Delayed diagnosis of duodenal rupture.

BACKGROUND: Although duodenal rupture is usually diagnosed during the course of surgery for other injuries, a small portion of such injuries occur in isolation. In such cases, the significance of the clinical and diagnostic findings may not be appreciated for an extended period. The primary determinant of mortality in duodenal rupture is the presence of associated injuries, but delay in diagnosis is often a secondary factor. METHODS: A retrospective case review of 8 patients with isolated duodenal rupture that was diagnosed more than 24 hours following the injury. RESULTS: In 5 cases, physic

Delayed diagnosis of arterial injuries.

During an 8 1/2 year period, 28 patients with a delayed diagnosis of an arterial injury in an extremity or the neck were treated. The median delay between injury and diagnosis was 10 days. The tibio-peroneal arteries were the most commonly injured vessels. After extensive analysis of the records and arteriograms of the involved patients, the following conclusions were drawn: Arteriograms are mandatory for penetrating wounds proximal to major arteries of the extremities because of the 5 to 15 percent incidence of occult injuries; the timing of arteriography in the distal leg is critical if subt…

Delay and causes of delay in the diagnosis of childhood cancer in Africa.

BACKGROUND: Although a few studies have investigated delays in diagnosis and treatment among children and adolescents with cancer, this has never been subject of study in South Africa. Early diagnosis is fundamental as it allows timely treatment and prevents unnecessary complications. PROCEDURES: Combined prospective and retrospective study of 194 children with cancer at Tygerberg Hospital, Cape Town, diagnosed between 2000 and 2009: 126 patients were included through review of the medical charts and 68 through interviews with the parents. RESULTS: The median total diagnosis delay was 34 day

"Delayed rupture of the spleen" or delayed diagnosis of the splenic injury?

Splenic rupture is the most frequent intraabdominal injury following blunt abdominal trauma. Massive hemorrhage commonly occurs from injuries to this friable vascular organ. The mortality rate from simple splenic rupture is 1%. Delayed diagnosis of a ruptured spleen increases the rate to 10%. During 1964-79, 293 patients underwent splenectomy for blunt splenic injury, of whom 278 were operated on within 24 h. All had typical signs of splenic lacerations with intraperitoneal bleeding from the time of injury. Fifteen patients were operated on more than 48 h after sustaining the injury. A detaile…

Delayed Diagnosis of Ectopic Cushing Syndrome.

Here, we present the case of a 40-year-old man in whom the diagnosis of ectopic adrenocorticotropin (ACTH) syndrome went unrecognized despit … med by histopathology, resulting in surgical cure. While many patients with Cushing syndrome have a delayed diagnosis, this case highlights the critical need to increase awareness of the signs and symptoms of hypercortisolism and to improve the understanding of appropriate screening tests among nonendocrine providers.

A Delayed Diagnosis of Myxedema Coma.

A 58-year-old man without any personal or familial cardiac history presented to the emergency department with complaints of worsening left-sided chest pain that began at rest, described as a burning sensation and escalating to a 9/10 severity. He denied any personal or familial cardiac history but admitted that he had not been medically evaluated in approximately five years. His physical examination was notable for obesity, xerosis, macroglossia, and bilateral lower extremity edema. His initial labs demonstrated critical troponin levels that peaked at 11.5 ng/mL and he was diagnosed with a myo

Delayed diagnosis of adult-onset mastocytosis.

Urticaria pigmentosa is most frequently observed in children. The associated symptomatology in mastocytosis is vague and not always diagnostic, and Darier's sign is often negative, making the diagnosis challenging. We describe a case of a 61-year-old man with extensive brown papules and macules in different locations of the body that slowly progressed in the last 10 years with a previous anaphylactic reaction after an ant bite. This suggests that urticaria pigmentosa in an adult should be furthered assessed for the possibility of systemic involvement for a correct diagnosis and treatment.

Ruptured Rudimentary Horn Pregnancy: Delayed Diagnosis.

Developmental anomalies of the genital tract result from defective fusion and absorption of various parts of Mullerian ducts in fetal life. Rudimentary horn pregnancy is a rare occurrence of one in 76,000 and one in 160,000. We present a case of a 24-year-old primigravida with ruptured rudimentary horn pregnancy initially managed in the line of an intrauterine pregnancy with severe anemia. Hemodynamic instability made us suspect ruptured rudimentary horn pregnancy and lifesaving laparotomy was performed for the same. A 1.5-liter hemoperitoneum was encountered with a right ruptured rudimentary

Delayed Diagnosis of McCune-Albright Syndrome.

