War crimes, sexual assault and medical confidentiality in Israel.

Hamas militants have raped and mutilated the bodies of dozens of men and women in Israel during their attack and captivity in Gaza. The exact extent of these atrocities, however, is unknown. For reasons of this sort and others, prosecuting sexual abusers during armed conflicts is notoriously difficult. In an attempt to make a legal case against Hamas militants, the Israeli authorities have recently ordered civilian hospitals to breach medical confidentiality and report unidentified data of patients who have suffered bodily mutilation and sexual abuse during the Hamas attack or during captivity…

Data security and patient confidentiality: the manager's role.

The maintenance of patient confidentiality is of utmost importance in the doctor patient relationship. With the advent of networks such as the National Health Service Wide Area Network in the UK, the potential to transmit identifiable clinical data will become greater. Links between general practitioners (GPs) and hospitals will allow the rapid transmission of data which if intercepted could be potentially embarrassing to the patient concerned. In 1994 the British Medical Association launched a draft bill on privacy and confidentiality and in association with this bill it is pushing for encryp…

Confidential enquiries into hypoxic ischaemic encephalopathy.

Hypoxic ischaemic encephalopathy (HIE) may be regarded as a near miss marker for perinatal death resulting from intrapartum hypoxia. Considering the serious long-term consequences of HIE and issues of blame and liability for clinicians, regional or national audit of HIE might best be done using confidential enquiries. These are conducted by independent multidisciplinary panels, and should identify weaknesses in delivery of health care. A confidential enquiry into HIE may determine intrapartum factors that could have caused the poor outcome. It should also consider the role of associated precon

Confidential enquiry into deaths due to prematurity.

The aim of this study was to audit the management of neonatal respiratory distress syndrome (RDS) in a geographically defined population using a retrospective peer review of case notes. The subjects were 49 infants of 24-36 wk gestation with a birthweight >499 g, and dying as a consequence of prematurity at <1 y of age in Wales during 1996. Forty-four infants (90%) were delivered in a unit with staff experienced in the management of preterm birth. Of the 30 infants <30 weeks' gestation, 29 (97%) received neonatal intensive care on a (sub)regional unit. Predelivery corticosteroids were indicate

[A study of the relationship between community networking and confidentiality].

Legal and ethical conflicts arise in building a community network for mental health workers with confidentiality. Another ethical problem is encountered by involving mental health volunteers who are not obliged to be confidential. The authors made a questionnaire from the following six points of view: 1) the current attitudes of mental health professionals and workers toward confidentiality, 2) the degree of disclosure of individual information, 3) the relation between collaboration with family and confidentiality, 4) the range of extended confidence, 5) the extent of sharing individual inform…

Confidential enquiry of stillbirths in current obstetric practice.

OBJECTIVE: Confidential panel enquiry into sub-optimal factors relating to stillbirths. METHOD: All 121 stillbirths in KK Women's and Children's Hospital in the years 1995 and 1996 were studied. Three assessors reviewed the case records of each death, and panel consensus was reached regarding sub-optimal antenatal care and factors leading to stillbirths. RESULT: The incidence rate of stillbirth was 4.04 per 1000 deliveries. A total of 76 cases (62.8%) were found to have grade II and III sub-optimal factors in their management. Patients themselves were involved in the sub-optimal management o

Navigating Consent and Confidentiality in Adolescent Substance Use Treatment.

Consent and confidentiality are central to adolescent substance use care, functioning not only as legal and ethical frameworks but as early preventive interventions during a critical developmental window. This urgency is highlighted by rising rates of substance use disorders and overdose deaths driven by illicit fentanyl and polysubstance exposure. Expanded harm reduction efforts have intensified tensions between adolescent autonomy, parental involvement, legal requirements, and the clinician's duty to protect safety. This article provides a clinically grounded framework for navigating consent…

Informed consent and confidentiality in community-level tuberculosis interventions.

Community-level tuberculosis interventions have been at the heart of worldwide initiatives on early detection, treatment adherence and transmission control. As program increasingly move beyond clinical settings into households, workplaces, and informal settlements they are faced with complex ethical challenges around issues of informed consent and confidentiality. This paper examines these challenges and raises issues about how they affect community participation, trust and long term programme effectiveness. Drawing on conceptual analysis and country experiences from India and South Africa, th…
