The erythropoietic activity and erythrocyte 2,3-diphosphoglycerate (2,3-DPG) were studied during and after the nadir of the post-natal anaemia in normal, rapidly growing rabbits, from the 12th to the 35th day after birth. The in vitro effects of a solution of fructose-1,6-diphosphate on hemoglobin-oxygen affinity and electrolyte distribution between plasma and red cells were evaluated. Fructose-1,6-diphosphate increased intracellular pH and decreased extracellular pH. There were no shifts in Na+, K+, Ca++ and Cl- between plasma and red cells. Red cell content of 2,3-diphosphoglycerate remained unchanged, while P50 was reduced (as a consequence of the intracellular alkalosis). The therapeutical implications of these results are discussed.

Erythrocyte 2,3-diphosphoglycerate (2,3-DPG) and adenosine triphosphate (ATP) were measured in 16 patients with polycythaemia vera and found to be normal. The determination of erythrocyte organic phosphate concentration can be helpful in the differential diagnosis of different types of polycythaemia due to the lack of 2,3-DPG rise in patients with polycythaemia vera, as compared to the 2,3-DPG increase usually observed in patients with secondary polycythaemia.

The authors assessed the levels of lactate and 2,3 diphosphoglycerate in the umbilical blood of 105 full-term neonates. A significant increase was found in the levels of 2,3 diphosphoglycerate in newborns of mothers with a history of imminent chronic intrauterine hypoxia. The lactate levels of these newborns were the same as in children of mothers without a history showing a risk of intrauterine hypoxia. The psychomotor development of newborns with increased levels of 2,3 diphosphoglycerate was significantly more altered than in those with normal levels in their first year of life.

OBJECTIVE: To describe a clinical case where an extremely low erythrocyte 2,3-diphosphoglycerate concentration (2,3-DPG) was discovered by routine blood gas analysis supplemented by computer calculation of derived quantities. The finding of a low 2,3-DPG revealed a severe hypophosphatemia.
DESIGN: Open uncontrolled study of a patient case.
SETTING: Intensive care observation during 41 days.
PATIENT: A 44 year old woman with an abdominal abscess.

Erythrocyte 2,3-DPG, PO2 50%, whole blood hemoglobin and available O2, and fixed acid Bohr effect were studied during the first 10 days after birth in rapidly growing suckling rabbits. The post-natal fall in hemoglobin concentration was accompanied by a marked rise in erythrocyte 2,3-DPG and a gradual increase in PO2 50%. The rise in PO2 50% was sufficient to keep the available O2 of the blood unchanged throughout the observation period.
