Interim versus standard methadone treatment: a benefit-cost analysis.

A benefit-cost analysis was conducted as part of a clinical trial in which newly-admitted methadone patients were randomly assigned to interim methadone (IM; methadone without counseling) for the first 4 months of 12 months of methadone treatment or 12 months of methadone with one of two counseling conditions. Health, residential drug treatment, criminal justice costs, and income data in 2010 dollars were obtained at treatment entry, and 4- and 12-month follow-up from 200 participants and program costs were obtained. The net benefits of treatment were greater for the IM condition but controlli

Influenza vaccination among healthy employees: a cost-benefit analysis.

The cost of influenza vaccination and influenza infections was evaluated in a controlled study among healthy municipal homemakers. Acute respiratory infections were followed clinically and with laboratory samples for 8 months. Full follow-up was achieved in 351 persons in the intervention group, of whom 47% obtained vaccination, and 492 controls. Influenza infection was confirmed in 10 employees (8 of these in the control group) and other viral infections in 6 employees (5 of them controls). All infections occurred in non-vaccinated persons. The relative risk of infection in the control group

Cost-benefit analysis of screening for toxoplasmosis during pregnancy.

Congenital toxoplasmosis is a risk for fetus both in 'low' and 'high risk' areas. A cost-benefit analysis based on data from a Finnish prospective study (20.3% seropositivity of pregnant mothers and incidence of 2.4/1,000 seronegative pregnancies) and on Finnish cost data was performed to compare the no-screening and screening alternatives for primary toxoplasma infections during pregnancy. A maternal-feto transmission risk of 40%, effectiveness of treatment of 50%, and discount rate of 4% were used as other baseline probabilities. The calculations were carried out by decision analysis combine…

Maternal serum alpha-fetoprotein screening: a cost-benefit analysis.

The prenatal detection of fetal neural tube defects (NTD) is now possible by means of a multi-tiered program utilizing maternal serum alpha-fetoprotein (MSAFP) screening coupled with ultrasonography and amniocentesis where indicated. This paper considers the economic consequences of screening for fetal NTD a theoretical cohort of 100,000 pregnant women at risk of having an affected fetus who would elect to terminate their pregnancies if an affected fetus were found. The cost of the program was balanced against the benefits of the program, such as the averted costs of institutionalization, medi

A Cost-Benefit Analysis Framework for Urban Methane Monitoring.

Despite recent advances in measurement and monitoring, methane (CH4) emissions remain challenging to resolve at subcity jurisdictions as needed to inform mitigation progress. Here we develop an Observation System Simulation Experiment (OSSE) for New York City (NYC) to test whether a small number of strategically located, fixed in situ CH4 measurement sites, together with existing sites, can effectively and accurately monitor NYC borough-level emissions. We identify candidate sites from tall, city-owned buildings screened for rooftop suitability and rank >300 network configurations by their abi

Cost-Benefit Analysis of Intraoperative Autofluorescence for Parathyroid Identification.

IMPORTANCE: Hypoparathyroidism (hypoPT) is the most common complication post-total thyroidectomy and is associated with increased morbidity … ich this technology would become economically feasible. DESIGN, SETTING, AND PARTICIPANTS: In this cost-benefit analysis, a decision tree economic model was developed to compare thyroidectomies performed with autofluorescence to visual inspection only. All costs associated with conventional surgery, autofluorescence, and the costs of managing hypoPT were captured. Mor …

Ultrasound screening of the neonatal hip: cost-benefit analysis.

AIM: To explore the economic justification for introducing ultrasound screening for developmental dysplasia of the hip in Croatia. METHODS: The analysis was based on the two formulas: that cost-benefit equals benefit/cost, and that net benefit equals benefit minus cost. Screening costs were expressed as a sum of training costs and fee for ultrasound screening of neonates. The neonatologists' working hours and utilization of ultrasound instruments were expressed by multiplying the number of infants born per year in Croatia (N = 47,792) with the standard time needed for one examination and then

A cost-benefit analysis of prenatal screening for toxoplasmosis.

