Central nervous system toxicity caused by xenobiotic exposure is a common reason for presentation to the emergency department. Sources of exposure may be medicinal, recreational, environmental, or occupational; the means of exposure may be intentional or unintended. Toxicity may manifest as altered thought content resulting in psychosis or confusion; may affect arousal, resulting in lethargy, stupor, or coma; or may affect both elements of consciousness. Seizures may also occur.

Tuberculosis (TB) has shown a resurgence in nonendemic populations in recent years and accounts for 8 million deaths annually in the world. Central nervous system involvement is one of the most serious forms of this infection, acting as a prominent cause of morbidity and mortality in developing countries. The rising number of cases in developed countries is mostly attributed to factors such as the pandemic of acquired immunodeficiency syndrome and increased migration in a globalized world. Mycobacterium TB is responsible for almost all cases of tubercular infection in the central nervous syste…

Central nervous system tumors are the most frequent malignant tumor in children and the main cause of death in this age group after traffic accidents. The current estimates are that one adult in 2500 is a survivor of a brain tumor that occurred during childhood. These tumors are particularly heterogeneous in terms of histology/biology, treatment, and outcome. They share, however, a high risk of neurological and cognitive morbidity due to the disease itself and the treatment modalities (radiotherapy, surgery, and chemotherapy). Diagnosis is frequently delayed because symptoms are usually nonspe…

Central nervous system (CNS) involvement, one of the most devastating clinical manifestations of tuberculosis (TB) is noted in 5 to 10% of extrapulmonary TB cases, and accounts for approximately 1% of all TB cases. Definitive diagnosis of tuberculous meningitis (TBM) depends upon the detection of the tubercle bacilli in the CSF. Every patient with TBM should preferably be evaluated by imaging with contrast enhanced CT either before or within the first 48 hours of treatment. An extra-neural focus of tuberculosis should be sought clinically and radiologically in all patients with CNS TB as it ma…

Central nervous system (CNS) tuberculosis is frequently encountered in tropical countries. Imaging plays an important role in its recognition and in its differentiation from other similar conditions. Specific magnetic resonance techniques, such as magnetization transfer imaging, proton magnetic resonance spectroscopy, diffusion, and perfusion imaging are useful in its characterization and management. This article reviews the various forms of CNS tuberculosis, including its complications and imaging features.

Central nervous system (CNS) histoplasmosis is one of the important endemic mycoses in America and Africa. Although the incidence of CNS histplasmosis in non-endemic areas is increasing, the diagnosis of isolated CNS histoplasmosis in these areas remains challenging. Since the diagnosis is often missed, the outcome of CNS histoplasmosis is occasionally poor. One of the important complications of this condition is hydrocephalus, which may be identified before meningitis is diagnosed. Antigen or serological tests are useful for the diagnosis of CNS histoplasmosis; however, it should be noted tha…

Fetal central nervous system abnormalities encompass a variety of lesions. Most of the anomalies are seen in association with an increased incidence of fetal aneuploidy and an increase in other anomalies both within and outside of the central nervous system. These associated findings and the underlying lesion all have a role in the eventual prognosis for the ongoing pregnancy. The antenatal diagnosis of a central nervous system abnormality should prompt a careful search for other defects, and consideration should be given for genetic evaluation and testing.

On the basis of a literature review the author discusses the aetiology, pathogenesis, diagnosis, clinical course and treatment of central nervous system tuberculosis. Attention is called to the necessity of further improvement of diagnostic methods for making possible early diagnosis and modern treatment of this disease.

Primary central nervous system lymphoma (PCNSL) is a diffuse large B cell lymphoma in which the brain, spinal cord, leptomeninges and/or eyes are exclusive sites of disease. Pathophysiology is incompletely understood, although a central role seems to comprise immunoglobulins binding to self-proteins expressed in the central nervous system (CNS) and alterations of genes involved in B cell receptor, Toll-like receptor and NF-κB signalling. Other factors such as T cells, macrophages or microglia, endothelial cells, chemokines, and interleukins, probably also have important roles. Clinical present…

Sarcoidosis with lesions in the central nervous system is a heterogeneous condition with diverse clinical presentations and various degrees of neurological disability. Although extremely rare among sarcoidosis cases, it is of relevance to routine clinical practice because it may be a differential diagnosis in brain tumors, meningitis, and myelopathy of unknown origin. Encephalic and myelopathic lesions of sarcoidosis are often resistant or only partially responsive to immunotherapies, including high-dose corticosteroids and immunosuppressants. This may lead to residual neurological disability …

In this review, we discuss primary central nervous system lymphomas: a rare type of tumours confined to the central nervous system. The disease is associated with a poor prognosis, which, however, generally has seen steady improvement over the last four decades, particularly in the younger population. Modern surgical techniques are reserved for diagnosis and has no place in the treatment, which mainly relies on high-dose polychemotherapy treatment regimes with methotrexate as the backbone.

BACKGROUND: Primary angiosarcoma of the brain is an extremely rare malignant tumor that that arises from vascular endothelial cells of the brain or meninges. The presentation, characteristics, and prognosis of this disease are not well understood. Here we report such a case.
CASE DESCRIPTION: A 68-year-old Chinese man presented with confusion and progressive impairment of right limb movement. Magnetic resonance imaging (MRI) revealed a hemorrhagic lesion in the left frontal lobe. Exploratory surgery revealed a hematoma-like lesion and extensive superficial hemosiderin deposition. The postoper

Opportunistic infections of the central nervous system are classically associated with immunosuppression arising from infection with human immunodeficiency virus and with various hematologic malignancies. However, over the past few years, they are increasingly associated with transplantation and various immunosuppressive treatments used to treat autoimmune diseases. They cause significant morbidity and mortality and remain a diagnostic challenge due to the absence of typical signs and symptoms of infection and mimicry by various noninfectious causes. The pathogens associated with these infecti…

PURPOSE OF REVIEW: Primary central nervous system (CNS) lymphoma is a rare and aggressive extranodal non-Hodgkin lymphoma confined to the brain, eyes, spinal cord, or leptomeninges without systemic involvement. This article provides an overview of the clinical features, diagnosis, and management of primary CNS lymphoma in patients who are immunocompetent, focusing on recent advances in treatment.
RECENT FINDINGS: Primary CNS lymphoma is sensitive to radiation therapy; however, whole-brain radiation therapy inadequately controls the disease when used alone and causes delayed neurotoxicity with…
