AIM: To compare neuropsychological function in juvenile myoclonic epilepsy (JME) and frontal lobe epilepsy (FLE) since frontal circuitry is involved in both conditions. By drawing on previously theory-guided hypotheses and findings, a particular emphasis is placed on the way different cognitive-pathophysiological mechanisms act upon to produce frontal dysfunction in JME (frontal-executive and attention-related problems: vigilance, reaction times, processing speed, and response inhibition) and in FLE (reflecting the coproduct of the functional deficit zone), respectively.
METHODS: A total of 1

The distribution of epilepsy types varies by age, etiology, provider diagnostic capabilities, and assessment criteria. No recent US study has examined the distribution of epilepsy types in a large, population-based sample of people with epilepsy. We used MarketScan data from January 1, 2010 through September 30, 2015, to estimate the proportion of epilepsy types among all (N=370,570) individuals diagnosed with epilepsy. We identified cases of epilepsy as individuals with at least one International Classification of Disease, 9th version (ICD-9) diagnostic code of 345.X and the use of at least o

There is good evidence for a genetic causation of a significant proportion of the human epilepsies. These genetic epilepsies include a large number of rare mendelian syndromes in which epilepsy usually occurs as part of a more complex neurological phenotype, and the more common familial epilepsies in which seizures occur in isolation but inheritance is not mendelian. The molecular basis of these epilepsies is entirely unknown. The current revolution in genetics and neuroscience is, however, providing methods for the molecular genetic analysis of epilepsy. Two principal strategies exist. The fi

This practice guideline from the American Epilepsy Society provides evidence-based recommendations for pharmacological, dietary, and surgical therapies for epilepsy for infants and children from 1 month of age to <36 months. The multidisciplinary panel updated an existing systematic review, which was funded by the Patient-Centered Outcomes Research Institute and conducted by ECRI (formerly the Emergency Care Research Institute) on behalf of the Agency for Healthcare Research and Quality. The updated review used the same search strategy, inclusion/exclusion criteria, and Grading of Recommendati

BACKGROUND AND AIMS: Epilepsy is highly heritable, with numerous known genetic risk loci. However, the genetic predisposition's role in post-acute brain injury epilepsy remains understudied. This study assesses whether a higher genetic predisposition to epilepsy raises post-stroke or Transient Ischemic Attack (TIA) survivor's risk of Post-Stroke Epilepsy (PSE).
METHODS: We conducted a three-stage genetic analysis. First, we identified independent epilepsy-associated ( p <5x10 -8 ) genetic variants from public data. Second, we estimated PSE-specific variant weights in stroke/TIA survivors from

A woman diagnosed with juvenile myoclonic epilepsy for over 30 years presented with stereotyped episodes of abnormal sensations in both arms. Continuous video-EEG monitoring for 14 days and MRI brain revealed that the patient's somatosensory events with associated postictal aphasia, as well as her myoclonic and generalised tonic-clonic seizures, were likely due to a symptomatic left fronto-polar epilepsy. Our case emphasizes the need for clinicians to consider fronto-polar epilepsy as a potential cause of myoclonic seizures, particularly when associated with other semiologic features suggestiv

This targeted review addresses the best accepted and most intriguing recent observations on the complex relationships between sleep and epilepsy. Ten to 15% of all epilepsies are sleep-related. Included in these is sleep-related hypermotor epilepsy, renamed from nocturnal frontal lobe epilepsy by a 2016 consensus conference since 30% of cases are extra-frontal, seizures are related to sleep rather than clock time, and the predominant semiology is hypermotor. Stereo-EEG is providing crucial insights into network activation in sleep-related epilepsies and definition of the epileptogenic zone. Pa

The diagnosis of idiopathic generalized epilepsies (IGEs) is not generally difficult if one follows the clinical and electroencephalogram (EEG) definitions of each subsyndrome that constitutes IGEs. In contrast, symptomatic epilepsies develop based on organic brain lesions and are easily diagnosed by the presence of developmental delay, neurologic abnormalities, and a characteristic seizure and EEG pattern. However, in clinical practice, it is sometimes difficult to differentiate IGEs from symptomatic epilepsies, especially when the clinical course from the onset of epilepsy is too short to ex

