Two weeks after rafting on a river in Thailand a Dutch 54-year-old male experienced chills and high fever. While rafting he had wounded his hand. Because of the history (water contact, the wound, high fever with chills), of the findings at examination (fever, conjunctivitis) and of the laboratory findings (leukocytosis, albuminuria, disturbance of liver enzymes), a clinical diagnosis of 'leptospirosis' was made. Cannabis is the most widely consumed illicit drug in the western world. Cannabis and tobacco smoke contain a similar mix of irritant and toxic chemicals. Therefore, there are reasons to suspect, that cannabis and tobacco have similar side effects. However, the pulmonary effects of smoking cannabis have not been extensively researched and the few findings are contradictory. Among the reasons for this uncertainty are its illegal status and the common practice of combining cannabis with tobacco. Separating the effects of the two substances is therefore difficult.

Q fever usually presents with high fever, headache and an atypical pneumonia. A case report of a 47-year-old patient with an atypical course of a coxiella infection is described. The dominant clinical symptoms were a liver and bone marrow involvement, whereas pulmonary manifestations were absent. Ictal piloerection is a rare autonomic seizure manifestation that can obscure localization and mimic non-epileptic conditions. We report a 21-year-old man with drug-resistant focal epilepsy whose seizures consisted of isolated bilateral piloerection and chills followed by coughing. INTRODUCTION: Chills are a complication of patients undergoing hemodialysis. The rate of infection among hemodialysis patients presenting with chills is not well established, and empirical broad-spectrum antibiotics are usually the rule.
