Comparative efficacy of combination drug therapy in refractory epilepsy.

OBJECTIVE: We retrospectively examined treatment records of developmentally disabled adults with highly refractory epilepsy to determine whether any combinations of 8 of the most commonly used antiepileptic drugs (AEDs) possessed superior efficacy. METHODS: We obtained the treatment records from 148 developmentally disabled adults with refractory epilepsy cared for in 2 state-run institutions. These records charted monthly convulsive seizure occurrence and AED regimen over 30 years. We studied the effects of 8 commonly used AEDs alone and in combination on seizure frequency in within-patient

Influenza Treatment: Limitations of Antiviral Therapy and Advantages of Drug Combination Therapy.

Influenza infection is serious and debilitating for humans and animals. The influenza virus undergoes incessant mutation, segment recombination, and genome reassortment. As a result, new epidemics and pandemics are expected to emerge, making the elimination challenging of the disease. Antiviral therapy has been used for the treatment of influenza since the development of amantadine in the 1960s; however, its use is hampered by the emergence of novel strains and the development of drug resistance. Thus, combinational therapy with two or more antivirals or immunomodulators with different modes o

Combination Drug Therapy for the Management of Chronic Neuropathic Pain.

Chronic neuropathic pain (NP) is an increasingly prevalent disease and leading cause of disability which is challenging to treat. Several distinct classes of drugs are currently used for the treatment of chronic NP, but each drug targets only narrow components of the underlying pathophysiological mechanisms, bears limited efficacy, and comes with dose-limiting side effects. Multimodal therapies have been increasingly proposed as potential therapeutic approaches to target the multiple mechanisms underlying nociceptive transmission and modulation. However, while preclinical studies with combinat

Drug synergies and the potential for combination therapy in onychomycosis.

Characterized by abnormal proliferation and migration of vascular smooth muscle cells (VSMCs), neointima hyperplasia is a hallmark of vascular restenosis after percutaneous vascular interventions. Vaccinia-related kinase 1 (VRK1) is a stress adaptionassociated ser/thr protein kinase that can induce the proliferation of various types of cells. However, the role of VRK1 in the proliferation and migration of VSMCs and neointima hyperplasia after vascular injury remains unknown. We observed increased expression of VRK1 in VSMCs subjected to platelet-derived growth factor (PDGF)-BB by western blott

[Combination drug therapy with individualized bronchoscopic intervention for pulmonary mucormycosis].

This article reported a 46-year-old male patient with pulmonary mucormycosis and a history of poorly controlled diabetes. Chest imaging revealed rapidly progressive patchy opacities and multiple cavities in both lungs. Bronchoscopy showed mucor plug obstructing multiple segmental and subsegmental bronchi. The patient underwent sequential treatment, including local nebulization, oral/intravenous antifungals, and bronchoscopic removal of mucor plugs combined with intrabronchial instillation of amphotericin B deoxycholate. However, this yielded no significant improvement. During the procedure, ma

Electrospun patches to deliver combination drug therapy for fungal infections.

Fungal infections, though affecting healthcare globally, receive insufficient attention in clinical and academic settings. Invasive fungal infections, particularly caused by combat wounds, have been identified as a critical threat by the US Department of Defense. Monotherapy with traditional antifungals is often insufficient, and so combination therapies are explored to enhance treatment efficacy. However, systemic combination treatments can result in severe adverse effects, suggesting the need for localised delivery systems, such as drug-loaded electrospun patches, to administer antifungals d

Amodiaquine-Ciprofloxacin: a potential combination therapy against drug resistant malaria.

Emergence of malaria parasites resistant to artemisinin necessitates the need for development of new antimalarial therapies. Ciprofloxacin (CFX) a second generation quinolone antibiotic possesses some antimalarial activities. We investigated the in vivo antimalarial activities of CFX in combination with amodiaquine in mice infected with chloroquine-resistant Plasmodium berghei ANKA. Animals were treated orally with 80 or 160 mg kg-1 body weight of CFX alone given twice daily or in combination with amodiaquine (AQ) 10 mg kg-1 body weight. Parasitological activity and survival of the animals wer

Targeted phototherapy in combination with drug therapy for segmental vitiligo.

According to the previous reports, segmental vitiligo usually shows a poor response to phototherapy. Here, we report two cases of recent onset segmental vitiligo that showed good or excellent response to targeted phototherapy in combination with drug therapy. These findings suggest that segmental vitiligo can be improved by combination therapy if its onset is recent.

Add-on combination therapy with monoclonal antibodies: Implications for drug development.

Three anti-amyloid monoclonal antibodies (MABs) including aducanumab, lecanemab, and donanemab have been approved by the FDA and lecanemab and donanemab are available in the US market and a variety of other national markets. The increasing use of anti-amyloid MABs to treat early AD will require that development of novel agents occur as add-on treatment to MABs. There is limited experience with add-on therapy to anti-amyloid agents. In most cases, it is prudent to initiate novel agents after at least six-months exposure to the MAB at the highest dose. Agents with extensive data on pharmacokinet

Multistimuli-Responsive Porous Organic Polymers for Drug-Gas Anticancer Combination Therapy.

