Cryptogenic organizing pneumonia caused by solanine: A case report.

RATIONALE: Cryptogenic organizing pneumonia (COP) is a type of pneumonia with unknown cause, presenting with symptoms like dyspnea, fever, and cough. Solanine poisoning can cause symptoms like increased heart rate, rapid breathing, sore throat, diarrhea, vomiting, and fever, but there are no known cases of it causing COP. PATIENT CONCERNS: A 43-year-old man had a dry cough, worse at night, with phlegm and chest tightness after eating sprouted potatoes. No history of surgeries or family medical issues. DIAGNOSIS: Laboratory tests and metagenomic next-generation sequencing of bronchoalveolar l…

Cryptogenic Acute Fibrinous and Organizing Pneumonia: A Case Report.

We describe the case of a middle-aged male who presented with incidental findings of a left retrocardiac opacity seen on a chest radiograph. Subsequent computed tomography (CT) of the thorax showed multiple subpleural lesions with central necrosis. A diagnosis of acute fibrinous organizing pneumonia was made histologically upon biopsy of the subpleural lesions. The patient responded well to steroids, with full resolution of the subpleural lesions on repeat CT of the thorax. This report illustrates a case of acute fibrinous organizing pneumonia wherein the patient responded well to corticostero

Cryptogenic organizing pneumonia associated with pregnancy: A case report.

BACKGROUND: Cryptogenic organizing pneumonia (COP), formerly known as bronchiolitis obliterans organizing pneumonia, is an extremely rare disease in pregnancy. In this case, we report on COP diagnosed in recurrent pneumonia that does not respond to antibiotics in pregnant woman. CASE SUMMARY: A 35-year-old woman with no prior lung disease presented with concerns of chest pain with cough, sputum, dyspnea, and mild fever at 11 wk' gestation. She was diagnosed with community-acquired pneumonia and treated with antibiotics; her symptoms improved temporarily. Four weeks after discharge, she was re…

Hashimoto's thyroiditis-induced cryptogenic organizing pneumonia: A case report.

A 49-year-old female patient developed chest tightness and shortness of breath without apparent cause and presented to a local hospital. Che … ymptoms did not improve after broadening anti-microbial coverage. Bronchoscopy and biopsy confirmed cryptogenic organizing pneumonia (COP). Thyroid ultrasound and thyroid function tests jointly confirmed the diagnosis of Hashimoto's thyroiditis (HT). After receiving corticosteroid treatment, the patient's condition improved and she was discharged. This case demonstrates that COP …

Fatal case of cryptogenic organizing pneumonia associated with everolimus.

Noninfectious pneumonitis (NIP) has been reported with everolimus; however, the majority of the reported cases were mild to moderate. We report a fatal case of cryptogenic organizing pneumonia (COP) in a 61-year-old man. About 4 weeks after starting everolimus, the patient was admitted to the hospital with complaints of a 1-week history of progressive dyspnea with exertion and cough. The chest radiograph showed bilateral multifocal dense opacities, and he was started on antibiotics. However, his respiratory status deteriorated, and he was subsequently intubated and transferred to the intensive…

[Cryptogenic organizing non-resolving pneumonia. Report of one case].

Non-resolving pneumonia is a common clinical problem that prolongs morbidity and increases hospitalization costs. We report an 82 year-old n … attern of lung infiltrates that motivated a second biopsy, this time by open thoracotomy, showing a cryptogenic organizing pneumonia. The patient's conditions improved after treatment with adrenal steroids. In patients with non-resolving pneumonia, a dedicated and comprehensive study should be done using invasive procedures and considering both infectious and non-infectious cause …

Multiple relapses of cryptogenic organizing pneumonia: A case report.

Cryptogenic organizing pneumonia (COP) is an important subtype of idiopathic interstitial pneumonia, triggered by abnormal inflammatory responses of the lung to unknown stimuli. The disease shows good initial sensitivity to glucocorticoid therapy, but ~50% of patients experience recurrence after glucocorticoid dose reduction or withdrawal. Cases of multiple recurrences (≥3 times) are rarely reported clinically, and no unified consensus has been formed on their diagnosis and treatment strategies, making clinical management challenging. In the present study, a 53-year-old female patient was admi…

Clinicopathologic features associated with relapse in cryptogenic organizing pneumonia.

Organizing pneumonia (OP) is a histopathologic pattern of response to lung injury. Fibrin is a marker of acute microvascular injury, and variable amounts of intraalveolar fibrin are seen in OP; however, its relevance to clinical outcomes is unclear. We examined lung wedge biopsies of 26 patients with cryptogenic organizing pneumonia (COP), assessed the amount of fibrin associated with airspace organization, and correlated fibrin levels with other histologic, clinical, and radiographic findings. Seven patients with COP had disease relapse. Patients with multifocal fibrin deposits or acute fibri…

Cryptogenic organizing pneumonia (COP), as presentation of rheumatoid arthritis.

We report a 65-year-old caucasian male, who presented cryptogenic organizing pneumonia (COP) as first manifestation of rheumatoid arthritis. The patient started with fever, myalgias and progressive dyspnea in October 2004. The chest X-ray (CXR) and high resolution computed tomographic scan (HRCT) showed diffuse alveolar exudates with air bronchogram in both the lungs. An open lung biopsy was done and the histological image was compatible with COP. Six months later, a diagnosis of RA was made. Treatment with oral methotrexate and etanercept was prescribed with improvement in symptoms, physical …

Cryptogenic organizing pneumonia in two patients with Behçet's disease.

