Post-vaccinial encephalomyelitis - Probable acute disseminated encephalomyelitis (ADEM) -: A case report.

Acute disseminated encephalomyelitis (ADEM) is an inflammatory demyelinating disease that typically occurs following a viral infection or vaccination. The incidence of ADEM following vaccination has fallen since the introduction of non-neural rabies vaccine and very few cases have been reported due to pure chick embryo derived rabies vaccine (PCERV). Here we are reporting a rare case of probable post vaccinial ADEM.

Acute disseminated encephalomyelitis.

The advent of magnetic resonance imaging (MRI) has contributed to increase the interest and awareness in childhood white matter disorders. Pediatric inflammatory demyelinating diseases of the central nervous system (CNS) are clinically heterogeneous with respect to their mode of presentation, clinical severity, rate of progression, and prognosis. Acute disseminated encephalomyelitis (ADEM) is an immune-mediated inflammatory disorder of the CNS, typically transitory and self-limiting. The highest incidence of ADEM is observed during childhood. It is characterized by an acute encephalopathy with…

Acute disseminated encephalomyelitis.

Acute disseminated encephalomyelitis is an immune-mediated inflammatory and demyelinating disorder of the central nervous system, commonly preceded by an infection. It principally involves the white matter tracts of the cerebral hemispheres, brainstem, optic nerves, and spinal cord. Acute disseminated encephalomyelitis mainly affects children. Clinically, patients present with multifocal neurologic abnormalities reflecting the widespread involvement in central nervous system. Cerebrospinal fluid may be normal or may show a mild pleocytosis with or without elevated protein levels. Magnetic reso…

Acute disseminated encephalomyelitis.

Acute disseminated encephalomyelitis (ADEM) is an immune-mediated inflammatory disorder of the CNS characterized by a widespread demyelination that predominantly involves the white matter of the brain and spinal cord. The condition is usually precipitated by a viral infection or vaccination. The presenting features include an acute encephalopathy with multifocal neurologic signs and deficits. Children are preferentially affected. In the absence of specific biologic markers, the diagnosis of ADEM is still based on the clinical and radiologic features. Although ADEM usually has a monophasic cour…

Acute disseminated encephalomyelitis.

Acute disseminated encephalomyelitis (ADEM) is a monophasic inflammatory disorder of the central nervous system (CNS). Unlike viral encephalitis, microorganisms do not invade the CNS. Instead, ADEM is a postinfectious disease mediated by auto-reactive cells or molecules. Clinical characteristics of ADEM are consistent with disseminated involvement of the CNS, including encephalopathy and pyramidal, cerebellar, and brainstem signs. Bilateral optic neuritis and transverse myelitis are particularly suggestive of demyelinating diseases such as ADEM. Unlike viral encephalitis, seizures rarely are a…

Venezuelan equine encephalomyelitis.

The emergence of epidemic VEE viruses has been reported ever since the virus was first described; this phenomenon is likely to continue to occur because of the high mutation rate of these RNA viruses. A vaccine that was first developed by the US Military for human use has proved helpful in curtailing the spread of VEE virus during epizootics of the disease in equids but not during human epidemics. It has not, however, eliminated the source of these highly pathogenic and transmissible viruses. Occurrences of VEE in equids in Mexico in recent years suggest that the present vaccine is not effecti

[Acute disseminated encephalomyelitis].

Acute disseminated encephalomyelitis (ADEM) occurs late in the course of viral infection, mainly exanthematous diseases such as measles, chicken pox, and rubella. Postinfectious encephalomyelitis is characterized by immune-mediated demyelination, which is found in experimental autoimmune encephalomyelitis (EAE), and virus cannot be isolated from the central nervous system. The investigations using animal models infected with. Theiler's virus or measles virus would be very useful for clarifying the mechanisms of demyelination induced by viral infection. Although the incidence of postvaccinal en…

Acute disseminated encephalomyelitis, multiphasic disseminated encephalomyelitis and multiple sclerosis in children.