BACKGROUND: McCune-Albright syndrome (MAS) is a rare heterogeneous genetic disorder that is characterized by a triad of polyostotic fibrous … d thyroid hormones since the age of 3½ years. The child developed severe morbidity as the result of delayed diagnosis and currently became wheelchair dependent. CONCLUSION: Thorough patient evaluation and appropriate interpretation of findings are crucial steps for timely diagnosis of MAS and better patient care outcomes.

[Peripartum cardimyopathy - risk of delayed diagnosis].

Peripartum cardiomyopathy is defined as heart failure, with ejection fraction less than 45% that presents late during pregnancy or the first five months postpartum. Despite being described first in 1849 by Ritchie the mechanisms behind the disease are still not fully understood. However, oxidative stress during pregnancy and the cleavage of prolactine into its 16 kDa fragment appears to play a role in the pathophysiology of peripartum cardiomyopathy. In addition to optimal therapy for heart failure bromocriptine, an inhibitor of prolactine release, should be considered. Prevalence and prognosi

Delayed diagnosis of postcardiac injury syndrome.

Postcardiac injury syndrome (PCIS) is a rare condition that is considered to have a trauma-induced autoimmune mechanism triggered by damage to pericardial and/or pleural tissues. We report a case of PCIS accompanied by systemic oedema after thymectomy. A 73-year-old woman was referred to our hospital for dyspnoea and oedema, 9 months after thymectomy. Evaluation revealed the presence of pericardial effusion, pleural effusion and systemic oedema. Differential diagnosis included constrictive pericarditis (secondary to tuberculosis), serositis caused by collagen disease and malignancy. Detailed i

Delayed Ejaculation: Pathophysiology, Diagnosis, and Treatment.

Delayed ejaculation (DE) is a poorly defined and uncommon form of male sexual dysfunction, characterized by a marked delay in ejaculation or an inability to achieve ejaculation. It is often quite concerning to patients and their partners, and sometimes frustrates couples' attempts to conceive. This article aims to review the pathophysiology of DE and anejaculation (AE), to explore our current understanding of the diagnosis, and to present the treatment options for this condition. Electronic databases were searched from 1966 to October 2017, including PubMed (MEDLINE) and Embase. We combined "d

Vaginal foreign body: a delayed diagnosis.

BACKGROUND: To describe a case of prolonged vaginal bleeding in a prepubertal girl. Review of medical record and literature search. CASE: A 7-year-old female was referred to our pediatric and adolescent gynecologic clinic for evaluation of vaginal bleeding and ovarian cyst on ultrasonography. Her parents denied any history of trauma or sexual abuse. Initial evaluation revealed pre-pubertal luteinizing hormone and follicle stimulating hormone levels, and follow-up ultrasonography revealed normal pre-pubertal pelvic anatomy. However, a skeletal survey, which was obtained to assess for the prese

Hereditary spherocytosis: Consequences of delayed diagnosis.

OBJECTIVE: To determine whether patients with undiagnosed hereditary spherocytosis hospitalized for transfusions might have avoided hospitalization via earlier diagnosis. STUDY DESIGN: Charts of all (N = 30) patients with hereditary spherocytosis seen in pediatric hematology at West Virginia University-Charleston were reviewed. Family and transfusion history and presence of neonatal jaundice were recorded. Complete blood count and reticulocyte values during infancy were available for 20 of 30 patients, while baseline steady-state values were available for all 30. RESULTS: Transfusions were g

A delayed diagnosis of cardiac tamponade.

A 64-year-old female was referred to the on-call surgical team by her general practitioner with a 2-week history of feeling generally unwell; more short of breath with lethargy and myalgia with a 2-day history of right upper quadrant abdominal pain. On initial assessment she was tachycardic and an abdominal examination revealed a soft abdomen with right upper quadrant tenderness. An abdominal ultrasound scan revealed a distended gall bladder with the diameter of the common bile duct at the upper limit of normal. It also revealed free fluid within the abdomen, bilateral pleural effusions and a

Delayed diagnosis of juvenile primary hyperparathyroidism.

Primary hyperparathyroidism (PHP) is most common in adults in the fifth and sixth decades of life, but is seen infrequently in adolescence and is extremely rare in children under the age of 14 years. We report the case of a young female whom we suspect had PHP from the age of 6 years. This condition remained undiagnosed for the next 11 years in spite of repeated hospital admissions. The hypercalcaemia which was initially labelled as 'idiopathic' was eventually found to be secondary to a parathyroid adenoma, which was treated successfully by surgery. The case illustrates two important advances:
