Pulmonary embolism (PE) is a common problem for which prompt diagnosis and treatment is essential to minimize mortality. The clinical presentation is more variable than sudden dyspnea and chest pain, especially in the critical care patient. Recognition of venous thromboembolic (VTE) risk factors can help develop a good clinical suspicion for PE. A wide range of diagnostic tests are available to the clinician. The ventilation/perfusion scan, pulmonary arteriogram, and lower extremity investigations are still important for diagnosis. Other noninvasive tests such as spiral CT with venography, ech…

The natural history of pulmonary embolism (PE) is incompletely characterized, because most episodes of PE go undetected, the clinical presentation mimics so many other common and uncommon diseases, the sensitivity and specificity of the diagnostic tests are poorly defined, and even detection at autopsy is difficult and requires close examination of the pulmonary arteries. Yet PE is a significant cause of morbidity and mortality in the hospitalized patient, and one reason for its extremely high incidence is the failure of physicians to provide adequate prophylaxis to patients who are at risk of…

Bullet embolism is a rare complication of vascular trauma. During the last ten years we have treated six patients with bullet embolism. Three patients had inferior vena caval injuries with embolizations of the bullets to the heart or pulmonary arteries. Two had infrarenal aortic injuries with embolization to vessels of the lower extremities. One patient with a shotgun injury to the superficial femoral artery and vein had both arterial and venous embolizations. The site of vascular penetration was repaired in all six patients. All peripheral arterial bullet emboli were removed except for an asy…

Pulmonary embolism is a commonly seen emergency that presents many diagnostic and therapeutic challenges. In order to meet these challenges, clinicians must have an organized approach to work-up and management of this emergency.

Patients who are bedridden because of debilitating illness and patients who are recovering from major surgery are at particularly high risk of deep venous thrombosis and subsequent pulmonary embolism. "Mini-dose" heparin therapy has proved useful in preventing deep venous thrombosis. Because the clinical signs of pulmonary embolism are nonspecific, the patient's condition may deteriorate before the diagnosis is suspected. Ventilation and perfusion scans or pulmonary angiography confirm the diagnosis. Heparin continues to be the mainstay of therapy for pulmonary embolism.

Paradoxical embolization of venous thrombus into the abdominal aortic bifurcation has been diagnosed by means of aortography. Lower limb phlebothrombosis accounting for the embolus formation has been demonstrated by phlebography. Arterial embolectomy and venous thrombectomy were successfully performed. Recovery was complete without sequelae.