Among patients with focal seizures of mesial temporal semiology, approximately 30 % experience persistent seizures after anterior temporal resections. The causes are multiple, including unrecognized extratemporal seizure onset. We report six patients from three tertiary care centres for drug-resistant epilepsy whose clinical semiology was mesial temporal but who were found to have an extratemporal seizure onset confirmed by stereoelectroencephalography (SEEG). Three patients were non-lesional, and three had mesial parietal lesions. In three patients (two lesional, one non-lesional), the conclu

OBJECTIVES: The objective were to evaluate structural alterations in the retina and choroid tissue of epilepsy patients and subtypes using enhanced depth imaging optic coherence tomography (EDI-OCT).
METHODS: 46 epilepsy patients and 50 sex- and age-matched control patients were analyzed in the study. Patients' epilepsy types were recorded. The central macular thickness (CMT), retinal nerve fiber layer (RNFL), ganglion cell layer (GCL), and choroidal thickness (CT) were investigated through the Spectralis-OCT device (SD-OCT). Image-J program was used to calculate the total choroidal area (TCA

PURPOSE OF REVIEW: The classification of seizures, epilepsies, and epilepsy syndromes creates a framework for clinicians, researchers, and patients and their families. This classification has evolved over the years, and in 2017 the International League Against Epilepsy (ILAE) published an operational classification of seizures and epilepsies. Understanding this classification is important in the diagnosis, treatment, and understanding of seizures and epilepsies, including epilepsy incidence.
RECENT FINDINGS: The 2017 ILAE classification system builds on newly formulated definitions of seizure

Seventy-one Chinese children with idiopathic generalized epilepsy, from among 1931 pediatric epileptics, were reviewed clinically and electroencephalographically. They were classified under childhood absence epilepsy (CAE, 50/71), juvenile myoclonic epilepsy (JME, 7/71) and epilepsy with grand mal on awakening (GMA, 14/71). All the patients' family members with epilepsy were classified under idiopathic generalized epilepsy. Hyperventilation was the most common precipitating factor for CAE, and sleep deprivation was the most common one for JME and GMA. Electroencephalographically, fast generali

BACKGROUND: Attributional bias, a tendency to overinterpret others' intentions as hostile (rather than situational or accidental), represents a component of social cognition and may affect everyday functioning. Neural models link attributional processing to fronto-temporal circuits and the default mode network, which are frequently altered in epilepsy. Difficulties in social participation and employment are common in people with epilepsy, and maladaptive attributional styles may contribute to these challenges. Attributional bias has not been systematically compared across epilepsy syndromes.

We report a case of graphogenic epilepsy as a variant of language-induced epilepsy. A 25-year-old, right-handed woman had noticed for the previous 10 years that writing almost always provoked jerks or a jerking sensation in her right hand. No other triggers, including facets of language function such as reading and speaking, elicited any epileptic seizures. Ictal EEG demonstrated an initial left central abnormality. In our report, we stress that graphogenic seizures as an expression of language-induced epilepsy should not be confused with those that are manifestations of juvenile myoclonic epi

The notion of symptomatic epilepsy was historically reviewed. The structural etiologies of epilepsy include cerebrovascular diseases, neurodegenerative diseases, neoplasms, and traumatic brain injuries. Infections are the most common etiology worldwide. Metabolic epilepsy directly results from a known or presumed metabolic disorder. Immune epilepsy occurs because of autoimmune-mediated central nervous system inflammation.

Reflex seizures (RS) are epileptic events that are objectively and consistently elicited in response to a specific afferent stimulus or by an activity of the patient. The specific stimulus can be a variety of heterogenous intrinsic or extrinsic factors, ranging from the simple to the complex, such as flashing lights or reading a book. These seizures can take a variety of forms, comprising either general or focal onset, with or without secondary generalization. Reflex epilepsies (RE) are classified as a specific syndrome in which all epileptic seizures are precipitated by sensory stimuli. The f
