[Endovascular management of chronic limb ischemia. Experience in 48 procedures].

BACKGROUND: Chronic limb ischemia can lead to high rates of limb loss and mortality. Open surgery is the gold standard for treatment of distal disease. Endovascular surgery should have less complications with similar outcomes. AIM: To report a cohort of patients with distal arterial disease treated with endovascular surgery at our institution. MATERIAL AND METHODS: Review of angioplasty records of patients undergoing distal lower extremity endovascular procedures between 2016 and 2019. Demographics, comorbidities, form of presentation, type of intervention, perioperative complications, and l…

Outcomes of secondary procedures after primary thoracic endovascular aortic repair†.

OBJECTIVES: The purpose of this study is to retrospectively evaluate, with an 'all-comers' approach, the survival and outcome of patients following secondary surgical or interventional procedures after thoracic endovascular aortic repair (TEVAR). METHODS: Between October 2002 and December 2013, 371 patients with different aortic pathologies underwent primary TEVAR at our institution. Fifty-six out of the 371 patients (15.1%, 18 females, mean age 62.3 ± 13.7 years) required secondary procedures, either interventionally (N = 31; 55.4%) or surgically (N = 25; 44.6%), due to stent graft-related c

Surgical site infection rates in dialysis patients undergoing endovascular procedures.

A surgical site infection (SSI) is an infection related to surgery that develops within 30 days after an operation or within 1 year of impla … alysis indicated a less than 3% superficial incisional SSI rate in hemodialysis patients undergoing endovascular procedures. The SSI rate for clean wound procedures is generally 2% or less. These data indicate that dialysis patients undergoing interventional procedures in vascular access centers may have a slightly greater risk of developing SSIs due to the presence of a …

Surgical exposure of the carotid artery for endovascular interventional procedures.

BACKGROUND: Transfemoral approach for endovascular interventions is not always possible in cases of unfavorable anatomy. We report our experience using a transcervical approach with carotid cut down and direct, controlled puncture of the carotid artery. METHODS: Four patients underwent surgical exposure of the carotid artery for endovascular procedures. One patient had retrograde placement of a stent in the common carotid artery, and three patients had coiling of an intracranial aneurysm. After the endovascular procedure, the sheath was removed and the vessel was closed, under direct visualiz…

Cognitive training improves clinically relevant outcomes during simulated endovascular procedures.

OBJECTIVES: Virtual reality (VR) simulation has been suggested to objectively assess endovascular skills. The aim of this study was to determine the impact of cognitive training on technical performance of inexperienced subjects on a commercially available VR simulator (VIST, Vascular Intervention Simulation Trainer, Mentice, Gothenburg, Sweden). METHODS: Forty-seven subjects treated an identical virtual iliac artery stenosis endovascularly. Surgical trainees without endovascular experience were allocated to two training protocols: group A(1) (n = 10) received a 45 minute didactic session fol

Brachial artery access for endovascular procedures: how safe is it?

PURPOSE: While femoral and radial artery access are routine for endovascular procedures, brachial artery access remains an important alternative in selected cases. This study aimed to evaluate the complication rate of percutaneous brachial artery access at a tertiary centre and identify potential risk factors for access-site complications. MATERIALS AND METHODS: A retrospective review was performed of all procedures utilising percutaneous brachial artery access over a 12.5-year period. Patient demographics, sheath size, closure technique and complications were recorded. Complications were cla…

Endovascular repair of aortic aneurysms: critical events and adjunctive procedures.

OBJECTIVE: We sought to define the learning curve relative to the incidence and range of intraoperative problems and to establish guidelines for troubleshooting during the endovascular repair of infrarenal aortic aneurysms. METHODS: We prospectively evaluated our first 75 consecutive cases over a 12-month period and focused on perioperative critical events and adjunctive procedures as categorical outcome measures collected during the operation. Patients were separated into three groups on the basis of the date of their operation, such that group 1 consisted of our first 25 cases, group 2 our

Iliac Endovascular Intervention Without Procedural Anticoagulation.

