Cryptorchidism (Undescended Testicle).

Cryptorchidism refers to an undescended testicle, the most common genitourinary malformation in male children. It is diagnosed with history and physical examination findings, and primary care physicians play a key role in the early identification of the condition. Early surgical intervention reduces the risk of testicular cancer and preserves fertility. Patients should be referred for surgical intervention at six months of age or at the time of diagnosis if the child is older. After surgery, patients require lifelong surveillance and counseling regarding fertility implications and increased ri…

Spermatogenesis and cryptorchidism.

Cryptorchidism represents the most common endocrine disease in boys, with infertility more frequently observed in bilateral forms. It is also known that undescended testes, if untreated, lead to an increased risk of testicular tumors, usually seminomas, arising from mutant germ cells. In normal testes, germ cell development is an active process starting in the first months of life when the neonatal gonocytes transform into adult dark (AD) spermatogonia. These cells are now thought to be the stem cells useful to support spermatogenesis. Several researches suggest that AD spermatogonia form betw…

Spermatogenesis in cryptorchidism.

Cryptorchidism or undescended testis is the most frequent congenital abnormality in newborn boys. The process of testicular descent to the scrotum is controlled by hormones produced in Leydig cells, insulin-like3, and androgens. Variation in genetic and environmental factors might affect testicular descent. A failure of spermatogenesis and germ cell apoptosis resulting in infertility as well as an increased risk of neoplastic transformation of germ cell are the direct consequences of cryptorchidism in adulthood.

Cryptorchidism and hypospadias.

PURPOSE: We quantified the burden of cryptorchidism and hypospadias in the United States by identifying trends in the use of health care resources and estimating the economic impact of the disease. MATERIALS AND METHODS: The analytical methods used to generate these results were described previously. RESULTS: Cryptorchidism is managed almost exclusively in the outpatient setting and insufficient data were available on inpatient health care use. Annual inpatient hospitalizations for hypospadias decreased by 75% between 1994 and 2000 from 2,669 (2.2/100,000 children) to 849 (0.6/100,000). Betw…

[Cryptorchidism and cancer].

OBJECTIVES: To demonstrate the presence of neoplasic degeneration within non descended testicles and the effect that late orchydopexy could have in the increased incidence. METHODS: We review the medical records of 22 patients with the diagnosis of malignant testicular tumor cared for at the Department of Urology of the Hospital CQ D Comandante M. Fajardo between 1982 and 2004, looking for history of cryptorchidism, age at the time of orchydopexy (if carried out), histological diagnosis and stage, and post operative outcome. We include one case which did not belong to this series because it w…

[Cryptorchidism--current opinions].

Cryptorchidism is the most common developmental disorder of endocrine organs in boys. Incidence depends on the age of the child--is the highest in the premature infants (30%), in term infants 2-5%, in adults 0.3%. Aetiology is still unclear and it is thought to be multifactorial including: dysgenesis of the testis, endocrine abnormalities and anatomical block on the way of the descent of the testis. In 90% of cases the pathology is unilateral. Abnormal localisation of the testis leads to the histomorphological changes which may be the causes of complications. The most dangerous are: infertilit…

Nomenclature of cryptorchidism.

Despite the fact that cryptorchidism is a common congenital anomaly, our understanding of this problem is hampered by confusion and uncertainty regarding diagnosis and treatment. A major factor producing this confusion is the lack of a common terminology that describes to the reader (or listener) exactly what the author means by a given term. Herein is proposed a classification system that, if adopted and utilized, could ameliorate this problem.

LHRH and cryptorchidism.

This paper is a historic review of two studies of the treatment of cryptorchidism. One was concluded 20 years ago with native LHRH, while the other took place 14 years ago and involved one of the potent LHRH analogues. The first study was initiated at the end of 1971-at a time when the different modes of action of the decapeptide were by far not evaluated to the full extent they are now-and included ten prepubertal boys with undescended testes aged five to ten years. The treatment consisted of intramuscular application of 20 micrograms LHRH once daily for three weeks. Additionally, three boys,…

Management of cryptorchidism.

The forces guiding testicular descent have not been completely elucidated. Both testosterone and anti-Müllerian hormone might play a role. Available evidence suggests that malfunction of the testes of some sort usually precedes maldescent. The proper management of cryptorchidism has long been a controversial issue. In unilateral cryptorchidism, hormonal function and fertility are generally normal. To maximize fertility in patients with bilateral cryptorchidism, surgical treatment should be completed ideally by the first birthday. GnRH is unlikely to be of much help in initiating testicular des…

[Endocrinology of cryptorchidism].

The testicular descent into the scrotum depends on a series of complex endocrine and mechanical interactions. The first stage or transabdomi … cryptorchid children show abnormalities in their hypothalamic-pituitary-testicular axis. Certainly cryptorchidism is a syndrome with various causes. Few cases show primary anatomical abnormalities or are associated to complex congenital anomalies or to clear hormonal defects. Most cases do not recognize an evident cause and an endocrinological abnormality has b …

[Cryptorchidism and fertility].

This article describes a fertility study based on 490 cases of surgically treated undescended testicles. Some of the patients were seen very early, and others later, even after puberty. The results are assessed as a function of the onset of pregnancy, or, failing this, on the basis of spermograms, and as a function of whether one or both testicles were undescended. Among the married patients with a unilateral complaint, the paternity rate was 69 p. cent, failing to 43 per cent for bilateral cases. Their spermograms showed an average count of 33.7 X 10(6)/ml for unilateral cryptorchids, and 3.6

Cryptorchidism in adults.

Semicastration in cryptorchidism of adults is the preferential therapy. A positive effect on fertility is usually to be expected. An aggravation did not occur in any case. If for any reason, in cases of cryptorchidism or especially if only one testis is present orchidopexy is carried out, there should be a periodical check-up of the patients in order to discern in time the development of a testicular tumor.
