There are few reports documenting the presence of melanin or melanocytes in epidermal cysts. One hundred and twenty five epidermal cysts from Indian patients were analyzed for the presence of melanin pigment and their sites were noted. A Masson's Fontana stain and immunohistochemistry for S-100 protein and HMB 45 was performed for localization of melanin and melanocytes within the epidermal cysts. While proliferating trichilemmal cysts are a well-known entity, so far there have been no reports on the proliferating processes of epidermal cysts. Two patients with multiple proliferating cysts are presented. Histologically, there was a well-developed stratum granulosum in the wall of the most cysts, and the cavity contained orthokeratotic horny material. The possible pathogenetic mechanisms of cyst formation are discussed.

Epidermoid cysts are rare benign tumors that are derived from the development of abnormally situated ectodermal tissue and are often an incidental finding. They are usually diagnosed between 15 and 50 years of age, with both sexes equally affected. In epidermoid cyst management, complete excision is the therapy of choice. The authors reported a case of a 24-year-old man with an epidermoid cyst located on the left side of the face, on the region of mandibular body, which was treated by complete surgical excision. The patient has been followed up for 2 years without signs of recurrence.
