Ulnar collateral ligament (UCL) injuries continue to be a major source of morbidity in baseball players. The throwing motion creates nearly supraphysiological levels of valgus stress on the medial elbow, placing these athletes at high risk of UCL injury. The incidence of injury continues to rise at an alarming rate, especially among adolescent baseball pitchers. Certain risk factors for UCL injury have been identified, including pitch velocity, fewer days between outings, and overall workload. Treatment of UCL injuries depends on the type of tear. Low- to medium-grade partial UCL tears (i.e.,

Suture augmentation is frequently being used for medial ulnar collateral ligament (UCL) repair and many surgeons now augment UCL reconstructions with a high strength suture. This article and video describe the technique of suture augmented UCL reconstruction. The technique uses an autograft from palmaris or hamstring that is supported by a suture across the reconstruction. The graft is further modified using multiple points of suture augmentation to strengthen the reconstruction including a 1.6 mm FiberTak at the humerus to bolster the proximal aspect of the reconstruction. The graft is also i

Medial ulnar collateral ligament injury is common in the elbow following repetitive use in overhead athletes and acute trauma such as a fall on to an outstretched hand. This leads to valgus elbow instability confirmed with provocative testing. In patients with functional limitation and radiographic evidence of a midsubstance or diffuse ulnar collateral ligament tear, reconstruction with tendon graft remains the best option. The docking technique, which consists of hubbing the graft in a humeral socket and securing it over a bone bridge, has become the cornerstone of medial ulnar collateral lig

BACKGROUND: Ulnar collateral ligament (UCL) reconstruction is an established surgical technique to restore UCL deficiency, especially in the overhead throwing athlete. Over the past decade, the number of patients requiring UCL reconstruction has increased significantly, particularly in the adolescent patient population. Return-to-play rates after UCL reconstruction reported in the literature have ranged from 33% to 92%, and a recent systematic review noted a return-to-play rate of 89.40% in all high school athletes.
PURPOSE: To evaluate the outcomes, particularly return-to-play rates and subj

Background: Thumb ulnar collateral ligament (UCL) injuries are common, but the kinematics of these injuries have not been comprehensively described, especially regarding kinematic changes with progressive UCL injury. Methods: Eleven cadaveric thumbs underwent kinematic testing under 4 conditions: intact, partial tear (50%) of the proper UCL, full tear of the proper UCL, and complete tear of both the proper and accessory UCL. Kinematic testing parameters included varus/valgus stress, pronation/supination, and volar/dorsal translation at -10 degree, 0 degree, 15 degree, and 30 degree of metacarp

BACKGROUND: The ulnar collateral ligament (UCL) complex of the elbow plays a primary role in valgus and posteromedial stability of the elbow. The anterior oblique ligament (AOL) of the UCL is believed to provide the majority of resistance to external forces on the medial elbow. The transverse ligament (TL) of the UCL is generally thought to have minimal contribution to the elbow's overall stability. However, recent studies have suggested a more significant role for the TL. The primary aim of this study was to identify the TL's contribution to the stability of the elbow joint in determining the

BACKGROUND: The rates of surgeries for ulnar collateral ligament (UCL) injuries continue to rise for baseball pitchers. The physical size of pitchers has also increased, bringing into question whether today's larger pitchers have proportionally bigger and stronger UCLs able to withstand greater elbow varus torque. Furthermore, controversy exists in biomechanics literature regarding whether kinetics during pitching should be reported as torque (in N·m) or normalized torque (scaled by body weight and height).

Since Dr. Frank Jobe performed the initial surgery on Tommy John in 1974, the ulnar collateral ligament (UCL) reconstruction (UCLR), colloquially "Tommy John Surgery," described in 1986 has evolved as the gold standard treatment for UCL tears. The crux of technique modifications involve flexor pronator mass (FPM) management, ulnar nerve transposition (UNT), graft selection, or graft-fixation options. Jobe used a figure-of-8 graft fixation through the cubital tunnel, necessitating FPM elevation and UNT. Although 68% of patients returned to play (RTP), 25% of patients experienced ulnar neuritis,

