The cerebrospinal fluid (CSF) fills the brain ventricles and the subarachnoid space surrounding the brain and spinal cord. The fluid compartment of the brain ventricles communicates with the interstitial fluid of the brain across the ependyma. In comparison to blood, the CSF contains very little protein to buffer acid-base challenges. Nevertheless, the CSF responds efficiently to changes in systemic pH by mechanisms that are dependent on the CO2/HCO3- buffer system. This is evident from early studies showing that the CSF secretion is sensitive to inhibitors of acid/base transporters and carbon…

Abnormalities of cerebrospinal fluid (CSF) pressure are relatively common and may lead to a variety of symptoms, with headache usually being the most prominent one. The clinical presentation of alterations in CSF pressure may vary significantly and show a striking similitude to several primary headache syndromes. While an increase in CSF pressure may be of primary or secondary origin, a pathologic decrease of CSF pressure is usually the result of a meningeal rupture with a resulting leakage of CSF. The pathophysiologic mechanisms of idiopathic intracranial hypertension (IIH) remain largely unk…

Cerebrospinal fluid leakage into the orbit (CSF orbitorrhea) or through the orbit to the exterior (CSF oculorrhea) occurs when there is a communication between orbit and subarachnoid space. It has rarely been described. We report a case of CSF oculorrhea following surgery for recurrent sphenoid wing meningioma. A 67-year-old patient who underwent craniotomy for a recurrent sphenoid wing meningioma complained of tearing from the ipsilateral eye. Fluid collection was observed in the right periorbital area contiguous with craniotomy wound. With wide retraction of eyelids, a 1-cm longitudinal full…

Abdominal cerebrospinal fluid pseudocyst in an infrequent complication of ventriculoperitoneal (VP) shunts. We reviewed ten patients with abdominal pseudocyst. There were five girls and five boys, aged between 4 months and 14 years. The number of shunt procedures prior to the presentation varied between one and five. Only one patient had had a previous shunt infection. No patients had undergone prior abdominal surgery other than VP shunting. The time from the last shunting procedure to the development of abdominal pseudocyst ranged from 3 weeks to 5 years. Presenting symptoms and signs were ma…

Because of its potentially serious sequelae, cerebrospinal fluid (CSF) leakage following surgery for lesions of the cranial base is given immediate attention by neurosurgeons. Despite a multitude of approaches used to prevent its occurrence, CSF leakage complicates up to 30% of difficult skull-base tumor operations. The authors describe the cases of 11 patients who developed a syndrome, not previously described in the literature, termed "pseudo-CSF rhinorrhea." This syndrome occurs after surgery of the cranial base, usually involving dissection or removal of the petrous or cavernous carotid ar…

A case of spontaneous cerebrospinal fluid otorrhea due to a congenital tegmen tympani defect is reported. The etiology and pathogenesis of cerebrospinal fluid otorrhea and of the spontaneous congenital form in particular is discussed, together with the clinical picture and the methods of diagnosis. Reviewing the literature, a middle ear exploration is considered to be the procedure of choice to ascertain the presence and determine the exact locus of the cerebrospinal fluid leak.

Spontaneous cerebrospinal fluid (CSF) otorrhea implies the observation of CSF draining from the ear or nose without any previous history of traumatic or infectious etiology. The term does not exclude embryologic or developmental abnormalities. The condition may be life-threatening. Thus far it has been reported about twenty times. Because it is rare, misdiagnosis or failure to make a timely early diagnosis is common and proper therapy may be delayed. The purpose of this article is to review pertinent literature on this subject and to present two additional case reports. Emphasis will be placed…

OBJECTIVE: Cerebrospinal fluid (CSF) hypovolemia is often difficult to diagnose because reduced CSF pressure and radiographic findings are not consistently observed. To address this limitation, we developed the CSF refill test, a novel diagnostic procedure based on temporary CSF replenishment and evaluation of symptom improvement. This study reports preliminary and prospective results of this method.
METHODS: We analyzed 27 patients with suspected CSF hypovolemia. Seventeen cases (2012-2019) were retrospectively assessed during procedures such as computed tomography myelography and the overfl…

Cerebrospinal fluid-venous fistulas (CSFVFs) were first described in 2014 and have since become an increasingly diagnosed cause of spontaneous intracranial hypotension due to increased clinical recognition and advancements in diagnostic modalities. In this review, the authors discuss CSFVF epidemiology, the variety of clinical presentations, the authors' preferred diagnostic approach, recent advancements in diagnostic methods, treatment options, current challenges, and directions of future research.

: Cerebrospinal fluid (CSF) viral escape is defined by detectable HIV-RNA in CSF despite undetectable or lower-than-CSF level in plasma of patients receiving combination antiretroviral therapy (cART). This condition may occasionally be associated with neurological problems, consisting of new and progressive cognitive decline and/or focal symptoms and signs, defining the 'symptomatic CSF escape'. Brain MRI usually shows diffuse white matter hyperintensities that recall the presentation of HIV encephalopathy in the precART era. However, patients develop symptomatic CSF escape with relatively hig…

BACKGROUND: Intracranial meningiomas are common and comprise 20% of all primary brain tumors. Meningiomas infrequently metastasize; however, to the authors' knowledge there are limited data regarding the spread of disease through cerebrospinal fluid (CSF).
METHODS: Eight of 200 consecutive patients (4%) with meningiomas manifested CSF dissemination. CSF cytology was positive in all patients, and neuroradiographic studies were consistent with CSF dissemination in eight patients. The patients (6 women and 2 men) ranged in age from 24-87 years (mean age, 52 years). All patients had undergone pri…

Cerebrospinal fluid (CSF) shunt infection is one of the most frequent and disabling complications. We reviewed the records of 306 patients who underwent CSF shunt surgery from 1983 through 1992. Six hundred and twelve procedures were performed in these 306 patients. Infection occurred following 46 of the procedures for an infection rate of 7.5% per procedure. The 46 infections involved 39 patients. There were 8 recurrent infections. The infection rate per child was 12.7%. Staphylococcal species were isolated in 50% of all infections. Patients younger than 1 year old and children with multiple …

Unconcentrated cerebrospinal fluid (CSF) was mixed in a constant ratio with carrier ampholytes and spacer amino acids and analysed by isotachophoresis. Examination of the cerebrospinal fluids from 113 patients showed characteristic patterns for normal, acute viral and acute bacterial meningitis. CSF from cases of subacute sclerosing panencephalitis (SSPE) and multiple sclerosis (MS) differed from these and from each other in the gamma-globulin region. This technique provides useful information on the integrity of the blood-brain barrier and is able to determine whether immunoglobulins present …

A case of spontaneous cerebrospinal fluid otorrhea with recurrent meningitis is reported. The route of cerebrospinal fluid leak was through the internal auditory meatus. The cerebrospinal fluid otorrhea was stopped by packing the meatus with muscle and Gelfoam.
