Four novel oral anticoagulant agents are currently available for the prevention and treatment of thromboembolic events in patients with nonvalvular atrial fibrillation. Dabigatran, a direct thrombin inhibitor, has robust data for the treatment of deep venous thrombosis and pulmonary embolism, stroke prevention in non-valvular atrial fibrillation, and the prophylaxis of venous thromboembolism (VTE) after knee and hip replacement. Recent studies have evaluated dabigatran to determine its safety and efficacy in such conditions as VTE in malignancy, coronary artery disease, mechanical and bioprosthetic valves, and antiphospholipid syndrome. Heparin-induced thrombocytopenia (HIT) is a potentially serious adverse drug reaction that can result in lethal vascular thrombosis. An 85-year-old female presented to hospital with haemoptysis. She underwent investigations which confirmed oesophageal submucosal haematoma. Oesophageal haematoma along with Mallory-Weiss and Boerhaave's syndromes make up acute mucosal injury of the oesophagus. It can be managed conservatively in the majority of cases.

Dabigatran etexilate is an oral anticoagulant with increased risk of bleeding at high drug exposure. It is available in three capsule strengths (75, 110 and 150 mg). We analysed New Zealand dispensing data from 2019 to 2023 to compare use between health districts. Overall use was similar across districts. Canterbury had the highest 75 mg use (22.2 vs 1.6-7.6 people per 10 000 in the other districts). This relates to differences in local guidelines, therapeutic drug monitoring and availability of clinical pharmacology services. It is unknown if there are differences in patient outcomes.