BACKGROUND: Adequate antithrombotic therapy is essential to prevent thrombus formation during percutaneous endovascular interventions (PVI). We hypothesize that interventions for non-complex lesions of iliac arteries do not need procedural anticoagulation if patients are under dual antiplatelet therapy (DAPT). METHODS: Iliac PVIs performed without procedural anticoagulation were retrospectively screened between 2017 and 2021. Baseline characteristics of patients, in-hospital events and 30-day follow-ups were obtained from hospital records. Each PVI was reviewed for procedural details. Primary

[Successful repeated endovascular procedure for TIPS thrombosis].

Transjugular intrahepatic portosystemic shunt (TIPS) is a minimally invasive and effective treatment for gastrointestinal variceal bleeding and refractory ascites associated with portal hypertension. However, TIPS dysfunction occurs for various reasons and can lead to recurrent complications. In such cases, repeated endovascular intervention can be performed for portal hypertension. This article presents one of the methods of endovascular correction (Y-stenting) for TIPS thrombosis.

Carbon Dioxide Angiography as a Sole Contrast Modality in Endovascular Procedures.

BACKGROUND: Iodinated contrast medium (ICM) carries risks of allergic reaction and post contrast acute kidney injury. Carbon dioxide (CO2) angiography offers a potential alternative for patients with renal insufficiency or ICM allergy. Although extensive international studies have demonstrated the efficacy of CO2 angiography, data on local populations remain limited. To provide region-specific data, we evaluated the accuracy and safety of CO2 as the sole contrast modality in regional endovascular procedures. METHODS: Clinical data of 28 patients who underwent various endovascular procedures u…

Regional and specialty-based Medicare reimbursement trends in arterial endovascular procedures.

OBJECTIVE: Medicare Part B reimbursements have been overall declining at the national level. Our goal was to characterize Medicare reimburse … d decreases of -29%, -28%, and -20% (P = .51), respectively. CONCLUSIONS: Our analysis of arterial endovascular procedures billed to Medicare Part B from 2018 to 2022 shows a continued decline in reimbursements regardless of physician specialty. However, regional patterns did reflect disproportionate changes in reimbursements, with 6 (South) and 9 (West Coast) having an …

Acute Renal Failure/Acute Kidney Injury (AKI) Associated with Endovascular Procedures.

AKI is one of the most common underdiagnosed postoperative complications that can occur after any type of surgery. Contrast-induced nephropa … pen or endovascular-perioperative AKI is associated with significant morbidity, mortality and cost. Endovascular procedures always require administration of a contrast media, which may cause acute tubular necrosis or renal vascular embolization leading to renal ischemia and as a consequence, contribute to increased number of post-operative AKIs.

Management of ambulatory (day case) endovascular procedures for peripheral arterial disease.

INTRODUCTION: Thanks to exceptional improvements in technological developments and vascular teams' expertise, endovascular repair of peripheral arterial disease (PAD) has drastically increased these past decades. Incidence of PAD rising, pressure to reduce costs and patient's empowerment make outpatient endovascular procedures for PAD an attractive alternative to conventional hospitalization. This review describes outpatient endovascular procedures for PAD in the literature. EVIDENCE ACQUISITION: We used PRISMA guidelines and Medline to conduct this systematic review. 448 relevant articles we…

Should We be Concerned About the Inflammatory Response to Endovascular Procedures?

Endovascular surgery represents a minimally invasive procedure for the treatment of occlusive and aneurysmal arterial disease. However, it i … ns. Future studies should consider the pathophysiology of the inflammatory response associated with endovascular procedures as well as predictors and risk factors including preventive strategies and therapeutic algorithms. Although the potential role of anti-inflammatory drugs after endovascular procedures has been observed, it needs to be validated in upcoming trials. T …
