[Arthroscopic arthrolysis of the elbow joint].

OBJECTIVE: Arthroscopic assisted improvement of range of motion in elbow stiffness. Detailed diagnostic evaluation including medical history and preoperative radiographs, CT and MRI scans are necessary for planning the operative treatment. INDICATIONS: Restricted range of motion < 30° in extension and/or more than 100° in flexion related to intraarticular causes (loose bodies, osteophytes or contracture of the capsule). CONTRAINDICATIONS: Extension deficit > 30°, extraarticular causes (e.g., heterotopic ossifications), nerve irritation, incongruity of joint surfaces, acute joint infection.

Tuberculous synovitis of the elbow joint.

Tuberculous synovitis in the elbow joint is extremely rare in developed countries. We describe a 68-year-old man who had had a gradually enlarging mass over the volar side of the left proximal forearm near the elbow joint for 4 months. Plain roentgenograms of the diseased elbow showed early osteoarthritic change. Magnetic resonance imaging revealed diffuse synovitis with a large 8 x 8 cm extra-articular synovial cyst. Synovectomy was performed and histopathologic examination of the surgical specimen revealed granulomatous inflammation with caseation, prominent Langhan's giant cells, and sparse

Tuberculous tenosynovitis in the elbow joint.

A 74-year-old woman was noted to have a mass lesion near the right elbow joint during medication for pulmonary tuberculosis. After discontinuation of medication, the mass gradually became enlarged with swelling and tenderness of the joint. Radiological evaluation disclosed tenosynovitis with an encapsulated abscess. Microscopic examination and culture of an aspiration biopsy specimen from the abscess showed no microorganisms. However, DNA extracted from the specimen contained mycobacterium tuberculosis DNA, permitting a diagnosis of tuberculous tenosynovitis. Mycobacterium is not always detect

[Complex injury of the elbow joint].

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as

[Arthroscopy of the wrist and elbow joint].

Arthroscopy of the wrist and elbow is a helpful diagnostic procedure in patients with chronic disability not demonstrated by clinical and radiological examinations. At the wrist, especially triangular fibrocartilage, chondral or ligamentous defects can often not be demonstrated by other tests. Arthroscopy of the wrist allows examination of more than a dozen separate articular surfaces and the function of eight discrete joints. The geometry of the elbow joint limits intra-articular inspection and the use of instruments. The risk of neurovascular injuries is great and depends on exact puncture s

[CT diagnosis of the elbow joint].

The normal anatomy of the elbow joint is described. Diverse pathologic joint alterations, e.g. fractures and dislocations, osteochondritis, inflammatory changes, and ulnar nerve lesion are demonstrated. Particular attention is paid to three-dimensional (3D-) reconstruction of demonstrable pathologic findings.

[Open fractures of the elbow joint].

Both in open and in closed elbow fractures, anatomical joint reconstruction and rigid internal fixation, which permit early initiation of motion, are essential for good joint function. However, in open distal intraarticular humerus fractures this is more difficult to achieve, owing to the high rate of comminuted types C 2 and C 3 fractures. In addition, the extensive soft tissue lesions often mean that physiotherapy cannot be started until later than would otherwise be desirable. This retrospective study shows that the functional results in 60 patients operated on for correction of open distal

[Computerized tomography of the elbow joint].

An essential pre-condition for the interpretation of computer tomographs of the elbow joint is a knowledge of the normal anatomy. The complexity of the joint requires a definite protocol for the examination, since only in this way is it possible to distinguish all anatomical structures and pathological processes. A clinical example is used to show that CT of the elbow joint is superior to all other radiographic methods for demonstrating osteochondritis dissecans. It is therefore an important additional method for demonstrating the elbow joint.

Syndromic craniosynostosis with elbow joint contracture.

This paper reports a new type of syndromic craniosynostosis that was diagnosed by DNA analysis of the patient's fibroblast growth factor receptor (FGFR) genes. At birth, a male infant had ocular proptosis, a pseudotail, and obstructed respiration. He developed craniosynostosis, craniofacial dysmorphism, hydrocephalus, and bilateral contracture of his elbow joints. His treatment included fronto-orbital advancements and a ventriculoperitoneal shunt. Genetic analysis revealed that he was heterozygous for a missense mutation in exon 9 of the FGFR2 gene that resulted in an amino acid substitution o

[Elbow joint dislocation. Diagnosis, therapy, results].

Thorough knowledge of the anatomy and biomechanics in the elbow joint together with an special reference to constitutional variations is an important factor in understanding the pathomechanisms of its dislocation. We are presenting 27 case-reports of elbow-dislocations treated at the I. University Clinic for Traumatology in Vienna between 1979 and 1983. The so-called carrying angle showed no pathological deviation besides a tendency to cubitus valgus and hence elbow-joint to dislocation. Our results showed clearly that conservative treatment is possible and advisable in most cases. Only disloc

[Exploring the mechanism of children elbow joint injury led to elbow varus].

OBJECTIVE: To explore the causes and mechanisms of children elbow joint injury led to elbow varus. METHODS: One hundred children with elbow varus causded by elbow joint injuries were retrospectively studied during January 2005 to April 2008 in department of child orthopedics involving 54 males and 46 females with an average age of 9.7 years old ranging from 7 to 16 years. The anterior-posterior and lateral position X-ray films of elbow joint continuous 12 to 18 months (means 15 months) of all the cases were collected. The postoperative X-ray films were observed to analyze the cause of elbow v

A muscle sparing posterior approach to the elbow joint for total elbow arthroplasty.

We describe a muscle sparing approach in which the triceps is elevated without injuring the muscle or disturbing its insertion. The entire extensor mechanism is preserved in continuity, thus preventing any extensor weakness. This can be used preferentially in cases of non-union intraarticular distal humerus fractures planned for Total Elbow Arthroplasty.

[Winter sports injuries to the elbow joint].

BACKGROUND: Elbow injuries occur quite often when practicing winter sports. Due to the high sports dynamics and risk of fall the elbow joint is prone to injuries. Hereby, the most common injury is the dislocation of the elbow. TREATMENT: Isolated ligamentous instabilities can usually be treated non-operatively when the joint is centered on post-reduction imaging. In the case of persistent symptomatic instability, secondary ligament reconstruction surgery is associated with reliable and good results. Most fractures of the elbow involve the radial head in terms of postero-lateral rotation insta

Solutions for the Unstable and Arthritic Elbow Joint.

An unstable, arthritic elbow presents a therapeutic challenge. Patients may have painful, limited range of motion, often due to trauma or progressive joint destruction from rheumatologic disease. The options for management may be particularly challenging when treating young, active patients. While elbow arthroplasty usually provides predictable pain relief and joint range of motion, concerns exist regarding postoperative activity limitations and implant survival. Therefore, these procedures are limited to select subsets of patients, typically low-demand, elderly patients. Interposition arthrop
