[Effectiveness of donor selection in a blood bank].

Transfusion safety in Mexico depends on 2 essential pillars: The proper clinical selection of donors and the effective application of screen … ning tests already performed on them. In this context, in this opinion article the effectiveness of donor selection and screening strategies was analyzed using the findings of an international collaborative study that analyzed pooled samples through massive parallel sequencing. Encouragingly, no traces of HIV, HBV, or HCV were detected, which suggests a favorable …

Risk analysis of extended pancreas donor selection criteria.

INTRODUCTION: The success of pancreas transplantation, in combination with a stable number of available allografts has resulted in an increasing waiting list. This study investigated donor potential by expanding age and Body Mass Index (BMI) criteria. METHODS: All reported donors in the Netherlands between 2013 and 2017 were analysed. Risk assessment of extended criteria donors was done by in-depth analysis of donor reports and calculation of the Pancreas Donor Risk Index (PDRI). The PDRI of these extended criteria donors was compared to standard criteria donors to evaluate the increased risk

Reasons for Donor Lungs Unsuitability After Radiographic Selection.

OBJECTIVES: Deceased lung donation requires optimum donor care, with lung viability greatly affected by the progress of the disease leading to brain death. In this study, we searched for causes of lung unsuitability for transplant in both primary and secondary evaluations. MATERIALS AND METHODS: Primary lung evaluations included chest radiography, oxygen challenge test, and donor clinical and history examination. Secondary evaluations for lungs that passed the first step included fiberoptic bronchoscopy and donor-recipient matching. RESULTS: From 2012 to 2017, there were 1020 potential decea

Brain-dead kidney donor: selection, care, and administration.

Over four and a half years a district general hospital provided 34 cadaveric kidneys for transplantation. All brain-dead patients were regarded as potential donors, flow charts being used to maintain circulation and urine formation and facilitate administration. With this system the time lapse between diagnosis of brain death and removal of kidneys ranged from three to six hours and ischaemia was minimised. It is concluded that adoption of the system by other hospitals of comparable size would result in enough good-quality kidneys to satisfy present needs, thus reducing the initial high failur

Optimizing donor selection in a cord blood bank.

OBJECTIVES: The main limitation factor for the wide use of umbilical cord blood (UCB) as a source of hematopoietic progenitor for transplantation is cell dose. One of the specific areas identified by some studies for improvement of UCB collection is donor selection. METHODS: Over a 3-mth period, 391 consecutive maternal-neonatal pairs were evaluated during the pre-partum period in the maternity ward at La Fe University Hospital (Valencia) by the Cord Blood Bank staff. Reasons for discarding umbilical cord blood donors and collected UCB units at the Cord Blood Bank in Valencia have been analys…

Donor selection: the exclusion of high risk donors?

Selection of donors is an important means to improve the overall safety of the blood supply. Since the AIDS epidemic emerged and after the i … epth questioning about risk behaviour, orally as well as by questionnaires. Although this policy of donor selection is recognized by most blood centers in the world, the efficacy of this selection has not been well documented. Therefore in future, studies should be performed to base these selection criteria on evidence.

Outcomes of strategic alternative donor selection or suspending donor search based on Japan Marrow Donor Program coordination status.

In allogeneic hematopoietic stem cell transplantation (allo-HSCT) from unrelated donors, delays in donor search are adversely associated with patient outcome. However, the optimal duration for either waiting for an unrelated donor or selecting alternative sources remains undetermined. Using data from the Japan Marrow Donor Program (JMDP) registry, we retrospectively analyzed 349 adult patients who had searched for unrelated donors. Two hundred and three patients received allo-HSCT from JMDP donors (Group A) with a median of 140 days required to identify a donor, 60 received allo-HSCT from alte

Current perspectives on living donor selection in liver transplantation.

The careful selection of donors is crucial to achieving a successful outcome in living donor liver transplantation. The evaluation process involves obtaining a comprehensive medical history and pertinent laboratory testing, evaluating surgical anatomy using cross-sectional radiologic imaging and understanding donor motivation and psycho social considerations. This review outlines the evaluation of a potential living liver donor and discussed frequently encountered special considerations that may need to be addressed by the transplant team.

Donor liver histology--a valuable tool in graft selection.

BACKGROUND: Due to a tremendous organ shortage, livers from donors with extended criteria are increasingly considered for transplantation. Pathologists are more and more requested to evaluate these livers histopathologically using frozen sections at high urgency for acceptability. METHODS: This article reviews the current knowledge on pre-transplant histology in liver transplantation. Prerequisites and conditions for proper pre-transplant evaluation of donor liver tissue are discussed as well as frozen section evaluation and reporting. Data sources include the relevant medical literature, web

Living donor liver transplantation: selection, perioperative care, and outcome.

A perception that living donor liver transplantation can be accomplished with an acceptable donor complication rate and recipient survival rate has led to the acceptance of living donor liver transplantation as a viable alternative to decreased deceased donor transplantation. Careful candidate evaluation and selection has been crucial to the success of this procedure. Advancements in the understanding of the lobar nature of the liver and of liver regeneration have advanced the surgical technique. Initial attempts at adult-to-adult donation utilized the left hepatic lobe, but now have evolved i

Noncompliance with blood donor selection criteria - Complidon 2017, France.

BACKGROUND: Blood donor selection, consisting of a pre-donation questionnaire and interview, excludes potential donors who may be at risk of transfusion-transmissible infections. Assessing the reasons for noncompliance with blood donor selection criteria is important to maintain a high level of viral safety of blood products. STUDY DESIGN AND METHODS: An anonymous French online survey of a sample of blood donors (Complidon) was conducted from September to December 2017. Data were poststratified to be representative of all donors who donated blood between July 2016 and December 2017. RESULTS:…

[Blood donor selection in providing anticytomegalovirus-negative blood components].

Sera of 1731 blood donors of the Ulm blood bank were screened for cytomegalovirus (CMV) antibodies. Prevalence of these antibodies was 37%. The seroconversion rate among 1090 anti-CMV-negative blood donors was 13 per thousand per year for the whole donor population over a period of 27 months and depended on age. It was 16 per thousand per year for donors aged 18-40 years, but only 7 per thousand for those over 41 years old. Seropositivity was higher in women than men. To provide anti-CMV-negative blood requires a prescreened anti-CMV-negative donor group and the rescreening of each further don

An Original Approach to Evaluating the Quality of Blood Donor Selection: Checking Donor Questionnaires and Analyzing Donor Deferral Rate.

Blood donor selection is a cornerstone for blood transfusion safety, designed to safeguard the health of both donors and recipients. In the Service du Sang, Belgian Red Cross, French and German-speaking part of Belgium (SFS), health professionals (HPs) are allowed to interview donors on their own after formal qualification. This qualification is afterward evaluated by means of two complementary quality indicators: monitoring of donor health questionnaires (DHQs) and analysis of donor deferral rate. The study aims to evaluate the degree to which both quality indicators may be useful and appropr…

Expanding the thoracic donor Pool: Ethical implications of contemporary donor selection and organ preservation strategies.

BACKGROUND: To address high waiting-list mortality and severe donor shortages in thoracic transplantation, clinical protocols have expanded donor selection criteria to include marginal, elderly, and controlled donation after circulatory death (cDCD) candidates. Concurrently, ex-vivo lung perfusion (EVLP), ex-vivo heart perfusion (EVHP), and normothermic regional perfusion (NRP) have reconfigured organ preservation. This study evaluates the clinical-ethical challenges triggered by these strategies to optimize graft utilization safely. METHODS: A systematic review was conducted evaluating peer-…