Combination therapies can significantly aid in the treatment of biologically complex diseases, including cancer. These powerful therapies, such as combination nitric oxide (NO) gas and chemotherapy, are able to simultaneously target different biological targets, thereby overcoming or reducing drug resistance and decreasing dose-related toxicity. However, in nearly all existing carriers, gases and drugs compete for loading in the same physical space of the carrier and can thus interfere with each other. Herein, we use a porous organic polymer (POP) as a design strategy to prepare a combination

Effects of individual drug and combination antiretroviral therapy on trophoblast proliferation.

BACKGROUND: Combination antiretroviral therapy (cART) has been reported to reduce perinatal transmission of human immunodeficiency virus (HIV) and improve maternal survival outcomes. Recent studies have associated in-utero exposure to cART drugs with adverse outcomes such as pre-eclampsia, preterm delivery, low birth weight and small-for-gestational-age births. However, the exact molecular mechanisms underlying cART-induced adverse pregnancy outcomes remain poorly defined. OBJECTIVES: To investigate the effects of cART drugs on trophoblast proliferation in the HTR-8/SVneo cell line. STUDY DE

Existing and investigational combination drug therapy for high-density lipoprotein cholesterol.

For the past 3 to 4 decades, clinical outcomes trials have shown that drugs that favorably alter serum lipid levels reduce the risk of coronary artery disease (CAD) events. However, despite these successes, the reduction in serum low-density lipoprotein (LDL) cholesterol levels with monotherapy lipid-altering drugs does not "cure" CAD to the same degree that antibiotics "cure" many infections, nor do they "prevent" CAD in the same way that childhood immunizations "prevent" the onset of such conditions as measles, mumps, and rubella. Clinical outcome trials of monotherapy lipid-altering drugs h

Optimizing combination therapy against drug resistance Mycobacterium tuberculosis: a modelling study.

Despite the prevalence of co-infection with drug-sensitive and drug-resistant Mycobacterium tuberculosis strains within a single host, the implications of such dual infections remain poorly understood. In this study, we develop a comprehensive within-host model that incorporates both bacterial strains, their mutation dynamics, and cross-reactive immune responses. We analyze the basic reproduction number ( R 0 ) and identify its dependence on key parameters, finding that R 0 is strongly influenced by the adaptive immune response rate, bacterial fitness cost, and macrophage engulfment rates. Lichenoid drug reaction after ipilimumab/nivolumab combination therapy: A case report.

Nivolumab (PD-1 inhibitor) and ipilumumab (CTLA-4 inhibitor) are recently approved checkpoint inhibitors for treatment of non-small cell lung cancer. Immune-related adverse events related to the usage of checkpoint inhibitors are growing with their popularity. We present the case of a patient in combination treatment of nivolumab and ipilimumab who developed a lichenoid drug reaction, notable because it worsened to a bullous lichenoid drug reaction. Treatment with prednisone and withdrawal of checkpoint inhibitors aided in clinical resolution. Initial presentation of a lichenoid reaction that

Hypoxia as a target for drug combination therapy of liver cancer.

Hepatocellular carcinoma (HCC) is the third most frequent cause of cancer deaths worldwide. The standard of care for intermediate HCC is transarterial chemoembolization, which combines tumour embolization with locoregional delivery of the chemotherapeutic doxorubicin. Embolization therapies induce hypoxia, leading to the escape and proliferation of hypoxia-adapted cancer cells. The transcription factor that orchestrates responses to hypoxia is hypoxia-inducible factor 1 (HIF-1). The aim of this work is to show that targeting HIF-1 with combined drug therapy presents an opportunity for improvin

Trigeminal neuralgia: successful antiepileptic drug combination therapy in three refractory cases.

Antiepileptic drug combination therapy remains an empirical second-line treatment approach in trigeminal neuralgia, after treatment with one antiepileptic drug or other nonantiepileptic drugs have failed. The results in three patients followed in our clinic are not sufficient to draw definitive conclusions, but suggest the possibility of developing this type of therapeutic approach further.

[Drug combination therapy for children infected with HIV/AIDS--own experience].

UNLABELLED: Background antiretroviral therapy (HAART) for primary perinatal infection with three drugs is recommended because it provides the best opportunity to preserve immune function and delay disease progression. The aim of HAART is to maximally suppress viral replication, preferably to undetectable levels. We evaluated the safety and efficacy of a three drug regimen in a small group 7 of maternally infected and 1 probably nosocomial infected children from 1998. METHODS: Triple drug combination regimen the most often was composed of: either AZT + 3TC + NFV, ddi + d4T + NFV, or ddI + d4T

Clinical experience with combination disease-modifying antirheumatic drug therapy with cyclosporine.

OBJECTIVES: We previously reported on the clinical use of cyclosporine (Neoral), alone or in combination with methotrexate (MTX), in the first 46 refractory rheumatoid arthritis (RA) patients treated at our centre between March 1996 and November 1997. Thirty of the 46 patients remained on cyclosporine at study completion (mean dose 2.98 mg/kg/day) with efficacy inferred by significant reductions in the prednisolone and MTX doses and creatinine maintained in an acceptable range. Early discontinuation was primarily related to non-serious side effects. METHODS: The 30 patients continuing cyclosp
