BACKGROUND: Papillary cystadenoma is a rare benign neoplasm of the epididymis. It may occur sporadically or in association with von Hippel-Lindau disease (VHLD). Papillary cystadenoma of the epididymis (PCE) is a benign mimic of metastatic clear cell renal cell carcinoma (CCRCC) given their histologic similarities.

Primary leiomyosarcomas rarely arise from epididymis. But they are the most common histopathological types of sarcoma arising from the epididymis. Primary epididymal leiomyosarcoma occurs usually in older patients. We report a young patient of 35 years presenting with leiomyosarcoma of left epididymis. He did not have any metastasis and underwent left high inguinal orchiectomy. He is on regular follow-up and disease free for last two years.

Papillary cystadenoma is a rare benign neoplasm of the epididymis, occurring mainly in young adult males. More than one-third of the cases reported in the literature have occurred in patients with von Hippel-Lindau disease. Conversely, epididymal nodules presumed to be papillary cystadenomas are found in one-third of males with von Hippel-Lindau disease. The association is stronger for bilateral tumors. Tumors reminiscent of müllerian epithelial tumors of the ovary have been reported outside of the ovary, including in the pancreas and paratesticular region. The latter are most commonly serous tumors of borderline malignant potential and involve the tunicae. We present the first case of a serous cystadenoma of the epididymis in English reports with a brief literature review.

The attainment of this biological role is partly due to the fusogenic properties of the sperm plasma membrane, which is particularly rich in polyunsaturated fatty acids (PUFA). We report a case of blastomycosis presenting as epididymitis and prostatitis. The diagnosis was suggested by pathologic findings in the prostate and epididymis and was further supported by serology. The diagnosis was confirmed by culture and special staining. Long-term cure was accomplished after a 12-month course of oral ketoconazole (400 mg/day). Therapy was monitored by culture and serology. The rare event of a bilateral leiomyoma of the epididymis is illustrated. The tumors were located near the tail of the epididymis in a man aged 58, which furthermore presented hydrocele and had been previously operated because of bilateral inguinal hernia. The differentiation with inflammatory pseudotumor, leiomyosarcoma and adenomatoid leiomyoma is discussed.

A 51-year-old man with known ethyl toxic chronic pancreatitis presented with a tumor of the left epididymis. Immunohistological examination disclosed it as a metastasis of a pancreatic carcinoma. Paratesticular metastases of a pancreatic carcinoma are very rare. Nevertheless a metastasis of a primary pancreatic carcinoma should be included in the differential diagnosis of a tumor of the paratesticular tissue. The epididymis should be integrated in the diagnostic procedures of pancreatic carcinoma.