BACKGROUND: Treatment of ruptured ulnar collateral thumb metacarpophalangeal (MCP) joint ligaments (UCL) necessitate a profound anatomic knowledge for optimal surgical repair in order to preserve range of motion and ensure postoperative joint stability. Therefore, knowledge of the angle between the UCL and the longitudinal axis of the first metacarpal bone could be useful.
METHODS: In this study 46 ulnar collateral thumb MCP joint ligaments in 15 male and 15 female embalmed anatomic specimens were dissected and the angles between the longitudinal axis of the first metacarpal bone and the prop

Injuries to the thumb ulnar collateral ligament (UCL) are common. Failure to address the ensuing laxity of the metacarpophalangeal joint can lead to compromised grip and pinch, pain, and ultimately osteoarthritis. Instability to valgus stress with the lack of a firm end point is a strong indicator of complete rupture of the UCL. Nonoperative treatment is reserved for incomplete ruptures of the thumb UCL. Operative intervention is typically performed for complete ruptures. Repair of acute ruptures and reconstruction for chronic injuries yield excellent results. Complications are rare and most p

The results after operations for rupture of the ulnar collateral ligament of the metacarpophalangeal joint of the thumb are reported to be good. Few authors describe the measurements used to assess the postoperative function of the thumb. We review the results of 15 patients, who underwent surgery between 1982 and 1987. We found that the number of patients satisfied with the operation was consistent with that reported in the literature. On the other hand, we found poor agreement between the subjective complaints and the objective findings. We therefore propose and describe a scoring system to

BACKGROUND: There has been a renewed interest in the pathomechanics of elbow dislocation, with recent literature having suggested that the medial ulnar collateral ligament is more often disrupted in dislocations than the lateral ligamentous complex. The purpose of this serial sectioning study was to determine the influence of the posterior bundle of the medial ulnar collateral ligament (pMUCL) as a stabilizer against elbow dislocation.
METHODS: An elbow dislocation was simulated in 5 cadaveric elbows by mechanically applying an external rotation moment and valgus force. Medial ulnohumeral joi

BACKGROUND: Several previous studies have described reconstructive methods for the treatment of an injury to the ulnar collateral ligament of the thumb. However, there are few biomechanical studies to date to analyze the strength of the surgical reconstruction.
PURPOSE: To evaluate 2 reconstruction techniques with use of a cadaveric model: (1) reconstruction with the use of a free tendon graft placed in a figure-of-8 fashion through drill holes in the metacarpal and proximal phalanx of the thumb, and (2) reconstruction with the use of the Bio-Tenodesis Screw System.

This study examines the treatment of 22 thumbs with symptomatic laxity of the ulnar collateral ligament of the metacarpophalangeal joint by ligament reconstruction 1 week to 10 years after injury. The existing ligament remnants were repaired in 13 thumbs and were re-attached to bone, using a bone anchor, in seven cases. Two ligaments were reconstructed using the extensor pollicis brevis tendon. Patients were reviewed at a mean of 16 months. Functional stability was regained in all patients, but two thumbs had ongoing pain in the metacarpophalangeal joint. All but one patient returned to their

BACKGROUND: While ulnar collateral ligament reconstruction (UCLR) of the elbow is an increasingly commonly performed procedure with excellent results reported in the published literature, less attention has been paid to specifically on the characterization of postoperative ulnar nerve complications, and it is unclear what operative strategies may influence the likelihood of these complications.
PURPOSE: The purpose of this study is to examine the prevalence and type of ulnar nerve complications after UCLR of the elbow based on the entirety of previously published outcomes in the English liter

Ulnar nerve (UN) injuries are a common complaint amongst overhead athletes. The UN is strained during periods of extreme valgus stress at the elbow, especially in the late-cocking and early acceleration phases of throwing. Although early ulnar collateral ligament (UCL) reconstruction techniques frequently included routine submuscular UN transposition, this is becoming less common with more modern techniques. We review the recent literature on the sites of UN compression, techniques to evaluate the UN nerve, and treatment of UN pathology in the overhead athlete. We also discuss our preferred te

Posterolateral rotatory instability is the most common form of elbow instability, secondary to valgus, supination, and axial loading in elbow extension. The lateral ulnar collateral ligament (LUCL) is the primary restraint to posterolateral rotatory instability and multiple techniques of repair and reconstruction of the LUCL have been reported in the literature with variable outcomes. The aim of this technical note is to describe a minimally-invasive technique using a tendon graft to perform a reconstruction of the LUCL using knotless suture anchors in an onlay fashion, without violating both