An elevated level of circulatory interleukin 6 (IL-6) is a biomarker for cytokine storm of various etiologies, including COVID-19, and contributes to poor prognosis. Vascular endothelial cells are one of the main targets of pathological action of IL-6. IL-6 activates the trans-signaling pathway via the formation of the IL-6/sIL-6Ra/gp130 receptor complex and subsequent activation of the JAK/STAT3 signaling pathway and PI3K/AKT and MEK/ERK kinases in some cases. Previously, it was shown by the authors' group and other researchers that reactive oxygen species (ROS), including mitochondrial ROS (

Cryptogenic Organizing Pneumonia with Sarcoidosis Overlap: An Atypical Case Study.

CASE SUMMARY: We present a case of a young female with subacute symptoms of cough and progressive dyspnoea. On evaluation, the patient was diagnosed as cryptogenic organizing pneumonia based on her histopathological reports. However, her significant elevation of serum angiotensin-converting enzyme (SACE) levels which drop after treatment with oral steroids, relapse, and clinical presentation pointed towards sarcoidosis as clinical diagnosis. DISCUSSION: Here, in this patient, transbronchial biopsy was suggestive of cryptogenic organizing pneumonia along with chest X-ray, and the HRCT finding …

Haemoptysis as a primary manifestation of cryptogenic organizing pneumonia (COP).

Cryptogenic organizing pneumonia (COP), previously called bronchiolitis obliterans organizing pneumonia (BOOP) is a clinicopathological disorder of unknown aetiology but increasingly reported. It usually presents with symptoms of dyspnea, cough, fever, weight loss accompanied by the presence of alveolar opacities on chest radiograph. Haemoptysis, described as blood streaking has only rarely been reported as primary presentation of COP. Herein, we report a case of COP in which submassive haemoptysis was the main clinical manifestation. The clinical, radiological, pathological, and therapeutic a…

Seven patients with cryptogenic organizing pneumonia succesfully treated with clarithromycin.

Cryptogenic organizing pneumonia is a type of idiopathic interstitial pneumonia with a subacute presentation characterized by variable degrees of cough and dyspnea. As a consequence of the upregulation of inflammatory mediators in cryptogenic organizing pneumonia, it typically responds to anti-inflammatory therapy. Although the majority of patients recover completely with long term oral corticosteroids, relapse is common and their long term use is associated with many adverse effects. Macrolides antiinflammatory agents that appear effective and safe for the treatment of cryptogenic organizing …

Different Responses to Clarithromycin in Patients with Cryptogenic Organizing Pneumonia.

Cryptogenic organizing pneumonia (COP) is an idiopathic interstitial pneumonia characterized by a subacute course and favorable prognosis with corticosteroids. However, some patients show resistance to steroids. Macrolides have been used with success in those patients showing resistance to steroids. A few reports showed treatment failure with macrolides in patients with COP who were resistant to steroids. In this report, we described two cases of COP who showed different responses to clarithromycin. One recovered completely, but the other gradually showed lung fibrosis with clarithromycin.

[Clinical features of reversed halo sign in cryptogenic organizing pneumonia].

Reversed halo sign (RHS) is often seen in computed tomography (CT) scans of cryptogenic organizing pneumonia (COP). To investigate its clinical features, we retrospectively reviewed 30 cases of COP in 13 men and 17 women, whose age range 28 to 73, with a mean of 58.4 years. All diagnoses were made with transbronchial lung biopsy (TBLB), but it took an average of 24.8 days from the first visit until the diagnosis of COP. RHS was seen in 7 cases (23%) and multiple RHSs were seen in 3 cases. We treated 5 cases (71%) with steroids. Their CT images showed parenchymal abnormalities which started as …

Findings on cryptogenic organizing pneumonia: a case report and literature review.

A 70-year-old man presented to our hospital because of a cough with bright red blood for 1 month. A chest computed tomography (CT) scan show … he lumen of distal airspaces and small bronchioles. These findings supported the diagnosis of focal cryptogenic organizing pneumonia. The patient was then treated with ceftazidime and prednisone. After this treatment, the patient visited the clinic and complete resolution of his respiratory symptoms and nearly complete resolution of the mass on chest CT were observed. The finding …

Immunophenotyping of T Cells in Lung Malignancies and Cryptogenic Organizing Pneumonia.

Background: Lung malignancies, including cancerous lymphangitis and lymphomas, can mimic interstitial lung diseases like cryptogenic organizing pneumonia (COP) on imaging, leading to diagnostic delays. We aimed to identify potential biomarkers to distinguish between these conditions. Methods: We analyzed bronchoalveolar lavage fluid from 8 patients (4 COP, mean age 59.8 ± 13.5 years; 4 lung malignancies including 2 cancerous lymphangitis, 1 MALT lymphoma, and 1 diffuse large B cell lymphoma, mean age 67.8 ± 4.5 years) using mass cytometry with 35 T cell markers. Data were analyzed using princi…