The objective of this study was to examine critically the validity of a toxoplasma prenatal screening program, in the context of a cost-benefit analysis, as it relates to the Canadian experience. Recently, studies have suggested that early treatment of infected infants with a combination of pyrimethamine and sulfadiazine is effective in reducing the sequelae of toxoplasmosis. It was concluded that a carefully planned screening program for detecting and treating infants infected with Toxoplasma gondii during pregnancy is cost beneficial. The cost of delivering a screening and treatment program …

Cost-benefit analysis of autologous blood donation in obstetrics.

OBJECTIVE: To determine whether there are predictors of peripartum transfusion, other than placenta previa, that identify a population of pregnant women whose risk of transfusion is high enough to justify antepartum autologous blood donation. METHODS: Using an established perinatal data base, we studied 14,267 consecutive term deliveries without placenta previa. Univariate and multivariate analyses were performed to assess ten predictors of peripartum transfusion that might reasonably be detected in the antepartum period. Costs were calculated for a hypothetical autologous blood donation prog

Modified acid reflux test. A benefit and cost analysis.

Although the standard acid reflux test is often used to diagnose gastroesophageal reflux (GER), the cost and benefit of this diagnostic test has never been evaluated. In this study, 184 consecutive referrals with esophageal symptoms were interviewed and had an esophagram, an esophageal manometry, and a modified acid reflux test (MART). The results were analyzed to determine how frequently MART altered the clinical diagnosis and to assess the cost of the new information. Patients with typical symptoms of GER (heartburn or regurgitation) were compared to those with atypical presentation (chest p

Screening for spina bifida cystica. A cost-benefit analysis.

The costs and economic benefits are examined of introducing a programme for the mass-screening of pregnancies for the detection and abortion of fetuses with spina bifida cystica. A benefit-cost index is derived, and the possible effects on it of making different input assumptions are discussed. It is considered that, on economic grounds, screening may be worthwhile only in populations in which the incidence of spina bifida is high.

Cost-effectiveness and cost-benefit analysis of oliceridine in the treatment of acute pain.

Aim: Oliceridine, a new class of μ-opioid receptor agonist, is selective for G-protein signaling (analgesia) with limited recruitment of β-arrestin (associated with adverse outcomes) and may provide a cost-effective alternative versus conventional opioid morphine for postoperative pain. Patients & methods: Using a decision tree with a 24-h time horizon, we calculated costs for medication and management of three most common adverse events (AEs; oxygen saturation <90%, vomiting and somnolence) following postoperative oliceridine or morphine use. Results: Using oliceridine, the cost for managing

A cost-effectiveness and cost-benefit analysis of contingency contracting-enhanced methadone detoxification treatment.

We examined treatment costs in an ongoing study in which 102 opioid-addicted patients had been randomly assigned to either 180-day methadone detoxification or the same treatment enhanced with contingency contracting. In the latter condition, study participants received regular reinforcers contingent on negative urine toxicology screens and breath analyses for a range of drugs and alcohol. Both conditions involved psychosocial treatment, and all participants were stabilized to a daily methadone dose of approximately 80 mg during the first 4 months, followed by a 2-month taper. Individuals parti

Cost-benefit analysis of community paramedics in an ambulance service.

BACKGROUND: There is an evidence gap on the cost-benefit of community paramedics compared with usual care (emergency ambulance paramedics) for non-emergency patients. METHODS: A prospective, controlled cost-benefit analysis was conducted from both healthcare system and patient out-of-pocket perspectives over a seven-day horizon. Fifty-two presentations were included (30 intervention, 22 control). Costs were measured bottom-up using 67 variables, and consequences were quantified using Willingness-To-Pay. RESULTS: Overall, community paramedics demonstrated an incremental net benefit of $3281 p…

Benefit-cost analysis of raccoon rabies control in Ontario, Canada.

Zoonotic diseases, particularly those originating in wildlife, pose significant public health and economic risks. Rabies, a viral zoonosis with near-100% case fatality in humans, is a prime example of such a threat, especially in regions like North America where wildlife-such as raccoons-serve as key reservoirs. This study assesses the economic efficiency of Ontario, Canada's raccoon rabies control program, which combines oral rabies vaccination (ORV), trap-vaccinate-release (TVR), and surveillance strategies. Using a spatial agent-based epidemiological model, the study estimates the benefits
