OBJECTIVE: Serotonin syndrome (SS) is an iatrogenic life-threatening condition caused by serotonergic agents. The treatment for SS involves the administration of a serotonin antagonist (cyproheptadine). However, the dosing schedule for cyproheptadine is not uniform in the literature.
METHODS: We retrospectively evaluated 23 adult patients (>18 years) admitted to the Neurology Department and met the Hunter criteria for SS.

The antiserotonergic agent cyproheptadine was evaluated in six patients as a medication for the management of spasticity due either to spinal cord trauma or to multiple sclerosis. Oral doses of cyproheptadine were progressively increased from 6 mg to 24 mg per day. Trial periods extended from 4 to 24 months and included a placebo substitution period. We evaluated long-term treatment with the antiserotonergic agent cyproheptadine in the management of the galactorrhea-amenorrhea syndrome. Fifteen women with a mean initial serum prolactin of 37 +/- 7 ng/mL received 16 to 24 mg of cyproheptadine daily; they had a significant decrease in prolactin concentration at 8 and at 16 weeks (P less than 0.01). In a model of conditioned feeding behavior, oral administration of cyproheptadine (1-100 mg/kg), 30 min before presentation of food, produced a dose-dependent reduction of food intake in the rat (ED50 congruent to 17 mg/kg during the 1st h of testing). This anorexic effect persisted for at least 24 h. These results provide further evidence that under certain conditions cyproheptadine, which is used as an orectic agent in man, can produce anorexia.

The teratogenic effect of cyproheptadine chlorhydrate was studied in Wistar rats. Rats were given the substance by gastric intubation on days 6-15 of pregnancy in two experimental series. In the first series rats were treated at doses of 25 and 50 mg/kg/d and in the second one they received 15, 25 and 35 mg/kg/d. Controls received only an equivalent volume of water by the same route.
