the axial CT images with the entry point marked 1 cm above the anterosuperior iliac spine and the endpoint defined just above the level of the ischial spine at the midpoint of the poster

Combined radiofrequency thermal ablation and percutaneous cementoplasty treatment of a pathologic fracture.

Skeletal metastases commonly occur in patients with cancer and treatment is necessary to alleviate pain and prevent complications, such as mobility deficiencies and pathologic fractures. The authors present a patient with a stage IV malignant melanoma and a pathologic fracture of the left tibial plateau treated with radiofrequency heat ablation and percutaneous cementoplasty for defect filling and stabilization. The authors believe that this approach may be an alternative to the standard treatments in palliative-care situations.

Navigation-guided percutaneous pelvic cementoplasty for metastatic bone pain: A case report.

RATIONALE: Percutaneous cementoplasty is a minimally invasive procedure that can provide immediate pain relief and improve range of motion in patients with metastatic bone pain. Conventionally, this procedure is guided by computed tomography (CT). However, to minimize exposure to radiation, we performed percutaneous cementoplasty under the guidance of a navigation system. PATIENT CONCERNS: A 60-year-old man presented with left hip pain for several months due to bone metastasis in the left ilium. DIAGNOSES: The patient was diagnosed with lung cancer and multiple bone metastases including ileu…
