Transendoscopic electrosurgery for epiglottal entrapment in the horse.

Monopolar electrosurgical cutting was used to correct epiglottal entrapment in 5 horses. The operations were carried out in the conscious animal, using topical anesthesia. The procedure required the use of a coagulation electrode designed specifically for electrosurgery, introduced through the instrument channel of a fiberoptic endoscope. The results were satisfactory and serious complications were not encountered. In 2 horses, excessive submucosal swelling developed at the site of the surgical wound, and the tumefaction took several weeks to subside in one of these horses. In both horses, the…

Scalpel versus electrosurgery for major abdominal incisions.

BACKGROUND: Scalpels or electrosurgery can be used to make abdominal incisions. The potential benefits of electrosurgery may include reduced blood loss, dry and rapid separation of tissue, and reduced risk of cutting injury to surgeons. Postsurgery risks possibly associated with electrosurgery may include poor wound healing and complications such as surgical site infection. OBJECTIVES: To assess the effects of electrosurgery compared with scalpel for major abdominal incisions. SEARCH METHODS: The first version of this review included studies published up to February 2012. In October 2016, fo…

Transcatheter Electrosurgery: JACC State-of-the-Art Review.

Transcatheter electrosurgery refers to a family of procedures using radiofrequency energy to vaporize and traverse or lacerate tissue despite flowing blood. The authors review theory, simulations, and benchtop demonstrations of how guidewires, insulation, adjunctive catheters, and dielectric medium interact. For tissue traversal, all but the tip of traversing guidewires is insulated to concentrate current. For leaflet laceration, the "Flying V" configuration concentrates current at the inner lacerating surface of a kinked guidewire. Flooding the field with non-ionic dextrose eliminates alterna…

Insulation failure in electrosurgery instrumentation: a prospective evaluation.

BACKGROUND: The use of electrosurgery has expanded to a wide variety of surgical specialities, but it has also been accompanied by its share of complications, including thermal injuries to nontargeted tissues, caused by a break or defect in the insulation of the instrument's coat. The purpose of this study was to determine the prevalence and the location of insulation failures (IFs) in electrosurgical instruments, then to assess the necessity of routine IF testing. METHODS: Electrosurgical instruments were visually inspected and checked for IF using a high-voltage detector. Two different dete…

Cutaneous electrosurgery in a patient with a deep brain stimulator.

BACKGROUND: Deep brain stimulators are implantable devices with electrical activity used to treat certain movement disorders such as essential tremor and Parkinson's disease. Similar to implantable cardiac devices, use of electrosurgery on patients with these devices may produce adverse effects. CASE REPORT: We describe the effects of electrosurgery on a patient with essential tremor and an implantable deep brain stimulator who required Mohs micrographic surgery to excise a basal cell carcinoma. The patient experienced immediate lancinating "electrical shock" using electrosurgery in the monop…

Comparison of CO2 laser and electrosurgery in the treatment of rhinophyma.

A number of surgical modalities, including the CO2 laser, have been advocated for the treatment of rhinophyma. The CO2 laser has been extensively promoted for the management of many skin lesions, including warts, keloids, and various nail problems. In many instances it has been suggested that the CO2 laser holds a distinct advantage over other surgical modalities. Three patients with rhinophyma are presented. Results are compared after treating one half of the nose with biterminal electrocutting current and the other half with the beam of the CO2 laser.

Percutaneous Excision of Left Atrial Myxoma Using Transcatheter Electrosurgery.

Left atrial myxoma is the most prevalent primary cardiac tumor, known for its high risk of systemic embolization. Although surgical excision remains the standard treatment, options are limited for high-risk patients. This case report introduces a novel approach using transcatheter electrosurgery to address a left atrial myxoma via a transseptal approach in a patient ineligible for conventional surgery due to the heightened risk of intracranial hemorrhage associated with cerebral amyloid angiopathy. Despite encountering technical challenges, this innovative procedure represents a pioneering app…

A Confidence-Based Shared Control Strategy for Robotic Electrosurgery.

Robotic-assisted surgery (RAS) systems take advantage of dexterous tools, enhanced vision, and motion filtering to improve patient outcomes. Whereas most RAS systems are directly controlled by surgeons, the development and application of autonomous RAS are growing owing to their repeatability and precision. Although full autonomy is a long-term goal, human intervention in RAS is still essential. In this work, we develop and test a shared control strategy for robotic electrosurgery in which autonomous robot controllers and human operators collaborate. We designed and implemented identification …
