A double-blind study of 43 patients undergoing excision of haemorrhoids under spinal anaesthesia was carried out to compare the analgesic effects of diflunisal (DFL, 21 patients) and dextropropoxyphene napsylate (DPN, 22 patients) on post-operative pain. The effects of dextropropoxyphene on the steady-state kinetics of oxcarbazepine and its metabolites were investigated in eight patients with epilepsy or trigeminal neuralgia. Paracetamol and dextropropoxyphene were implicated on clinical and analytical grounds in 237 poison deaths in England and Wales in 1979. We studied deaths following intoxication with dextropropoxyphene (D) and opioids (M) in Denmark 1979-1992 with special reference to the sex, age group, contributory cause of death (secondary diagnosis) and manner of death. Deaths following D increased until 1985 for both sexes, where a total of 46 women and 64 men died. The authors report a prospective study on 944 cancer pain patients treated with one of the following opioids: codeine, oxycodone, dextropropoxyphene, buprenorphine, and pentazocine. Level of analgesia, duration of treatment, side effects, and drop out were evaluated for each drug. To compare the characteristics of dextropropoxyphene (DXP) poisoning victims with those of victims of poisonings by antidepressants and sedatives, we examined all fatal poisonings due to DXP, antidepressants or sedatives among autopsies performed at one department of forensic medicine in Sweden during the six-year period from 1992 to 1997. The plasma elimination half-life of dextropropoxyphene and its metabolite nordextropropoxyphene was investigated in seven elderly hospital patients after the administration of both single and multiple doses of Distalgesic. The mean elimination half-life of dextropropoxyphene after multiple dosing was 35.7 h (range 24.0-50.6 h) and the mean half-life of nordextropropoxyphene was 53.3 h (range 25.1-76.3 h). The half-lives of both dextropropoxyphene and nordextropropoxyphene are much longer than those reported by other investigators in younger subjects.
