Comprehensive mixed dentition treatment.

At the University of the Pacific, mixed dentition treatment is an integral part in the graduate program in orthodontics. One patient had a severe skeletal ANB discrepancy of 7 degrees, lower incisors crowding, and 12 mm of overjet. He was considered an excellent candidate for Phase I treatment to reduce the severity of the problem. It is our philosophy to take advantage of the growth potential in a patient by initiating Phase I treatment between the ages of 7 and 9 years, depending on the eruption of the permanent first molars and incisors and the severity of the malocclusion. We use leeway sp…

Mixed dentition case report.

Malocclusions can be successfully corrected by phase I mixed dentition therapy and significantly reduce the need for comprehensive orthodontic treatment in the permanent dentition. Phase I orthodontic treatment is designed to correct the skeletal and dental malocclusions at an early age with 12 to 18 months of treatment. After phase I treatment, there is a supervision phase of typically 18 to 30 months to hold the correction achieved and to supervise the eruption of the remaining permanent teeth. Good management during the supervision phase is key to maintaining the phase I result and to minim…

A Deep Learning Approach to Automatic Tooth Caries Segmentation in Panoramic Radiographs of Children in Primary Dentition, Mixed Dentition, and Permanent Dentition.

OBJECTIVES: The purpose of this study was to evaluate the effectiveness of dental caries segmentation on the panoramic radiographs taken from children in primary dentition, mixed dentition, and permanent dentition with Artificial Intelligence (AI) models developed using the deep learning method. METHODS: This study used 6075 panoramic radiographs taken from children aged between 4 and 14 to develop the AI model. The radiographs included in the study were divided into three groups: primary dentition (n: 1857), mixed dentition (n: 1406), and permanent dentition (n: 2812). The U-Net model implem…

Duplication of the premolar dentition.

Multiple supernumerary teeth, often associated with various syndromes, is a relatively uncommon isolated dental anomaly. We present a case of a 9-year-old black male patient with complete duplication of the premolar dentition and premolarization of the permanent canines.

Dental anomalies in the primary dentition and their repetition in the permanent dentition: a diagnostic performance study.

This study investigated the recurrence in the permanent dentition of dental anomalies of the primary dentition. A sample of 189 subjects (100 males, 89 females, mean age of 5 years and 7 months) with anomalies of primary teeth (tooth hypodontia, supernumerary teeth, geminated teeth, and fused teeth) was selected and re-analyzed at a mean age of 11 years and 2 months for the recurrence of the dental anomalies in the permanent dentition. As a control group, 271 subjects (123 males, 148 females) without dental anomalies in the primary dentition were selected. The recurrence in the permanent denti…

Solitary mesiodens in primary dentition.

This article reports a rare case of a preschool-aged girl child with a supernumerary tooth in the maxillary anterior region between the primary central incisors. Clinical and radiographic evaluations confirmed the presence of a conical supernumerary tooth without any associated pathology or anomalies in the developing permanent dentition. The supernumerary tooth was extracted under local anaesthesia to prevent obstruction or deflection of the permanent central incisors. Postoperative healing was uneventful, with follow-up confirming satisfactory recovery. The article highlights the importance …

Molariform mesiodens in primary dentition.

Mesiodens is a midline supernumerary tooth commonly seen in the maxillary arch, and incidence of molariform mesiodens in the maxillary midline is rare in permanent dentition and extremely uncommon in primary dentition. A midline supernumerary tooth in the primary dentition can cause ectopic or delayed eruption of permanent central incisors which will further alter occlusion and may compromise esthetics and formation of dentigerous cysts. This paper reports a rare case of the presence of a molariform mesiodens in the primary dentition. On clinical and radiographic examination, flaring of the pr…

Compound odontoma in deciduous dentition.

Odontomas are benign tumors containing all the components of the odontogenic tissue. Disturbances in tooth eruption are one of the most common complications associated with odontomas. They are generally asymptomatic and are usually seen in second and third decades of life, rarely seen below 10 years of age. Herewith, we report an unusual occurrence of compound odontoma in a 4-year-old child visualized as a swelling of the cortical bone in the anterior maxilla.

Hypophosphatasia affecting the permanent dentition.

Reports on dental abnormalities in connection with hypophosphatasia almost exclusively describe changes in primary teeth. A 23-year-old man with hypophosphatasia, first diagnosed at the age of 8 months, is described; histologically and radiographically verified signs of the condition were present in the permanent dentition. The findings included a reduced level of the marginal alveolar bone supporting the upper central incisors, which had to be extracted. The molars displayed large coronal pulp chambers. Histologically, the upper incisors demonstrated abnormal root cementum, with areas of dent…

Comparison of the pattern of agenesis in the primary and permanent dentitions in a population characterized by agenesis in the primary dentition.

The first aim of this study was to collect a large sample of radiographs from children with congenitally missing teeth (CMT) in the primary dentition and to analyse the local relationship between agenesis of a primary tooth and the presence/absence of its permanent successor. The second aim was to compare, in the same sample, the pattern of agenesis in the primary dentition with the developmental pattern seen in the permanent dentition. 124 dentists from 72 municipalities contributed to the investigation of a total of 213 children. The dentists were asked to lend existing radiographic material…

[From the mixed dentition to the permanent dentition: how to manage space while guiding eruption?].

In the mixed dentition, when there is a discrepancy between the bony structures and the teeth, in certain cases, serial extractions are performed in order to guide the eruption of permanent teeth and obtain a correct occlusal function. The treatment consists in the extraction of primary teeth and then of first bicuspids. The choice of the first tooth to be extracted is based upon the position of the crowns of the permanent teeth and their degree of root maturation. Then the first bicuspid is extracted as soon as or prior to its eruption. This interceptive treatment has limited indications whic…

Development of Yorkshire Terrier Dentition.

The development of dentition in dogs has been associated with several problems including tooth over-crowding, missing permanent dentition, and persistent deciduous teeth (PDT). Information on dentition development in different breeds is lacking. This study of 61 Yorkshire terriers aimed to determine the (i) average age at deciduous tooth exfoliation, (ii) average age at permanent tooth eruption, (iii) PDT incidence, and influencing factors such as body weight. The ages of exfoliation of deciduous teeth and eruption of permanent dentition were influenced by body weight and tooth type. These den…

Supplemental tooth in primary dentition.

An extra tooth causing numerical excess in dentition is described as supernumerary tooth, and the resultant condition is termed as hyperdontia. Hyperdontia is more commonly seen in the permanent dentition than primary one. Supernumerary tooth which resembles tooth shape and supplements for occlusion is called as supplemental tooth. We present a case with supplemental tooth in primary dentition.
