This study attempts to evaluate the oral manifestations of and the limited available dental pulp information on diabetes mellitus, a common metabolic disorder of carbohydrate and lipid metabolism affecting over 16 million Americans. Diabetics are particularly prone to bacterial or opportunistic infections. This vulnerability is caused by a generalized circulatory disorder whereby the blood vessels are damaged by the accumulation of atheromatous deposits in the tissues of the blood vessels lumen. In addition, blood vessels, particularly capillaries, develop a thickened basement membrane, which …

Infection of the root canal system following dental trauma induces pulp and periapical disease and prevents healing of previously healthy pulp. A clinical goal in treating trauma is the maintenance of pulp vitality, and clinicians should be aware of factors that influence pulp healing. The learning objective of this article is to review the factors and techniques that influence pulp vitality and examine the influence pulp has on the healing of adjacent tissues. The potential routes for bacterial infection of the root canal system are discussed, with the clinical crown as the primary portal of

Dental caries, trauma, and other possible factors could lead to injury of the dental pulp. Dental infection could result in immune and inflammatory responses mediated by molecular and cellular events and tissue breakdown. The inflammatory response of dental pulp could be regulated by genetic and epigenetic events. Epigenetic modifications play a fundamental role in gene expression. The epigenetic events might play critical roles in the inflammatory process of dental pulp injury. Major epigenetic events include methylation and acetylation of histones and regulatory factors, DNA methylation, and…

The NLRP3/caspase-1 inflammasome pathway plays an important role in cellular immune defence against bacterial infection; however, its function in human dental pulp tissue and human dental pulp fibroblasts remains poorly understood. We demonstrate that NLRP3 protein expression occurs to a greater extent in pulp tissue with irreversible pulpitis than in normal pulp tissue and in tissue with reversible pulpitis. Caspase-1 is present in its active (cleaved) form only in pulp tissue with irreversible pulpitis. NLRP3 and caspase-1 are expressed in the odontoblast layers in normal human dental pulp t…

In this second part of our 2-part review, we discuss recent research about pulp tests that determine the vitality of the tooth and clinically accepted pulp testers. A pain response to hot, cold or an electric pulp tester indicates the vitality of only a tooth's pulpal sensory supply; the response does not give any idea about the state of the pulp. Although the sensitivity of these tests is high, when false-positive and false-negative results occur, they may affect the treatment of the tooth. A tooth falsely diagnosed as nonvital with an electric pulp tester may undergo an unnecessary root cana

Chemical decomposition and fragmentation may limit the detection of ancient host and microbial DNA while some proteins can be detected for extended periods of time. We applied paleoproteomics on 300-year-old dental pulp specimens recovered from 16 individuals in two archeological funeral sites in France, comprising one documented plague site and one documented plague-negative site. The dental pulp paleoproteome of the 16 teeth comprised 439 peptides representative of 30 proteins of human origin and 211 peptides representative of 27 proteins of non-human origin. Human proteins consisted of conj…

Dental pulp is a soft mesenchymal tissue densely innervated by afferent (sensory) fibers, sympathetic fibers, and parasympathetic fibers. This complexity in pulp innervation has motivated numerous investigations regarding how these 3 major neuronal systems regulate pulp physiology and pathology. Most of this research is focused on neuropeptides and their role in regulating pulpal blood flow and the development of neurogenic inflammation. These neuropeptides include substance P, calcitonin gene-related peptide, neurokinin A, neuropeptide Y, and vasoactive intestinal polypeptide among others. Th…

Common pathologies of the dental pulp differentiate between acute and chronic inflammatory states caused by caries or dental trauma. Inflammations of the dental pulp as a result of neoplastic alterations are generally considered non-existent. In fact, using the search phrase "dental pulp" combined with "sarcoma", "carcinoma", or "neoplasms" in PubMed when using the MeSH search mode yielded no reports on primary malignant neoplasms. However, a hand search yields clinical reports on pulpal tumours that were published over a century ago. In this Essay, the results of a hand search in historic pub…

We have looked for the presence of beta-endorphin and somatostatin in normal and inflamed pulps. In 25 adult rats, under anesthesia, small o … s (t test, p < 0.05). beta-endorphin and somatostatin are both produced in increased amounts in the dental pulp during inflammation attributable, at least in part, to the presence of T lymphocytes producing these substances.

Uncontrolled or poorly controlled diabetes mellitus may be a risk factor for the development of oral complications. The objective of this investigation was to determine the effect of diabetes mellitus progression on inflammatory and structural components of dental pulp. Male Wistar rats were given a single injection of Streptozotocin (STZ), and induction of diabetes was confirmed 24 h later. Dental pulp tissue samples were taken from central incisors and molars of diabetic rats 30 and 90 days after the STZ treatment. Plasma glucose levels in diabetic rats 30 and 90 days after STZ treatment wer…

We investigated which structural components are responsible for maintaining interstitial fluid equilibrium in the pulpal tissue, for which the existence of an effective lymph drainage is postulated. There have been only a small number of investigations on pulpal lymph tissue. Therefore, we decided to perform a detailed structural analysis. Twenty vital, healthy teeth that had to be extracted for orthodontic reasons were immersed in Patent Blue for 10 to 15 minutes after opening the pulpal cavity. They were then extracted and the dental pulps were opened by cleavage of the surrounding hard toot…

Recent studies have described microleakage under full crowns cemented with several different cements. This study tested three different types of crown margin preparations--a chamfer, a shoulder, and a shoulder plus a bevel to determine whether or not the margin preparation could affect microleakage. All crowns were cemented with zinc phosphate cement. The crowns were tested for leakage in thermocycled dye. All crowns demonstrated significant leakage following the path of the dentinal tubules into the pulp. This could possibly be one of the causes of pulpal inflammation and even pulpal death un
