Hemorrhagic cholecystitis.

Hemorrhagic cholecystitis is a rare cause of abdominal pain that can present in the setting of trauma, malignancy, and bleeding diathesis, such as renal failure, cirrhosis, and anticoagulation. Its symptoms are easily confused with acute calculous cholecystitis and might include hemobilia or hematemesis as blood drains from the gallbladder into the gastrointestinal tract. Imaging of hemorrhagic cholecystitis can be misleading unless the possibility of this diagnosis is considered. In this report, we present 2 cases of hemorrhagic cholecystitis along with relevant imaging and a review of the li…

Emphysematous cholecystitis.

Emphysematous cholecystitis is an uncommon variant of acute cholecystitis, and a communication between biliary ducts and gastrointestinal tract should be evaluated. Making the diagnosis often is straightforward on plain abdominal radiography. Prompt diagnosis of emphysematous cholecystitis is critical, and the standard treatment is emergent cholecystectomy. In severely ill patients, percutaneous cholecystostomy with broad-spectrum antibiotics may be an alternative choice for treatment.

Campylobacter cholecystitis.

There are 13 cases of campylobacter cholecystitis reported so far in the medical literature. Among them, only 4 patients had diarrhea. We report another case of acalculous cholecystitis in a setting of campylobacter enteritis. The case report is followed by a literature review regarding this rare condition.

Emphysematous cholecystitis.

Emphysematous cholecystitis is a relatively rare variant of acute cholecystitis with infection by gas-producing organisms. Diagnosis involves the demonstration of gas within the lumen or wall of the gallbladder by ultrasound or CT scan. In contrast to acute cholecystitis, emphysematous cholecystitis occurs more commonly in elderly and diabetic patients, and is frequently associated with perforation and death. We report here a case of a 75-year old man who developed emphysematous cholecystitis.

Xanthogranulomatous cholecystitis.

Twelve cases of xanthogranulomatous cholecystitis are reported. The clinical presentation was similar to chronic cholecystitis. All patients had associated gallstones. The diagnosis was achieved at histopathological examination of the resected gall bladders and none had any focus of malignancy. Cholecystectomy was curative.

Actinomycotic cholecystitis.

Actinomycotic cholecystitis is rare. We report a 65-year-old woman who was admitted with repeated attacks of pain in the abdomen and vomiting. Clinical and laboratory findings suggested the diagnosis of cholecystitis with cholelithiasis. At cholecystectomy the gall bladder was inflamed with a small perforation, and contained many calculi. Gram's staining of the gall bladder wall revealed Gram-positive actinomycotic colonies.

Xanthogranulomatous cholecystitis.

Xanthogranulomatous cholecystitis (XC) is a rare benign disease often confused with carcinoma gall bladder (CaGB). We retrospectively reviewed the clinical, radiological and operative features of 81 patients with XC alone (73) or XC with associated CaGB (8). Patients with XC alone had no gender preponderance, had a mean age 48 years and mean duration of symptoms of 34 months. Most XC patients (75%) had clinical features of chronic cholecystitis. On the other hand, patients with both XC and CaGB had a female preponderance, a mean age of 60 years and mean duration of symptoms of 6 months. In thi…

Acute cholecystitis.

Two thousand and twenty-one patients treated surgically for acute cholecystitis over a 42 year period with a mortality of 3.4 per cent are reported. This experience reveals that acute cholecystitis among patients 65 years of age and older is a serious condition and is associated with an appreciable operative mortality. However, nonsurgical management results in an even greater mortality. An aggressive surgical approach, cholecystectomy without prolonged delay after the demonstration of calculous disease of the biliary tract, is urged. In the future, this should diminish the proportion of patie…
