Ear trauma.

Understanding basic ear anatomy and function allows an examiner to quickly and accurately identify at-risk structures in patients with head and ear trauma. External ear trauma (ie, hematoma or laceration) should be promptly treated with appropriate injury-specific techniques. Tympanic membrane injuries have multiple mechanisms and can often be conservatively treated. Temporal bone fractures are a common cause of ear trauma and can be life threatening. Facial nerve injuries and hearing loss can occur in ear trauma.

Tropical ear.

Tropical ear is a commonly used term for otitis externa, which is an inflammatory condition of the external auditory canal. It is the most commonly seen cause of otalgia. This article reviews the diagnosis, management and classification of otitis externa, a condition that has a number of causes.

Ear replantation.

The authors' tactical and technical contribution to this type of microsurgical replantation can be summarized in three main points: (1) direct sutures to the superficial temporal vessels, when these are undamaged, which simplifies the operation and makes it safer; (2) use of Kunlin's technique to make up for the great difference in diameter between the arterial vessels; and (3) deepithelialization of the posterior auricular skin to allow for creation of venous drainage channels during the first postoperative days and the enlargement of the area of contact with the recipient bed, which also hel

Ear Abnormalities.

Congenital ear abnormalities present an aesthetic and psychosocial concern for pediatric patients and their parents. Diagnosis of external ear deformities is based on clinical examination and is facilitated by an understanding of normal ear anatomy. Ear anomalies can be categorized as malformations or deformations. Malformations are characterized by absent anatomical structures of the ear (or absence of the ear itself), as exemplified by microtia and anotia. Ear deformations are characterized by ear anatomical landmarks that are present but are distorted or abnormal, with Stahl ear, constricte…

Ear reduction.

Four cases of ear reduction for congenital macrotia and ear asymmetry are presented. To minimize the visible scarring the technique of helical advancement was used. The indications for this uncommon procedure are discussed with a review of the literature. This simple technique has been effective in achieving the desired reduction, leaving the scar hidden in the eaves of the helix, and we endorse its wider use.

Ear psoriasis.

Psoriasis of the ears occurs in approximately 18 percent of all patients at some time. Prevention by avoidance of picking and scratching and careful cleansing are helpful. Treatment with intralesional injection of triamcinolone acetonide is effective in patients resistant to other therapies.

Ear infections.

Infections of the ear are among the most frequent causes for patients to seek care in the ED. Although most infections are fairly simple to diagnose and treat, the anatomic complexity of the ear, its shared innervation with other body structures, its proximity to other body systems, and the risk of serious complications necessitate an orderly approach to the care of these problems. This article outlines an approach to the history, physical examination, diagnosis, and management of these infections. Adherence to such an approach should maximize the likelihood of attaining relief and obliteratio…

Endoscopic Ear Surgery: Redefining Middle Ear Anatomy and Physiology.

Middle ear anatomy and physiology is highly complex, yet familiarity is important to perform middle ear surgery and understand surgically relevant ventilation pathways of the ear compartments. The middle ear is divided into five subspaces: the mesotympanum, the retrotympanum posteriorly, the epitympanum superiorly, the protympanum anteriorly, and the hypotympanum inferiorly. The Eustachian tube plays a crucial role in maintaining middle ear aeration and atmospheric pressure. There are two independent aeration routes of the epitympanum. Thanks to the advent of the endoscope, this anatomic and p…

Ear Scaffold Reconstruction Using Ultrasonic Aspirator for Cauliflower Ear.

Untreated auricular hematomas from ear trauma can result in an ear deformation known as cauliflower ear, secondary to fibrosis and new cartilage overgrowth. Cauliflower ear reconstruction has traditionally utilized tools such as a drill or a scalpel in order to improve auricular cosmesis. We present a case report utilizing an ultrasonic aspirator to recontour the fibrosed cartilage of a cauliflower ear. The ultrasonic aspirator has advantages over traditional tools in its ability to provide finely controlled bone removal without damage to surrounding soft tissue. The patient in this case repor…

Totally avulsed ear: new technique of immediate ear reconstruction.

INTRODUCTION: Reconstruction of the avulsed auricle remains a challenge to the plastic surgeon. Many surgeons suggest reattachment of the total or subtotal amputated ear as a composite graft, but this solution is unreliable. Some surgeons advocate the use of the retro auricular pocket to enhance survival of the reattached ear. However, this burying method entails multistage procedures with somewhat poor results due to the associated fibrosis. Microsurgical replantation can be performed but its applicability is limited due to difficulty with venous drainage. In this paper we advocate the use of…

Ear reconstruction after auricular chondritis secondary to ear piercing.

The recent fad of high ear piercing in the pinna has led to an increased incidence of auricular chondritis, which leads to dissolution of the cartilage and residual ear deformity. The typical postpiercing chondritis deformity presents as a structural collapse of the superior helical rim, scaphal cartilage, and the adjacent antihelix. The skin envelope is usually preserved, but it may be scarred from the infectious process and from previous drainage incisions. In the present article, the authors present a systematic approach to reconstruction of these acquired ear deformities. Careful assessmen…