Forty-eight children with disseminated demyelination of the CNS, 28 with acute disseminated encephalomyelitis (ADEM), seven with multiphasic disseminated encephalomyelitis (MDEM) and 13 with multiple sclerosis were studied for a mean follow-up period of 5.64 years. The presentation findings of the ADEM/MDEM group were compared with those of the multiple sclerosis group. The following findings were more commonly seen in ADEM/MDEM presentation compared with the multiple sclerosis presentations: predemyelinating infectious disease (74 versus 38%, P: < 0.05); polysymptomatic presentation (91 versu…

THE HISTOLOGY OF EQUINE ENCEPHALOMYELITIS.

The virus of equine encephalomyelitis (eastern strain) evokes in the horse, calf, sheep and dog an unusually intense encephalomyelitis characterized by acute primary degeneration of nerve cells, the appearance in neurons of the brain stem and elsewhere of nuclear inclusions resembling those in Borna disease and poliomyelitis, polymorphonuclear infiltration in the nervous tissues with early microglial proliferation, and perivascular cuffing with mononuclears and polymorphonuclears in varying proportions. The grey matter is affected more than the white. Lesions may be less marked in the striatum…

Postdengue Acute Disseminated Encephalomyelitis.

A 38-year-old gentleman, following an uncomplicated dengue fever 2 weeks back, developed acute onset bilateral lower limb weakness and numbness for 5 days, associated with bladder and bowel incontinence and a band-like sensation in T4 dermatome. On examination, he had paraparesis with normal cranial nerves except for left upper motor neuron-type 7th cranial nerve palsy and normal higher mental function. Magnetic resonance imaging (MRI) of the brain and spine detected multiple demyelinating lesions. A diagnosis of postdengue acute disseminated encephalomyelitis (ADEM) was made as part of postin…

Echovirus 7 associated encephalomyelitis.

BACKGROUND: Hand, foot, and mouth disease (HFMD) is endemic in Malaysia. In 1997, a large outbreak of enterovirus 71 (EV-71) associated HFMD … two of the patients. RESULTS: Echovirus 7 was the sole pathogen isolated from three cases of acute encephalomyelitis, two of which were fatal due to severe left ventricular dysfunction resistant to inotropic support. The survivor had residual bulbar palsy, but is considered to have had a good neurological outcome. CONCLUSION: Echovirus 7 infection associated with …

Disseminated encephalomyelitis in adults.

Disseminated encephalomyelitis (DEM) is an inflammatory demyelinating disease that is common in children, but also appears in adults. It is often misdiagnosed as multiple sclerosis (MS) from which it differs in its clinical presentation, course of disease and prognosis. Some aspects of DEM overlap with neuromyelitis optica (NMO), another demyelination disease of CNS that was for a long time regarded as part of the MS spectrum, until discovery of the aquaporin-4 antibodies, claimed to be specific for NMO. The clinical symptoms of both may be similar, and their clinical courses may be monophasic…

Encephalomyelitis in primary hypogammaglobulinaemia.

The neurological features of 13 patients with primary hypogammaglobulinaemia are described. Seven patients had X-linked agammaglobulinaemia (XLA) and six had common variable immunodeficiency (CVID). Three clinical pictures emerged: (i) a progressive myelopathy (one case); (ii) a myelopathy progressing to an encephalopathy (four cases); (iii) a pure encephalopathy (eight cases). In four patients the encephalopathy was temporarily reversible; the relationship of this to immunoglobulin therapy is unclear. Additional features occurred in some patients. Three had retinopathy interpreted as retiniti

Postinfectious and postvaccinial encephalomyelitis.

Addressed are the features of postviral and postvaccinial syndromes affecting the CNS. The relationships among acute disseminated encephalomyelitis, EAE, and MS are discussed. Among the postvaccination syndromes covered are the neurologic sequelae of pertussis immunizations, with a description of a mouse model of this encephalopathy.

[Diagnosis of avian encephalomyelitis].

The diagnostic value of laboratory examination methods was assessed in the diagnosis of avian encephalomyelitis in seven to twenty-two days old chickens. Histological examination and immunofluorescent test are expedient methods meeting the requirements for timely application of infection-control measures. The agreement of histological examination with the over-all laboratory examination is 70 to 81% and the agreement of the immunofluorescence test is 76%. Bioassay on chickens followed by a histological examination appears to be the best method, providing results with the highest significance. …
